[Arrhythmic syncope and invasive electrophysiologic heart function tests].

The authors submitted to electrophysiological examination a total of 100 patients (66 men and 34 women) with brief disorders of consciousnes … topic arrhythmias and high-grade a-v blocks. Invasive examination confirmed or diagnosed arrhythmic syncopes in 49% of the entire group of 100 patients. It assessed the arrhythmic cause of syncopes in 33% of 57 subjects without severe arrhythmias and in 70% of 43 subjects with severe arrhythmias detected during ECG monitoring.

Evaluation of syncope from head and neck cancer.

Five patients suffering from recurrent syncope in association with metastatic squamous cell carcinoma of the head and neck were examined. Two patients had exhaustive diagnostic work-up for syncope, which eventually disclosed previously undiagnosed, recurrent squamous cell carcinoma. Case reports describe glossopharyngeal neuralgia, a well recognized cause of syncope in the head and neck cancer patient, characterized by acute unilateral head or neck pain preceding each syncopal episode. The literature on the diagnosis of syncope is reviewed, and the syncopal mechanisms unique to the head and ne…

Cardiovascular reflex responses in patients with unexplained syncope.

1. This study was undertaken to determine whether, in a group of patients complaining of recurrent syncopal attacks but with no apparent cause, there was evidence of abnormal cardiovascular reflex control. 2. The steady-state responses of blood pressure, heart rate and cardiac output to head-up tilting were determined in 67 patients using entirely 'non-invasive' methods. In some patients we also studied the immediate response of pulse interval to carotid baroreceptor stimulation by neck suction. 3. Two of the patients developed vasovagal attacks during the 20 min test period of head-up tilting

Paroxysmal vagally mediated AV block with recurrent syncope.

Paroxysmal complete atrioventricular (AV) block without associated electrocardiographic (ECG) abnormality is not a well recognized entity. A … mal, vagally mediated complete AV block should be seriously considered in patients with unexplained syncope.

[Psychosomatics of psychogenic faint (vasovagal syncope) (author's transl)].

Psychogenic faint (vasovagal syncope) shows no difference with respect to age and frequency of attacks in women and men. A marked increase in frequency is found in earlier ages up to 25. Physical complaints include: blackout, weakness of the limbs, sensation of heat, nausea, palpitations, drowsiness. Mental sensations include: feeling of being overpowered, fear, helplessness, being unable to run away and of relief. Subjective emotional involvement in the situation seems to be decisive for eliciting psychogenic syncope, the subject being no longer able to control the situation from the psychic …

Clinical features of cough syncope: a systematic review.

BACKGROUND: Cough syncope (CS) is a rare subtype of syncope that remains underrepresented in evidence-based medicine. We aimed to characterize the clinical features of CS, with the aim of improving recognition and optimizing management strategies. METHODS: We present five cases of CS alongside a systematic review of its clinical characteristics based on published literature to improve recognition and optimize management. Reports of eligible patients were screened, and demographic, clinical, diagnostic, treatment, and outcome data were analyzed. RESULTS: This study included 60 patients with a…

Giant Right Coronary Artery Aneurysm Presenting as Syncope.

Coronary artery aneurysms (CAAs) are rare findings, most commonly detected incidentally during coronary angiography or advanced cardiac imaging. Giant aneurysms of the right coronary artery are exceptionally uncommon and may lead to severe complications, including thrombosis, embolization, myocardial infarction, or sudden cardiac death. We present the case of a 78-year-old male who was diagnosed with a giant right CAA during evaluation for syncope. The patient underwent successful surgical excision of the aneurysm with revascularization of the right coronary artery. This case highlights the di…

Syncope and Ventricular Tachycardia: Revisiting the Evidence Base.

BACKGROUND: Syncope is common across cardiac disorders, yet its arrhythmic mechanisms are often misattributed. A pivotal point is frequently overlooked: sustained ventricular tachyarrhythmias (VT) are not self-terminating. CONTENT: We synthesize evidence from community ED cohorts, ILR registries, and ICD trials to clarify when ventricular rhythms can plausibly explain brief, self-limited loss of consciousness. Across structural heart disease and inherited conditions, sustained monomorphic or polymorphic VT, when they occur, persist or degenerate and require device/external termination; by nat…

Cardioinhibitory reflex syncope : Cardioneuroablation as a therapeutic alternative.

The case of a 36-year-old female patient with recurrent reflex syncope characterized by predominant cardioinhibition and episodes of asystole lasting up to 9 s, as documented by an implantable loop recorder, is reported. Given her young age and symptomatic burden, cardioneuroablation (CNA) was performed without complications as an alternative to pacemaker implantation, in accordance with European Heart Rhythm Association (EHRA) recommendations. In this case, CNA has shown to be an effective and safe treatment option for managing recurrent cardioinhibitory reflex syncope. CNA should particularl…

Pediatric syncope: pearls and pitfalls in history taking.

Syncope is a heterogeneous syndrome with complex underlying mechanisms, hence, the spectrum of patients presenting with syncope is broad. The diagnosis of syncope begins with history taking, and an accurate diagnosis can be established through correct history taking and interpretation. Building and interpreting patient history are the main factors that cause a diagnostic yield gap between experts and nonexperts. The most frequent source of error is a clinician's misconception rather than an inaccurate account of patient symptoms. Clinicians can have several diagnostic pitfalls while evaluating…

Permanent pacing versus cardioneuroablation for cardioinhibitory vasovagal syncope.

BACKGROUND: We compared the efficacy and safety of cardioneuroablation (CNA) vs. permanent pacing (PM) for recurrent cardioinhibitory vasovagal syncope (CI-VVS). METHODS: One hundred sixty-two patients (CNA = 61, PM = 101), age 36 + 11 years) with syncope frequency of 6.7 ± 3.9/year were included in this multicenter study. All patients with CNA were provided by a single center, while patients with PM were provided by 4 other centers. In the CNA arm, an electroanatomic mapping guided approach was used to detect and ablate ganglionated plexus sites. Dual chamber rate drop response (RDR) or clos…

Syncope in the athlete - Minor changes, major diagnosis!

We report the case of a 17-year-old athlete who resorted to the emergency department for palpitations and dizziness while exercising. He mentioned two exercise-associated episodes of syncope in the last six months. He was tachycardic and hypotensive. The electrocardiogram showed regular wide complex tachycardia, left bundle branch block morphology with superior axis restored to sinus rhythm after electrical cardioversion. In sinus rhythm, it showed T-wave inversion in V1-V5. Transthoracic echocardiography revealed mild dilation and dysfunction of the right ventricle (RV) with global hypocontra…

Syncope and COVID-19 disease - A systematic review.

BACKGROUND: Syncope is not a common manifestation of COVID-19, but it may occur in this context and it can be the presenting symptom in some cases. Different mechanisms may explain the pathophysiology behind COVID-19 related syncope. In this report, we aimed to examine the current frequency and etiology of syncope in COVID-19. METHODS: A systematic review across PubMed, ISI Web of Knowledge and SCOPUS was performed, according to PRISMA guidelines, in order to identify all relevant articles regarding both COVID-19 and syncope. RESULTS: We identified 136 publications, of which 99 were excluded…
