There have been only a few cases of desmoplastic fibroma of the spine in the literature and only one of them was purely located on the cervical spine. We report a new patient with the diagnosis of desmoplastic fibroma of the fourth cervical spine. The patient had the complaints of left arm and neck pain. After his radiological evaluation, a mass lesion was found on the left lamina of the fourth cervical spine. Surgical treatment was performed, and the histopathological examination revealed the diagnosis of desmoplastic fibroma. Patients with desmoplastic fibroma of the cervical spine may prese…

OBJECT: Thoracic synovial cysts (TSCs) are rare and are usually the subject of case reports. The authors studied the clinical manifestations, radiological aspects, and surgical treatment in a series of patients at their institution who harbored TSCs. They also review the literature to discuss the potential factors involved in the pathogenesis of this lesion.
METHODS: A database search of 16,000 patients who underwent decompressive spine surgery at the Mayo Clinic (Rochester, MN) between 1976 and 2003 disclosed nine patients (0.06%) in whom a diagnosis of TSC had been made. All patients were m…

The role of MRA, as an adjunct to conventional MR imaging of the spine and spinal cord, is evolving. The older MRA methods that have been applied to spinal vascular imaging include 2D and 3D phase contrast techniques and a derivative of 3D time-of-flight techniques with data acquired for about several minutes after gadolinium contrast injection (standard 3D CE MRA). Newer 3D gradient-echo techniques, which allow the acquisition of each volume of data in tens of seconds as a contrast bolus traverses the region of interest (fast 3D CE MRA), offer the possibility of temporally resolving intradura…

Different sports show different patterns and frequencies of injuries, which are discussed in this paper. About 3% of all sports accidents relate to the spine. These injuries often have far-reaching consequences for the patients. A very early and extensive diagnosis of all changes is decisive for the start of an adequate therapy and thus for the prognosis of the injury. Radiological diagnosis is also of decisive importance for the documentation of late injuries and in the question of rehabilitation. Here special focus is put on MRT and CT diagnostics.A healthy spine of humans is normally able t…

Advances in imaging have greatly improved the ability to display spine anatomy and pathology. This article describes the imaging findings in degenerative, traumatic, infectious, and neoplastic disease. Anatomic information, however, must be tempered by an understanding of asymptomatic disease and interpreted in the context of the clinical syndrome.

UNLABELLED: SOFT TISSUE INJURY: Whiplash injuries of the cervical spine are common, usually after traffic accidents. Soft tissue injuries may involve the muscles, ligaments, disks, and facet joints (excluding severe cases of fracture or dislocation not discussed here).
CLINICAL MANIFESTATIONS: There are 2 cardinal signs: neck pain and headache. These signs may be associated or not with other symptoms.
CLINICAL COURSE: Most cases follow a benign course and resolve within a few weeks or months. Symptoms persist however in 20 to 40% of the cases. Evolutive organ injury, sensitization of the cen…

CLINICAL DATA: Thirty-one patients with 33 lesions of spinal tuberculosis (C1-D4) are reported. The distribution of lesions was C1-C2 (11), C3-C6 (13), C7-D4 (9). Neurological complications were present in 6 (55%), 8 (61%) and 7 (78%) in each region respectively.
DIAGNOSIS: Increase in the prevertebral soft tissue shadow in a standard radiograph was a useful guide to resort to CT Scan/MRI to diagnose tuberculosis of C1 and C2 region at an early (pre-subluxation) stage. The diagnosis of TB spine from C3-C6 was made confidently on clinico-radiological features. The anterior convexity and forwar…

Injuries of the spine in children rarely occur. They amount to about 0.2% of all fractures and dislocation and to 1.5 to 3% of all lesions of the spine. The younger an injured child is, the more likely it has sustained a lesion of the upper cervical spine. This spinal segment in comparison to adults is concerned more often and accounts for 50% of all C-spine injuries. Important differences between the adult spine and the spine in the child disappear with the age of 10 years. Later diagnostics, classification and treatment correspond widely with the principles valid in adults. The knowledge of …

The most common inflammatory disorders affecting the cervical spine include adult and juvenile rheumatoid arthritis, ankylosing spondylitis, Reiter's syndrome, and psoriatic arthritis. These disorders are characterized by typical deformities and instabilities of the cervical spine that result from the destruction caused by synovitis in bony and ligamentous structures in the neck. The treatment of these inflammatory lesions differs from the treatment of similar lesions found in the posttraumatic or degenerative spine. This article attempts to outline the epidemiology, clinical manifestations, a…

The advanced degenerative changes of the zygapophyseal and uncovertebral joints of the cervical spine are of essential clinical interest because the compression of the vertebral artery or the narrowing of the intervertebral foramina by these processes may cause severe neurological symptoms. Although open laminotomy remains an acceptable and, at times, necessary method of treatment for herniated intervertebral discs, spinal stenosis, and various intracanal spinal disorders, arthroscopic disc surgery is emerging as an alternative method of treatment for contained and nonmigrated sequestered herniated discs. Successful performance of arthroscopic disc surgery requires technical skill and a learning curve. Prudent patient selection, adherence to inclusion and exclusion criteria, and avoidance of entry into the spinal canal when possible will reduce the incidence of perineural and int

A retrospective study of 13 patients with metastases to the upper cervical spine was designed to examine the clinical efficacy of surgical treatment. All patients had severe neck pain and two had quadriplegia. Eleven patients underwent operative posterior stabilization, and two patients were treated with a brace and radiotherapy. Pain relief after surgery was significant so that 10 of the 11 patients could leave bed and resume their normal activities. The other two patients who were treated nonoperatively had severe dementia and sudden death from a respiratory arrest after a fall, despite temp…
