Detection of vascularity in wrist tenosynovitis: power doppler ultrasound compared with contrast-enhanced grey-scale ultrasound.

INTRODUCTION: We sought to assess vascularity in wrist tenosynovitis by using power Doppler ultrasound (PDUS) and to compare detection of intra- and peritendinous vascularity with that of contrast-enhanced grey-scale ultrasound (CEUS). METHODS: Twenty-six tendons of 24 patients (nine men, 15 women; mean age ± SD, 54.4 ± 11.8 years) with a clinical diagnosis of tenosynovitis were examined with B-mode ultrasonography, PDUS, and CEUS by using a second-generation contrast agent, SonoVue (Bracco Diagnostics, Milan, Italy) and a low-mechanical-index ultrasound technique. Thickness of synovitis, ext…

Post-traumatic Candida infection leading to bilateral wrist rice body tenosynovitis: a case report and literature review.

BACKGROUND: Rice body tenosynovitis is a rare condition of uncertain etiology characterized by the formation of multiple fibrinous particles within the synovium or synovial fluid. It is frequently associated with chronic inflammation due to infection or rheumatoid arthritis and most commonly affects large joints such as the shoulder and knee. Simultaneous bilateral wrist involvement following trauma is exceptionally uncommon. CASE PRESENTATION: A 59-year-old man presented with persistent bilateral wrist swelling for 10 months after a fall. MRI demonstrated extensive fluid accumulation and syno…

Two Cases of Flexor Hallucis Longus Tenosynovitis With Highly Viscous Fluid Accumulation: The Role of Ultrasound Imaging.

Soft tissue injuries of the medial ankle including the flexor hallucis longus can be difficult to diagnose because of the complex functional regional anatomy. Flexor hallucis longus tenosynovitis is a relatively uncommon but well-described phenomenon in sports that require repetitive forefoot push-off in extreme plantar flexion. Although more work needs to be done to differentiate subtleties in imaging, identifying if the fluid can be easily compressed by sonographic pressure may help differentiate a typical flexor hallucis longus tenosynovitis from one with viscous, ganglion-like fluid. Here,…

Deep Learning Based Automatic Ankle Tenosynovitis Quantification from MRI in Patients with Psoriatic Arthritis: A Feasibility Study.

Background/Objectives: Tenosynovitis is a common feature of psoriatic arthritis (PsA) and is typically assessed using semi-quantitative magnetic resonance imaging (MRI) scoring. However, visual scoring s variability. This study evaluates a fully automated, deep-learning approach for ankle tenosynovitis segmentation and volume-based quantification from MRI in psoriatic arthritis (PsA) patients. Methods: We analyzed 364 ankle 3T MRI scans from 71 PsA patients. Four tenosynovitis pathologies were manually scored and used to create ground truth segmentations through a human-machine workflow. For e…

Iatrogenic flexor tendon rupture caused by misdiagnosing sarcoidosis-related flexor tendon contracture as tenosynovitis: A case report.

BACKGROUND: Sarcoidosis is a multisystem disease characterized by granuloma formation in various organs. Sarcoidosis-related flexor tendon contractures are uncommon in clinical settings. This contracture is similar to stenosing tenosynovitis and potentially leads to misdiagnosis and mistreatment. Herein, we report a rare case of sarcoidosis-related finger flexor tendon contracture that was misdiagnosed as tenosynovitis. CASE SUMMARY: A 44-year-old woman presented to our department with flexion contracture of the right ring and middle fingers. The patient was misdiagnosed with tenosynovitis an…

De Quervain's tenosynovitis: a non-randomized two-armed study comparing ultrasound-guided steroid injection with surgical release.

PURPOSE: Ultrasonography is currently used for both diagnostic and therapeutic purposes in de Quervain's tenosynovitis. There is a dearth of information on how effective an ultrasound-guided (USG) steroid injection is when compared to surgical release of the first extensor compartment. Hence, we performed a non-randomized two-armed comparison study to test our hypothesis that USG guided steroid injection is equally effective as surgery. METHOD: 62 consecutive patients participated in the study with 32 of them selecting the option of USG guided injection (Set A), and the rest undergoing surgic…

Pathological and molecular findings of avian reoviruses from clinical cases of tenosynovitis in poultry flocks from Brazil.

Avian reoviruses (ARVs) can infect a variety of species worldwide. Birds can present stunting syndrome, respiratory and/or enteric diseases, immunosuppression, malabsorption, viral arthritis/tenosynovitis, and even secondary infections by other microorganisms. Flaws in conventional vaccines and the increase in the diagnostic rate of disease in the last 5 yr suggest the emergence of pathogenic ARVs in the poultry flocks worldwide. This study aimed to characterize birds lesions and to detect/genotype ARVs from a severe outbreak of tenosynovitis in broiler poultry flocks from Brazil. Tissue sampl…

Diagnosis of flexor hallucis longus tenosynovitis using the cross-sectional area of the flexor hallucis longus tendon.

BACKGROUND: Flexor hallucis longus tenosynovitis (FHLTS) is the inflammation of the sheath through which the flexor hallucis longus tendon (FHLT) passes. The FHLT thickness (FHLTT) is correlated with ankle pain. Thus, the FHLTT is considered a characteristic FHLTS feature. However, asymmetrical thickening of the FHLT can occur at any point. Therefore, we used the FHLT cross-sectional area (FHLTCSA) as a new morphological parameter to diagnose FHLTS. METHODS: 26 participants in the Control group (Group 1) and 26 in the FHLTS group (Group 2) underwent magnetic resonance imaging. The FHLTCSA was…

Anatomic location of the first dorsal extensor compartment for surgical De-Quervain's tenosynovitis release: A cadaveric study.

BACKGROUND: De-Quervain's tenosynovitis is a disorder arising from the compression and irritation of the first dorsal extensor compartment of the wrist. Patients who fail conservative treatment modalities are candidates for surgical release. However, risks with surgery include damage to the superficial radial nerve and an incomplete release due to inadequate dissection. Currently, there is a paucity of literature demonstrating the exact anatomic location of the first dorsal extensor compartment in reference to surface anatomy. Thus, this cadaveric study was performed to determine the exact loc…

Pulley Reconstruction Following Surgical Release of DC1 Pulley in De Quervain's Tenosynovitis: Surgical Technique and Case Series.

De Quervain's disease (DQD) is tenosynovitis of the first dorsal compartment (DC1) of the wrist between the osteofibrous tunnel and the tendons involving the APL and EPB sheaths at the radial styloid. Surgical intervention is indicated when pain does not resolve despite 3 to 6 months of conservative management. Release of the first dorsal compartment is an effective treatment of DQD. In addition to surgical release, we performed pulley reconstruction using a new technique in the present series of 20 patients which has not been previously described with a followup of over 1 year. All patients s…
