Dichotomous Locomotor Recoveries Are Predicted by Acute Changes in Segmental Blood Flow after Thoracic Spinal Contusion Injuries in Pigs.

Neuroimaging facilitates the translation of animal pre-clinical research to human application. The large porcine spinal cord is useful for testing invasive interventions. Ideally, the safety and efficacy of a delayed intervention is tested in pigs that have recovered sufficiently after spinal cord injury (SCI) to allow either deterioration or improvement of function to be detected. We set out to create moderate severity T9 injuries in Yucatan minipigs by conducting a bridging study adapting methods previously developed in infant piglets. The injury severity was varied according to two pneumati

Penetrating abdomino-thoracic injuries: report of four impressive, spectacular and representative cases as well as their challenging surgical management.

UNLABELLED: Gunshot wounds are rare events in European countries, but stab and impalement injuries occur more frequently and are often spectacular. The aim of the study was to describe several types of penetrating abdomino-thoracic injuries as well as the appropriate surgical interventions, including complex wound management. MATERIAL AND METHODS: The representative case series includes four patients with abdomino-thoracic penetrating trauma (two impalements and two stabbings), who were treated in a surgical university hospital (tertiary) centre during a 12-month period. RESULTS: 1. A man wa…

Therapeutic management of in-stent thrombosis after thoracic endovascular aortic repair for blunt thoracic aortic injury in a coronavirus disease 2019 patient.

A 27-year-old man underwent thoracic endovascular aortic repair for blunt thoracic aortic injury. Fourteen months later, he presented with intermittent paraplegia, congestive heart failure, and a decline of kidney function as a result of high-grade aortic stenosis caused by in-stent thrombosis. He had a concurrent infection with coronavirus disease 2019. The patient was successfully treated using axillofemoral bypass, followed by stent relining 2 weeks later. The possible risk factors and the optimal therapeutic approach for in-stent thrombosis remain unknown, because only a limited number of

Seal the bleed or wait to proceed? Resuscitation and early vs. delayed timing for thoracic endovascular aortic repair in blunt thoracic aortic injury.

BACKGROUND: In patients with blunt thoracic aortic injury (BTAI) undergoing thoracic endovascular aortic repair (TEVAR), the relationship between resuscitation (defined by markers such as lactate, base excess (BE), and need for transfusion) and TEVAR timing is not well defined. Several studies suggest repair after 24 h is associated with decreased mortality. The impact of related injuries and their competing need for resuscitation and intervention on the timing of TEVAR is not well defined. This study aims to compare patients who underwent early versus delayed TEVAR and determine mortality-re

Hemorrhagic Shock due to Internal Thoracic Artery Injury After Cardiopulmonary Resuscitation.

Internal thoracic artery injury can occur after cardiopulmonary resuscitation (CPR). Aggressive fluid resuscitation may mask additional bleeding sites in cases of hemorrhage-induced cardiac arrest. Persistent hemodynamic instability after return of spontaneous circulation should prompt evaluation for CPR-related hemorrhagic complications.

The effectiveness of an early management guideline for older patients following blunt thoracic injury.

BACKGROUND: Thoracic injury is a leading cause of trauma related morbidity and mortality worldwide. Older patients (aged ≥65 years) who have sustained rib fractures following blunt chest wall trauma are at greater risk of serious pulmonary complications, prolonged hospital stay, and death when compared with the general population. The Blunt Chest-Wall Injury and Rib Fracture Pathway (BCWP) was introduced at our institution in July 2021 and advocates for early, aggressive analgesia, specialist support from the Acute Pain Service as well as consideration of regional techniques and Intensive Care

A case of fatal thoracic penetrating injury caused by Hokkaido sika deer antlers.

The deceased was a man in his 70s who was found near a hunting trap set up close to a farmland. A male Hokkaido sika deer was trap and appeared to have attacked the man. Autopsy findings included lacerations, skin abrasions, and subcutaneous muscle hemorrhage on the left side of the man's body. The deepest wound was a laceration on the upper anterior chest. Further examination revealed a dislocation of the right sternoclavicular joint, separation at the sternal attachment of the right first rib, laceration on the anterior surface of the ascending aorta, and surrounding mediastinal hematoma. Thoracic Packing Failure due to Occult Diaphragmatic Injury after Empyema Surgery.

INTRODUCTION: Intrathoracic gauze packing is an established damage control technique for life-threatening thoracic hemorrhage in trauma settings. However, its role in nontraumatic coagulopathic bleeding remains unclear, and the mechanisms underlying packing failure have not been well characterized. We report a case in which intrathoracic packing failed due to an unrecognized diaphragmatic injury, highlighting an important limitation of this strategy. CASE PRESENTATION: An 82-year-old man with chronic kidney disease, diabetes mellitus, and hypoalbuminemia underwent emergency video-assisted tho

Complete High Thoracic Spinal Cord Injury Causes Bowel Dysfunction in Mice.

Bowel dysfunction, is a prevalent and life-impacting comorbidity of spinal cord injury (SCI) with no long-term treatment available. SCI-induced colon changes including motility and fibrosis are understudied as are strategies to address SCI bowel dysfunction. This need remains partly due to the lack of a mouse model that recapitulates the human condition. We hypothesized that a high thoracic spinal transection in mice would trigger bowel dysfunction with coincident colon pathology similar to humans and rats after SCI. We observed bowel dysfunction as increased fecal pellet numbers within the co

Multidisciplinary management of thoracic aortic injury with hemodynamically unstable pelvic fracture in the elderly.

Blunt thoracic aortic injury (BTAI) is a critical clinical emergency often presenting as severe trauma. The early diagnosis and treatment of BTAI pose significant challenges in its management, especially when patients experience early-stage hemodynamic instability. Presently, in the context of multiple traumas, there is an absence of clearly-defined and comprehensive guidelines capable of accurately determining the optimal timing and sequence of interventions for aortic injuries. This issue is particularly prominent when aortic injuries are complicated by abdominal vascular injuries, which exa

A 12-year experience in the management of blunt thoracic aortic injury in Otautahi Christchurch.

BACKGROUND: Blunt thoracic aortic injury (BTAI) is associated with a high mortality and is the second most common cause of death from trauma. The approach to major trauma, imaging technology and advancement in endovascular therapy have revolutionised the management of BTAI. Endovascular therapy has now become the gold standard technique replacing surgery with its high mortality and morbidity in unstable patients. We aim to assess the outcomes following management of BTAI. METHOD: This is a retrospective study of all patients with BTAI between 1 January 2010 and 1 January 2022. Data were obtai

Obstructive shock induced by internal thoracic artery injury with traumatic sternal fracture.

Internal thoracic artery (ITA) injuries associated with sternal fractures can lead to shock. Several studies have documented injuries resulting in hemorrhagic shock, yet there is limited reporting on obstructive shock. Opinions differ regarding which is superior between transcatheter arterial embolization (TAE) and open thoracotomy. We report the case of an 80-year-old female patient presented with blunt chest trauma when driving. Her vital signs were normal. However, ultrasonography revealed a hypoechoic anterior mediastinal lesion. Her blood pressure decreased immediately before undergoing a

Spinal Cord Injury Risk in Open Repair for Descending Thoracic and Thoracoabdominal Aneurysm.

Objective: In open repair for descending thoracic aortic aneurysm (DTAA) or thoracoabdominal aortic aneurysm (TAAA), the influence of re-interventions on spinal cord injury (SCI) remains unclear. This study evaluated the relationships between re-interventions, atherosclerosis, and SCI. Methods: We retrospectively reviewed 78 patients who underwent open surgical repair for DTAA or TAAA between April 2011 and May 2023. The associations of SCI with (i) re-interventions with a history of endovascular therapy and graft replacement and (ii) atherosclerotic factors, including monocyte count, triglyce

Successful surgical management of a combined abdominal and thoracic penetrating injury: a case report.

Penetrating rebar injuries are extremely rare occurrences, but they are very life-threatening, particularly when involving the thoracic and abdominal cavities. The surgical approach to these traumatic injuries depends upon the length and diameter of the rebar as well as the path of penetration into the abdominal and thoracic regions. Due to the highly uncommon occurrence of penetrating rebar injuries, there is very limited information and studies pertaining to this topic in the literature. In this case report, we present a 43-year-old male patient sustaining a rebar penetrating injury, with th

Ultrasonography in thoracic and abdominal stab wound injury: results from the FETTHA study.

BACKGROUND: While the role of Extended Focused Assessment with Sonography in Trauma (eFAST) is well defined in the management of severe blunt trauma, its performance in injuries caused by stab wounds has been poorly assessed. METHODS: Prospective single centre study which included all patients with stab wounds to the thorax or abdomen between December 2016 and December 2018. All patients underwent initial investigation with both eFAST and CT scan, except in cases of haemodynamic or respiratory instability, and in cases with a positive diagnosis by eFAST in which case surgery without CT scan w

A Rare Case of Bilateral Neurogenic Thoracic Outlet Syndrome Following Neck Injury.

Thoracic Outlet Syndrome (TOS) is a rare condition caused by compression of the neurovascular structures of the thoracic outlet. Patients with TOS are found to have a greater prevalence of supernumerary or abnormal first ribs compared to the general population. Symptoms can sometimes be incited by a neck injury, especially in patients with abnormal cervical anatomy. This case report highlights a unique patient with bilateral cervical ribs who presented with complicated symptoms following a neck injury who required a series of surgical interventions to improve.

Uncontrollable Bleeding Managed by Thoracic Gauze Packing in Blunt Traumatic Aortic Injury.

Blunt traumatic aortic injury is a deadly phenomenon in traumatic injuries. Damage control is essential in the management of traumatic patients. We reported a 20-years-old man brought to the trauma center from a car turnover crash scene. Blunt traumatic aortic injury grade III was revealed by contrast-enhanced computed tomography requiring urgent intervention. After an uneventful open repair of the aorta with Dacron graft, diffuse blood oozing occurred from the mediastinum and left pleural cavity. We packed the thoracic bleeding sites and removed the packings after four days. The patient devel
