Standard video-assisted thoracoscopic surgery has been reported as a minimally invasive approach alternative to sternotomy for management of myasthenia gravis (MG) associated with thymoma or thymic hyperplasia. Uniportal video-thoracoscopy is an evolution of standard multi-portal video-thoracoscopy for management of several thoracic diseases but its role for resecting mediastinal tumor remains under-evaluated. Herein, we describe our experience with bilateral uniportal thoracoscopic sequential extended thymectomy with case and video illustrations.

Between, 2000 and 2015, for 200 patients video-assisted thoracic surgery (VATS)-patent ductus arteriosus (PDA) has been performed, including 130 infants and children, and 70 low birth weight babies. Robotically assisted procedure was used in 105 patients. The mean age was 10 months, and body weight was 5.3 kg, ranged from 420 g to 61 kg. The mean procedure time was 48 min, ranged from 20 to 180 min. The size of PDA was around 5 mm in diameter. Conversion to thoracotomy was 4 cases. Carinal reconstruction and omental flap harvesting are traditionally performed through open approaches. We report a case in which carinal reconstruction with bronchial flap and omental flap reinforcement was performed using minimally invasive approaches. The omental flap was harvested laparoscopically and wrapped around the anastomosis, which reduced the risk of airway anastomosis complications. We report a 64-year-old male with iatrogenic innominate artery haemorrhage during video-assisted thoracic surgery (VATS) lymph node dissection, who underwent successful thoracoscopic repair. For centres with experienced minimally invasive thoracic surgeons and rapid multidisciplinary support, thoracoscopic repair of such an injury is feasible and can avoid thoracotomy.

Hereditary multiple exostoses is a rare autosomal dominant condition resulting in the development of multiple osteochondromas. Between 1976 and 1994 we performed 26 thoracic sympathectomies for treatment of therapy-resistant palmar hyperhidrosis. Until the end of 1992 the operation was performed using an open transaxillary approach, since 1993 sympathectomy was done by video-assisted thoracoscopy. Single Port Video-Assisted Thoracic Surgery (VATS) is now the standard of care for many pulmonary operations. The adoption of Single Port VATS for mediastinal surgery has been considerably slower. Nonetheless, intercostal, subxiphoid, and robot-assisted single-port surgery approaches for mediastinal conditions have been explored in recent years. This narrative review looks at the variety of techniques described and offers an appraisal of the clinical evidence surrounding them.

Thymoma has malignant potential and is the most common anterior mediastinal tumor. Video-assisted thoracic surgery (VATS), which is less invasive surgical procedure, is a good option for resecting Masaoka stage I tumors. Whether VATS is appropriate, depends on the surgeon's judgment and accurate imaging diagnosis. We introduce a technique involving a combination of video-assisted thoracoscopic thymectomy (VAT-T) and lateral thoracotomy for stage II and some stage III thymomas that have locally invaded the lung and/or pericardium.

A 60-year-old woman underwent a video-assisted thoracoscopic wedge resection of intralobar pulmonary sequestration instead of a lobotomy because the lesion was localized in the right basal segment. Preoperative 3-dimensional computed tomography was useful for identifying an aberrant artery arising from the thoracic aorta and distributing to the lesion. A successful outcome more than 4 years after the surgery indicates that a wedge resection under video-assisted thoracoscopy may prove to be a therapeutic option for localized pulmonary sequestration.
