A series of 85 thymis biopsies and specimens from thymectomies were re-examined. Forty-five of these were taken from patients with myasthenia gravis. A relatively low frequency of thymitis (57%) was found in these patients, while the frequency of thymoma (9%) was as expected. Two cases of thymitis in thyrotoxicosis were included in the material. In all, 22 thymomas of various kinds (18% of the total material) were diagnosed. A fairly low proportion of these thymomas (18%) were from patients with myasthenia gravis.

Many long-term, follow-up studies have shown that steroids are effective in patients with myasthenia gravis (MG) who have undergone thymectomy. However, few long-term studies have documented the response of MG to steroids alone. We describe two patients who successfully resolved the symptoms of MG without myasthenic crisis or serious side effects by steroid treatment without thymectomy or other alternative therapies for more than 19 years. Our experiences raise an open question of the role of thymectomy or steroid treatment alone, especially in MG patients without thymoma.

OBJECTIVE: To evaluate the clinical value of video-assisted thoracoscopic extended thymectomy for myasthenia gravis(MG).
METHODS: Sixty-two MG cases underwent video-assisted thoracoscopic extended thymectomy. The operative extension included thymus tissue and adipose tissue in anterior superior mediastinum.
