Intermediate- to long-term outcomes of transvaginal mesh for treatment of Asian women with pelvic organ prolapse.

INTRODUCTION: Short-term follow-up analyses suggest that transvaginal mesh has limited application for pelvic organ prolapse (POP) treatment. This study evaluated the intermediate- and long-term outcomes of transvaginal mesh surgery. METHODS: This retrospective study included all women who underwent transvaginal mesh surgery in one urogynaecology centre. Inclusion criteria were women with stage III/IV POP, age ≥65 years, and (preferably) sexual inactivity. Concomitant sacrospinous fixation and mid-urethral slings were offered for stage III/IV apical POP and urodynamic stress incontinence, res

Concomitant transobturator tape and anterior colporrhaphy versus transobturator subvesical mesh for cystocele-associated stress urinary incontinence.

INTRODUCTION AND HYPOTHESIS: Stress urinary incontinence (SUI) and cystocele often occur concomitantly and thus may potentially be treated via a single surgical procedure. This study evaluated the efficacy and safety of concomitant transobturator tape (TOT) with anterior colporrhaphy versus subvesical transobturator mesh (TOM) for cystocele-associated SUI. METHODS: This prospective, clinical trial included women with cystocele-associated SUI. Patients were randomly allocated into either group I (anterior colporrhaphy with concomitant TOT "in-out" fixation) or group II (implantation of a subve

Narrated video of a re-do colposuspension.

AIMS: To present a narrated video designed to demonstrate the steps involved in an open re-do colposuspension. METHODS: This was in a 70-year-old woman who presented with recurrent severe stress urinary incontinence. Urodynamics confirmed severe urodynamic stress incontinence, with no detrusor over activity. Her maximum urethral closure pressure (MUCP) was 8 cm/water. She previously had a total abdominal hysterectomy (TAH), bilateral salpingo-oophorectomy (BSO) and colposuspension in 1998. Subsequently, she had a TVT in 2002 and then partial excision of the tape in 2003 due to erosion. This w

[Diagnosis and surgical treatment of postprostatectomy stress incontinence: recommendation of the working group Urologische Funktionsdiagnostik und Urologie der Frau].

Today, for the surgical treatment of postprostatectomy incontinence, several treatment options are available, e.g., adjustable and functional sling systems, artificial sphincter, bulking agents, and balloons. However, no recommendations in terms of specific diagnostic tools and differentiated treatment options for everyday life are available. Our aim is to provide some clinically relevant recommendations for the necessary diagnostic workup and different treatment options of postprostatetectomy incontinence to support clinical decisions in everyday life. Treatment selection should be based on c

Groin granuloma formation after midurethral sling.

BACKGROUND: Complications of midurethral transoburator slings include injuries directly associated with trocar placement or subsequent erosion of the mesh that is placed. While tissue reaction to implanted mesh material occurs, rarely does it produce clinical symptoms. We report a case of granuloma formation after a transobturator tape midurethral sling presenting as a groin mass. CASE: A 59-year-old para 2 postmenopausal woman underwent uncomplicated placement of a transobturator tape midurethral sling concomitantly with prolapse repair. Two years later, she presented to her primary care phy

A comprehensive view on the actual trend in pelvic organ prolapse repair.

OBJECTIVE: The objective of this paper is to review the more recent literature on pelvic organ prolapse (POP). A detailed Medline search was performed. BACKGROUND: An understanding of the published literature is important in deciding, which surgical approach has to be used and how to counsel patients during informed consent. FINDINGS: In the review, we analyze the various interventions for POP and provides also a survey of the current controversies and areas where knowledge is incomplete and need further evaluation for definitive answers regarding optimization of surgical care for POP. While

Do urodynamic parameters predict persistent postoperative stress incontinence after midurethral sling? A systematic review.

INTRODUCTION AND HYPOTHESIS: It is unclear whether preoperative urodynamic study (UDS) values are predictive of outcomes after midurethral sling. MATERIALS AND METHODS: We systematically reviewed multiple databases from January 1989 to October 2011 for English-language studies correlating UDS data with postoperative outcomes after midurethral slings. We performed random effects model meta-analyses, as indicated. Relative risk (RR) ratios for the outcome of stress urinary incontinence (SUI) cure were calculated using high maximum urethral closure pressure (MUCP) and Valsalva leak point pressur

Urodynamic measures do not predict stress continence outcomes after surgery for stress urinary incontinence in selected women.

PURPOSE: We determined the prognostic value of preoperative urodynamic results in patients with stress urinary incontinence. MATERIALS AND METHODS: In a 9-center surgical trial, women with stress urinary incontinence were randomized to a Burch or pubovaginal sling procedure. Women were eligible for the study if they had predominant stress urinary incontinence symptoms, a positive cough stress test, a bladder capacity more than 200 ml and urethral hypermobility. Preoperative free uroflowmetry, filling cystometry and pressure flow studies were performed in all. Overall treatment success require

Stress urinary incontinence: the evolution of the sling.

Stress urinary incontinence is one of the most prevalent and costly problems encountered by the international medical community. The exact mechanism of stress incontinence remains elusive. Early management relied on behavioral modification but, as more advanced anatomic and urodynamic research surfaced, the focus shifted to surgical correction. Initial innovations provided a compressive force/hammock to support the urethra and bladder neck. For almost a century, the pubovaginal sling provided this support, with 70-90% cure rates at the expense of significant voiding dysfunction. Later work has
