A sixteen-year-old male with sickle cell anemia and congenital strabismus developed malignant hyperthermia a few minutes after the administration of succinylcholine, used as the general anesthetic for corrective eye surgery. The patient's hemoglobin S level was reduced to fifteen percent before the operation. He recovered uneventfully within a few hours. Increased serum creatinine phosphokinase activity and pathological changes observed in the muscle biopsy along with strabismus suggest that the patient had an inherited susceptibility to malignant hyperthermia.

Spinal muscular atrophy (SMA) is a rare lower motor neurone disease in which anesthetic management is often difficult as a result of muscle weakness, hypersensitivity to non-depolarizing neuromuscular blocking agent, and succinylcholine induced hyperkalemia. We describe a 50-year-old patient with type IV SMA for synovectomy. We decided to use an epidural technique to avoid muscle relaxants and tracheal intubation. After operation, there was no exacerbation of neurologic signs and symptoms.

Huntington's chorea is a rare hereditary disorder of the nervous system. It is inherited as an autosomal dominant disorder and is characterized by progressive chorea, dementia and psychiatric disturbances. The best anesthetic technique is yet to be established for these patients with increased risk of aspiration due to involvement of pharyngeal muscles and an exaggerated response to sodium thiopental and succinylcholine. The primary goal in general anesthesia for these patients is to provide airway protection and a rapid and safe recovery. We report the anesthetic management of a 51-year-old p…

We describe five patients with status asthmaticus whose respiratory acidosis persisted despite conventional treatment. Four were intubated with ketamine and succinylcholine and mechanically ventilated with immediate improvement of respiratory acidosis. One patient had been intubated previously with diazepam and succinylocholine and had a rise in pCO2 to 97. Ketamine was given IV with a rapid fall in pCO2. This improvement immediately after intubation is in contrast to previous reports of asthmatics whose respiratory acidosis and bronchospasm worsened immediately after intubation.

In anesthetized cats selective activation of fine muscle afferents of the hindlimb by KCl or bradykinin (Brad.) induced reflex changes in the urinary bladder. The nature of the reflex responses depended on the functional state of the bladder: they were excitatory when the bladder was quiescent and inhibitory during large, slow, rhythmic micturition contractions. Selective activation of large muscle primary afferents by succinylcholine was ineffective. All reflexes disappeared after cutting the sciatic or the pelvic nerves. Transection of the hypogastric nerves to the bladder had no effect.

Non-depolarizing neuromuscular blocking agents (NMBAs) produce neuromuscular blockade by competing with acetylcholine at the neuromuscular junction, whereas depolarizing NMBAs open receptor channels in a manner similar to that of acetylcholine. Problems with NMBAs include malignant hyperthermia caused by succinylcholine, anaphylaxis with the highest incidence for succinylcholine and rocuronium, and residual neuromuscular blockade. To reverse these blocks, anticholinesterases can act indirectly by increasing the amount of acetylcholine in the neuromuscular junction; sugammadex is the only selec…

The case histories are presented including the anaesthetic and postoperative management, of two children, a two-year-old with undiagnosed Duchenne muscular dystrophy (DMD) and a three-year-old with known DMD. The child with undiagnosed DMD had no symptoms of DMD and had received halothane twice before, without succinylcholine, with no apparent difficulty. Following an uneventful induction of anaesthesia with halothane, nitrous oxide and O2, succinylcholine resulted in bilateral masseter muscle spasm and then, in rapid sequence, ventricular tachycardia and cardiac arrest. Resuscitation was diff…

PURPOSE OF REVIEW: The regimen of thiopental, succinylcholine (SCh) and unsupplemented nitrous oxide/oxygen for Caesarean section was first introduced 50 years ago. This review examines which elements have endured, what has changed, and how practice might alter in the very near future.
RECENT FINDINGS: Sugammadex, a cyclodextrin, is capable of rapid reversal of profound rocuronium neuromuscular blockade.
SUMMARY: It is now feasible that SCh might be replaced with a high-dose rocuronium-sugammadex combination.

Problems encountered during halothane anesthesia in 3 horses included increasing rectal temperature, muscle rigidity or movement during anesthesia, irregular breathing patterns, and difficulty in stabilizing blood pressure. One horse had prolonged muscle fasciculations and failure to relax after administration of succinylcholine in addition to halothane. That horse developed severe, bilateral myositis of the triceps, lumbar, and gluteal muscles. Problems encountered were similar to those caused by the disease known as malignant hyperthermia.

Malignant hyperthermia is a rare disease triggered by succinylcholine and the volatile anesthetic agents in genetically predisposed individuals. Recent studies have implicated an abnormality in the calcium release channel of the sarcoplasmic reticulum in skeletal muscle as the likely etiology. Genetic studies have narrowed the search for the chromosomal abnormality to human chromosome 19. Although the mortality from this disorder has dramatically decreased in the past decade due to the discovery of dantrolene, elective diagnosis of the disorder is only now appearing on the horizon.

An injection port pyrolysis method for the analysis of quaternary ammonium compounds is reported. Identification and confirmation of the analyte was accomplished by gas chromatography/mass spectrometry. With minor modifications to pH used and solvent composition, a common ion-pair method of extraction of quaternary ammonium compounds from biological matrices with an iodide counter ion is described. Although the combined extraction-identification methodology was limited to neostigmine, succinylcholine, and paraquat, it should easily be applicable to any of the other compounds examined in this s…

Malignant hyperthermia is induced by potent inhalation anesthetics. Enflurane must be added to the list of those anesthetic agents (such as halothane and succinylcholine) that are associated with this condition. The patient in our study was a young woman with no history of prior exposure to general anesthetics, and no family history of complications following administration of anesthetics. The other possible causes of hyperthermia in the patient were investigated and eliminated, and the condition was finally associated with enflurane. Enflurane should not be used in patients with a family hist…
