LEARNING OBJECTIVES: After reading this article, the participant should be able to: 1. Make decisions on flexor tendon repair based on current evidence. 2. Perform some important tendon transfers after viewing Dr. Kozin's videos. 3. Inject local anesthesia for wide-awake flexor tendon repair after viewing the appropriate videos in the article. 4. Use relative motion extension splints for the postoperative management of extensor tendon injuries.
SUMMARY: This article provides a practical, clinically useful overview of some of the current best techniques and evidence available to the plastic su

Peroneal tendon injuries are underdiagnosed and should be considered in every patient who presents with chronic lateral ankle pain. Ankle sprains are common, and up to 40% of affected individuals experience subsequent chronic ankle pain. Identifying the source of chronic ankle pain can be difficult because of the large number of possible causes. The peroneal tendons are the primary evertors of the foot and function as lateral ankle stabilizers. A careful physical examination, along with a thorough patient history and imaging studies, are critical in arriving at an accurate diagnosis. Understan…

The operative management of acute Achilles tendon rupture marks the beginning of a comprehensive rehabilitation program. The goals of the rehabilitation program start with the reduction of pain and swelling and the recovery of ankle motion and power. They conclude with the restoration of coordinated activity and safe return to athletic activity. The rehabilitation protocol is directed by the injury and the quality of the repair, along with the patient's age, medical and social history, and athletic inclination. The protocol is dynamic and responsive to changing clinical findings.

Professional ballet dancers require an extraordinary anatomic, physiologic, and psychologic makeup to achieve and sustain their level of ability and activity. They are subject to a myriad of injuries as a result of the extreme demands of this profession. Tendon injuries are common and often coexist with other pathologies of the bone, ligaments, and psyche. It is critical that the dance doctor not examine the tendon injury in isolation, but rather the cause of the injury, either intrinsic from anatomic malalignment or from external sources, including poor form.

The case reported is of a 46 year old male who presented with a history of acute on chronic knee pain. The clinical features and investigations suggested a tear of the quadriceps tendon, with pre-existing chronic calcific enthesopathy. The operative findings were of an acute collection of calcific material within the quadriceps tendon. This acute presentation and calcific collection have not previously been reported in the quadriceps tendon.

Pathology of the subscapularis tendon is both infrequently identified and not commonly considered as a major source of shoulder pain and dysfunction. Subscapularis tendon pathology can present as isolated tears; partial-thickness tears; anterosuperior tears, also involving the supraspinatus tendon; complete rotator cuff avulsion; and rotator interval lesions, in which instability of the long head of the biceps tendon may dominate the clinical presentation. Although an accurate physical examination is paramount, modalities such as arthroscopy, magnetic resonance imaging, and ultrasound have adv

The refinement of high frequency transducers has improved the ability of ultrasound (US) to detect fine textural abnormalities of tendons and nerves and to identify a variety of pathologic conditions. Characteristic echotextural patterns, closely resembling the histologic ones, are typically depicted in these structures using high US frequencies. In tendon imaging, US can identify tendon dislocations, degenerative changes, differentiate partial from complete tears and determine whether the patient has to be treated surgically or conservatively. A spectrum of findings in inflammatory conditions…

The past decade has seen some remarkable advances in the management of acute tendon repair, both flexors and extensors. New surgical techniques, such as the epitenon-first technique for flexor tendons, combined with early motion rehabilitation, pharmacologic intervention to prevent adhesions, and noninvasive imaging techniques such as MR imaging to assess repair integrity lead to one inescapable conclusion. Continued evolution in attempts to improve the overall results of tendon injury can only mean that hand surgeons have not yet reached the perfect solution in tendon repair strategy. Much pr…

641 patients who had undergone ultrasonic investigation of the shoulder joint were reviewed retrospectively. Thirty-four cases (5.3%) with sonographic abnormalities of the biceps tendon or the soft tissue surrounding the biceps tendon were found. In 15 cases a synovitis or effusion of the bicipital (convexity of the transverse ligament) groove was found. Four times a mineralization of the transverse ligament (high echogenic zone with interruption of the humerus coticalis) was observed. In 10 cases rupture of the tendon (sagging of the transverse ligament) was diagnosed. One case was diagnosed

Rupture of the patellar tendon is a relatively infrequent, yet disabling, injury, which is most commonly seen in patients less than 40 years of age. It tends to occur during athletic activities when a violent contraction of the quadriceps muscle group is resisted by the flexed knee. Rupture usually represents the final stage of a degenerative tendinopathy resulting from repetitive microtrauma to the patellar tendon. This injury may also occur during less strenuous activity in patients whose tendons are weakened by systemic illness or the administration of local or systemic corticosteroid medic…

In conclusion, the sports medicine specialist is able to use a spectrum of diagnostic, surgical, and rehabilitation techniques to identify etiologic factors and to choose optimal treatment regimens for patients with Achilles tendinitis or traumatic rupture. Correction of pathomechanic factors, anatomic restoration, and aggressive postoperative rehabilitation allows an early return to sport without significant loss of strength or mobility.

An exercise program for tendon gliding is introduced for use in hand treatment and rehabilitation. The program incorporates thumb range of motion and three basic fist positions: book, fist, and straight fist. The anatomic and experimental basis of this program is discussed. The exercises allow the flexor tendons to glide to their maximum potential and can greatly facilitate therapeutic activities when incorporated in a comprehensive program of occupational and hand therapy.

In 1979 Verdan wrote, "whatever the situation may be, adhesions are until now certainly not a technically avoidable accident, but rather a c … cation of adhesions. Armed with this information, each surgeon dealing with interruptions of flexor tendons must develop a rational, systematic approach to the management of these difficult injuries. The principles of atraumatic technique, as set down many years ago by Bunnell, remain inviolate. Repair procedures should be carried out by surgeons who are …

The decision to reconstruct the flexor mechanism should be based upon a thorough understanding of the principles and techniques of tendon grafting and comprehension of the patient's functional needs. By itself, insertion of a tendon implant with later grafting will not guarantee a satisfactory result; this procedure is no substitute for an awareness of basic rules and axioms. Careful selection, meticulous technique, and an attentive rehabilitation program will provide patients with the best results.

An extensive search of the English and foreign language literature yields clinical reports on only 87 patients with open injuries of various tendons about the foot and ankle. This retrospective review is presented to document case histories in 80 additional patients, to present the results of 46 patients, and to assess the treatment recommendations outlined in the literature.

Avulsion of the triceps tendon should be suspected in patients who have pain about the elbow following a deceleration stress to the upper extremity. In three cases, the avulsion occurred both with or without a concomitant blow to the posterior aspect of the arm. Examination revealed pain, swelling, and a palpable depression just proximal to the olecranon. Roentgenograms showed avulsed osseous material. In the evaluation and management of triceps avulsion, it was important to differentiate between complete and partial tears. Careful examination of active range of motion of the elbow was the mos

This series of case suggests that the entity of tenosynovitis of the popliteus tendon is more common than once recognized. A high index of sucpicion and accurate palpation of the lateral aspect of the knee lead one to the diagnosis. Knowledge of this entity may prevent future misdiagnosis of tear of the lateral meniscus and unnecessary meniscectomy as experienced by Helfet, Holden, and myself. There is a definite correlation with activities requiring downhill walking or running. The runners invariably complained of the oneset of symptoms during downhill running rather than uphill running.
