A rapid, simple questionnaire to assess gastrointestinal symptoms after oral ferrous sulphate supplementation.

BACKGROUND: Oral iron supplementation is often associated with rapid onset of gastrointestinal side-effects. The aim of this study was to develop and trial a short, simple questionnaire to capture these early side-effects and to determine which symptoms are more discriminating. METHODS: The study was a double-blind placebo-controlled randomized parallel trial with one week treatment followed by one week wash-out. Subjects were randomized into two treatment groups (n = 10/group) to receive either ferrous sulphate (200 mg capsules containing 65 mg of iron) or placebo, both to be taken at mealti

Assessing DSM-IV symptoms of panic attack in the general population: an item response analysis.

BACKGROUND: Unexpected panic attacks may represent a non-specific risk factor for future depression and anxiety disorders. The examination of panic symptoms and associated latent severity levels may lead to improvements in the identification, prevention, and treatment of panic attacks and subsequent psychopathology for 'at risk' individuals in the general population. METHODS: The current study utilised item response theory to assess the DSM-IV symptoms of panic in relation to the latent severity level of the panic attack construct in a sample of 5913 respondents from the National Epidemiologi

Assessment of non-neurogenic incontinence and lower urinary tract symptoms in adolescents and young adults.

AIMS: The aim of this article is to provide insight to clinicians who principally treat adults, about non-neurogenic lower urinary tract symptoms (LUTS) either pre-dating or presenting in adolescence. Assessment fundamentals, diagnostic classification, and rationale behind the process of care are discussed. METHODS: Standardized terms are used to describe the current classification of symptoms of LUT dysfunction in young adults. The etiology and symptomatology of continuous and organic incontinence, along with the intermittent presentations of incontinence, nocturnal enuresis, and bowel dysfu

Ultrasonography in early assessment of elderly patients with polymyalgic symptoms: a role in predicting diagnostic outcome?

OBJECTIVE: To study the usefulness of ultrasonography (US) in predicting the diagnostic outcome in patients with polymyalgic symptoms. METHODS: Sixty-one elderly patients with polymyalgic syndrome were recruited in a secondary care setting and followed up in a prospective way. Clinical, laboratory, and US data obtained at onset were re-evaluated after 1 year when diagnostic outcome was defined. RESULTS: A diagnostic shift was observed in 32 polymyalgic patients (52%). Calcium pyrophosphate deposition disease (CPDD) was diagnosed in nine patients, elderly-onset rheumatoid arthritis (EORA) in

The Picture Anxiety Test (PAT): a new pictorial assessment of anxiety symptoms in young children.

This study examines the psychometric properties of a newly developed pictorial anxiety assessment, the Picture Anxiety Test (PAT), designed for young children. A sample of 71 5-7-year-old children (clinically anxious and healthy controls) participated in an assessment comparing the PAT to other, established anxiety measures. Children's ratings were also compared to parent ratings of internalizing and externalizing problems. The PAT demonstrated favorable psychometric properties in this sample, including moderate to high internal consistency, high convergent validity with related constructs, an

New method for minimally invasive urodynamic assessment in men with lower urinary tract symptoms.

OBJECTIVES: Urodynamic studies are considered the reference standard to diagnose bladder outlet obstruction. However, the procedure is invasive, expensive, and time-consuming. The purpose of this study was to evaluate a new minimally invasive urodynamic assessment model and compare the results with those of conventional urodynamic evaluation. METHODS: The study included 50 male patients who presented with lower urinary tract symptoms. Their mean age was 62 years (range 34 to 82). After undergoing a conventional urodynamic study, they underwent the minimally invasive evaluation. The urethral d

Symptom exaggeration by college adults in attention-deficit hyperactivity disorder and learning disorder assessments.

To test the hypothesis that sub-optimal effort detected by one popular symptom validity measure, the Word Memory Test (WMT), should be interpreted as symptom exaggeration, the authors examined attention-deficit hyperactivity disorder (ADHD) and learning disorder (LD) assessment data collected from healthy adult patients over the past four years at one mid-size Southeastern college. They conducted six tests of this hypothesis, drawing upon extant research. Rates of apparent symptom exaggeration comparable to those found in medicolegal settings (e.g., personal injury cases), particularly in the

Prospective assessment of patient-rated symptoms following whole brain radiotherapy for brain metastases.

To prospectively assess patient-rated symptoms in patients with brain metastases treated with whole brain radiotherapy, these patients were asked to rate their symptoms on the Edmonton Symptom Assessment Scale (ESAS) before, and 1, 2, 4, 8, and 12 weeks following the radiation treatment. ESAS evaluates pain, fatigue, nausea, depression, anxiety, drowsiness, appetite, sense of well-being, and shortness of breath on a scale of 0-10 (0=absence of symptom and 10=worst possible symptom). Patients with a language barrier or significant cognitive impairment were excluded. The mean difference of ESAS …

Assessment of respiratory symptoms with dual pH monitoring in patients with gastro-oesophageal reflux disease.

BACKGROUND: Gastro-oesophageal reflux disease (GORD) is a commonly underestimated aetiological factor in patients with respiratory symptoms. In this study, acid reflux in healthy volunteers and patients with GORD with and without respiratory symptoms was investigated by dual pH monitoring. METHODS: Thirty healthy volunteers and 43 patients with GORD underwent oesophageal manometry and dual pH monitoring with one probe in the proximal and one in the distal oesophagus. Nineteen of the 43 patients complained of respiratory symptoms. RESULTS: There were no differences in proximal probe measureme

Use of changes in symptoms to predict changes in lung function in assessing the response to asthma therapy.

BACKGROUND: The majority of adult patients with asthma are managed by primary care providers. Although there is no generally accepted gold standard for the assessment of asthma severity in general practice, treatment decisions and modifications to therapy are strongly influenced by patients' symptoms and history of asthma medication use. OBJECTIVES: The primary goal of this study was to determine whether there is a correlation between changes in asthma symptoms during treatment and changes in lung function, as measured by peak expiratory flow (PEF). A secondary goal was to compare the relativ

Measuring affective symptoms of depression in aphasia: development of an accessible ecological momentary assessment tool.

PURPOSE: Post-stroke depression is highly prevalent in aphasia, yet existing depression measures rely heavily on language and lack sufficient validity for this population. The aim of this study was to develop an aphasia-accessible Ecological Momentary Assessment (EMA) of depression based on the input of people with aphasia, their care partners, and speech-language pathologists (SLPs). METHODS: Nine focus groups were conducted with people with aphasia (n = 15), care partners (n = 13), and SLPs (n = 13) to identify relevant depression symptoms. Items were selected based on factors such as endor

Is thumb carpometacarpal instability assessed in young patients with thumb base symptoms? A multicenter study.

UNLABELLED: Thumb base complaints in younger patients are diagnostically challenging, and symptoms are commonly attributed to either carpometacarpal (CMC) osteoarthritis or ligamentous instability. These conditions require fundamentally different treatment strategies, making accurate diagnosis essential. The reported frequency of CMC instability varies considerably across clinical practices, possibly reflecting differences in diagnostic awareness rather than true differences in prevalence. This study therefore aimed to describe the diagnostic workup of younger patients with nontraumatic thumb

Ecological Assessment of Transdiagnostic Clinical Symptoms in Serious Mental Illness with Daily Smartphone Surveys.

Clinical symptoms in serious mental illness (SMI) fluctuate dynamically, yet standard interview-based assessments often fail to capture these daily changes. Smartphone-based ecological surveys offer a scalable approach to monitoring symptoms in naturalistic settings. We analyzed longitudinal data from 56 outpatients with psychotic or affective disorders who completed 12,984 daily surveys and 1,028 clinical assessments over one year. Machine learning models showed that smartphone surveys moderately estimated Montgomery-Åsberg Depression Rating Scale (rrm = 0.57; p < 0.001) and Young Mania Ratin

Feasibility of assessing symptoms associated with ovarian cancer using an electronic medical intake questionnaire.

OBJECTIVE: Symptoms occur in up to 75% of patients with early-stage ovarian cancer; as such, using a symptom inventory (SI) to stratify which patients should receive screening for ovarian cancer is an attractive approach to mitigate false-positives. We report on the feasibility and results of a prospective four-question SI that assessed the following symptoms: abdominal pain, bloating, early satiety, and urinary complaints. Frequency and duration of symptoms were not assessed. METHODS: A SI was added to the standard pre-appointment check-in process via the electronic medical record's (EMR) pa

The Role of Urodynamics in Assessing Lower Urinary Tract Symptoms Post-Radical Prostatectomy: A Review.

INTRODUCTION: This review examines the role of urodynamics (UDS) in evaluating lower urinary tract symptoms (LUTS) following radical prostatectomy. We first present typical urodynamic findings in post-prostatectomy men, then discuss applications of UDS, and finally examine treatment pathways for post-prostatectomy LUTS beyond stress urinary incontinence (SUI) surgery. METHODS: A narrative review was performed focusing on the current primary literature and society guidelines on the role of UDS post-prostatectomy. RESULTS: LUTS after prostatectomy are common, most frequently storage LUTS, spec
