PURPOSE OF REVIEW: Assessment of cardiovascular stability using ventilation-induced changes in measured physiological variables, referred to … incidence of acute cor pulmonale is decreasing proportionally. When present, however, it induces a stroke volume variation that is 180 degrees out of phase with that seen in hypovolemic states, such that left ventricular stroke volume increases during inspiration rather than decreasing as seen in hypovolemia. Further, when either tidal volume or positive end-e …

AIMS: To investigate the value of assessment of mitral regurgitant fraction (RF) using left ventricular (LV) volumes obtained by three-dimensional echocardiography (3DE) to quantify primary mitral regurgitation (MR).
METHODS AND RESULTS: Sixty patients with primary MR in sinus rhythm were prospectively enrolled. RF was calculated using either 2DE or 3DE LV volumes obtained as follows: (LV total stroke volume - LV forward stroke volume by Doppler)/LV total stroke volume. Severity of MR was graded independently by two cardiologists blinded to LV volumetric data using an integrative approach, as…

Of the parameters in consideration for control of physiological rate responsive pacing special interest is directed to stroke volume. In search of indirect parameters which are correlated to stroke volume it is demonstrated on principle that systolic time intervals could be used. From the relation ejection time versus pre-ejection time (LVET/PEP) which is known for its correlation to stroke volume a simple algorithm is derived which is based on the realization that PEP is related to the reciprocal of cardiac output. First investigations with patients demonstrated the possibility of the computa…

In 9 patients with patent ductus arteriosus, quantification of left-to-right shunt was performed with dye dilution curves after peripheral injection and with radionuclide ventriculography. The study was repeated within 7 days after successful transluminal occlusion of the ductus with an Ivalon-plug. Reproducubility of the method could be studied in one patient in whom reopening of the ductus occurred. Dye dilution curves were analyzed using the method of Carter et al. Radionuclide ventriculography was performed as a combined first-pass and equilibrium study: effective stroke volume was derived…

(1) Background: We used four-dimensional phase-contrast magnetic resonance imaging (4D PC-MRI) to evaluate the impact of an endovascular aor … tive blood transport per heartbeat. Post-stenting quantitative flow revealed that the reductions in stroke volume, backward flow volume, and absolute stroke volume were greater when covered stents were used than when bare stents were used in the FL of the descending aorta. Bare stents had a higher backward flow volume than covered stents did. (4) Conclusions: T …

BACKGROUND: After a stroke, urinary dysfunction and urinary tract infection are estimated to be approximately 20%. Increased postvoid residual (PVR) is a risk factor for urinary dysfunction and urinary tract infections. These complications can negatively impact recovery weeks after the initial hospitalization. Early identification of risks and patient education are important.
PROJECT DESCRIPTION: This project was initiated for those patients admitted for a diagnosis of stroke on a 55-bed neuroscience unit in an acute care hospital. Patients were scanned after the initial void post stroke. Nur

Heart failure (HF) classification is still primarily based on left ventricular ejection fraction, even though this parameter only partially reflects the biological mechanisms determining disease progression and therapeutic response. The aim of this review was to present a conceptual interpretive framework that analyzes the HFrEF (heart failure with reduced ejection fraction) and HFmrEF (heart failure with mildly reduced ejection fraction) phenotypes through the lens of the network organization of pathophysiological processes. This review integrates data on molecular and cellular mechanisms, cl

Conventional modeling of cardiovascular function prioritizes the interplay between cardiac contractility and venous return curves as primary determinants of stroke volume. However, this approach does not capture the influence of arterial load in flow generation, thus hindering a more comprehensive understanding of macrovascular dynamics. Since both heart and arteries are elastic chambers with distinctive geometries housing blood volume, their coupling could be derived from pressure-volume curves, which ultimately denote time-varying elastance relationships. Accordingly, the left ventricular-ar…

During the initial workup of a patient with chronic heart failure, increased left ventricular filling pressures at rest or during exercise, and a left ventricular ejection fraction ≥50%, physicians are expected to exclude the presence of established diseases that masquerade as heart failure with preserved ejection fraction (HFpEF), referred to as "HFpEF mimics." These diseases include (1) cardiac amyloidosis; (2) hypertrophic cardiomyopathy; (3) infiltrative, restrictive, and inflammatory cardiomyopathies (sarcoidosis, hemochromatosis, Fabry disease, and radiation- and chemotherapy-induced res

Left atrial (LA) hypertension is central in the pathophysiology of heart failure (HF) in general and of HF with preserved ejection fraction (HFpEF) in particular. Despite approved treatments, a number of HF patients continue experiencing disabling symptoms due to LA hypertension, causing pulmonary congestion, pulmonary hypertension, and right heart dysfunction, at rest and/or during exercise. LA decompression therapies, i.e., left atrial shunting through a specifically designed device (either implant-based or implant-free), are being studied in various forms of HF to alleviate LA hypertension

Heart failure with preserved ejection fraction (HFpEF) is a heterogeneous syndrome with vast prevalence worldwide. Despite recent advances in understanding its pathophysiology, HFpEF remains under-diagnosed in clinical practice. Obesity-related HFpEF is a distinct and frequent phenotype with an additionally challenging diagnosis. We address the importance of overweight and obesity in HFpEF, focusing on the influence of adipose tissue in inflammation and neurohormonal activity. We also discuss atrial and ventricular remodelling in obesity-related HFpEF and potential clinical implications. Obesi

Heart failure with preserved ejection fraction (HFpEF) accounts for 50 % of heart failure (HF) cases, making it the most common type of HF, and its prevalence continues to increase in the aging society. HFpEF is a systemic syndrome resulting from many risk factors, such as aging, metabolic syndrome, and hypertension, and its clinical features are highly heterogeneous in different populations. HFpEF syndrome involves the dysfunction of multiple organs, including the heart, lung, muscle, and vascular system. The heart shows dysfunction of various cells, including cardiomyocytes, endothelial cell

There is uncertainty regarding the clinical effects of discontinuation of drugs for heart failure after long-term use. The withdrawal of long-term treatment can follow 1 of 4 distinct patterns: 1) loss of on-treatment effect with no observed changes following discontinuation (eg, prazosin); 2) attenuation or loss of on-treatment effect with rebound clinical worsening following discontinuation (eg, nitroprusside); 3) persistence of deleterious on-treatment effect followed by clinical worsening after discontinuation (eg, milrinone and flosequinan); and 4) persistence of favorable on-treatment ef

AIMS: To evaluate the prevalence of pathogenic variants in genes associated with dilated cardiomyopathy (DCM) in a clinical trial population with heart failure and reduced ejection fraction (HFrEF) and describe the baseline characteristics by variant carrier status.
METHODS AND RESULTS: This was a post hoc analysis of the Phase 3 PARADIGM-HF trial. Forty-four genes, divided into three tiers, based on definitive, moderate or limited evidence of association with DCM, were assessed for rare predicted loss-of-function (pLoF) variants, which were prioritized using ClinVar annotations, measures of

Heart failure with preserved ejection fraction (HFpEF) represents one of the greatest challenges facing cardiovascular medicine today. Despite being the most common form of heart failure worldwide, there has been limited success in developing therapeutics for this syndrome. This is largely due to our incomplete understanding of the biology driving its systemic pathophysiology and the heterogeneity of clinical phenotypes, which are increasingly being recognized as distinct HFpEF phenogroups. Development of efficacious therapeutics fundamentally relies on robust preclinical models that not only
