Ultrasound appearances in de Quervain's subacute thyroiditis with long-term follow-up.

In order to determine whether permanent echo abnormalities are produced by subacute thyroiditis, ultrasound examination of the thyroid was performed repeatedly after a mean interval of 23 months in 17 subjects (8 women and 9 men) aged 31-52 (mean 41) years, suffering from subacute thyroiditis. The diagnosis was based on typical clinical symptoms, and cytological confirmation was obtained in all subjects but one. In the primary phase the ultrasonic finding was in all cases abnormal, showing either diffuse or focal hypoechogenicity and enlargement of thyroid lobes. At follow-up, a homogenous ech

CA 19-9 expression in subacute (de Quervain's) thyroiditis: an immunohistochemical study.

Carcinoma antigen 19-9 (CA 19-9) and carcinoembryonic antigen (CEA) expression was immunohistochemically investigated in 48 cases of subacute granulomatous (de Quervain's) thyroiditis, two of focal lymphocytic thyroiditis, three of Hashimoto's thyroiditis, two of Graves' disease, and seven follicular adenomas, 27 follicular carcinomas, and eight papillary carcinomas of the thyroid. CA 19-9 expression was found in all cases of subacute thyroiditis, lymphocytic thyroiditis, and papillary carcinomas examined and in approximately 50% of follicular adenomas and carcinomas. The strongest CA 19-9 sta

Susceptibility to subacute thyroiditis is genetically influenced: familial occurrence in identical twins.

Subacute thyroiditis is thought to be virally induced in genetically predisposed individuals because a strong association has been suggested recently between HLA-B35 and patients in whom subacute thyroiditis has developed. Two identical twin brothers were seen at our clinic with the same symptoms and date of onset of hyperthyroidism and enlargement and tenderness of the thyroid, which gave us a unique opportunity to study the genetic predisposition and treatment of this thyroid disease. Diagnostic criteria for subacute thyroiditis were met in both twins, including hyperthyroxinemia, suppressio

Iodine-induced subacute thyroiditis with thyrotoxicosis presenting as fever of unknown origin.

A 26-year-old woman with features of bulimia nervosa presented with fever of unknown origin, hepatomegaly, marked leukocytosis, and increased erythrocyte sedimentation rate. Following prolonged observation, slight tenderness over the thyroid gland and signs of thyrotoxicosis occurred. A thyroid scan demonstrated no isotope uptake and the patient admitted abusing an organic iodine preparation in order to control her weight. The diagnosis of iodine-induced subacute thyroiditis with thyrotoxicosis was, therefore, considered. A brief course of low-dose steroids normalized both thyroid function and

[The value of immunologic tests in predicting the outcome of subacute thyroiditis].

Relapses of subacute thyroiditis, hypothyrosis or autoimmune thyroiditis more often develop in the patients with manifest immune shifts after therapy than in those with normal or moderately shifted immune status; this factor appears to be useful for the prediction of the disease outcome and for planning the treatment strategy.

[T-lymphocyte subpopulations in autoimmune and subacute thyroiditis treated with T-activin].

The results of examination of 15 patients with autoimmune and subacute thyroiditis are analyzed. T-cellular immunity parameters have been examined, i.e. total T-lymphocytes, T-suppressors, T-helpers, killers. The study has revealed a reduction of the total T-lymphocyte count and a shifted ratio of the T-lymphocyte subpopulations, these changes being more manifest in autoimmune thyroiditis. T-activin++ therapy resulted in a manifest tendency to normalization of the counts of T-lymphocytes and their subpopulations.

Carbohydrate metabolism in subacute thyroiditis: effect of acute elevation of thyroid hormones.

Carbohydrate metabolism was studied in nine patients with subacute thyroiditis before treatment and after recovery. The mean fasting free fatty acid level before treatment was significantly higher than that after the recovery, although the mean fasting blood glucose and insulin levels were not significantly different. Seven out of nine patients showed glucose intolerances. They returned to normal after the recovery except two cases with family histories of diabetes mellitus, whose glucose intolerances remained even 6 months after the recovery. Glucose area during oral glucose tolerance test wa

Lessons of the month 1: A diagnostic dilemma: thyroid abscess or subacute thyroiditis?

Thyroid abscess is rare but potentially fatal, whereas subclinical thyroiditis is a self-limiting condition that is more commonly seen. The clinical features of both diseases can be overlapping but the treatment approach differs. We report a case of a painful left thyroid nodule not responding to conventional therapy for thyroiditis or thyroid abscess.

Association of the ultrasonographic findings of subacute thyroiditis with thyroid pain and laboratory findings.

We aimed to determine whether ultrasonography is a useful diagnostic tool by correlating its findings with biological data of patients with subacute thyroiditis (SAT). Thirty-two SAT patients were evaluated in a retrospective study. Thirty-one patients (96.9%) had tenderness, 14 (43.8%) had localized pain, and 11 patients (34.4%) had radiating pain during a state of SAT. With ultrasonography, we found 51 hypoechoic areas in 32 patients. The hypoechoic volume per unilateral thyroid gland (%) was significantly larger in areas accompanied with pain (P<0.001). Out of 27 patients measured, 18 (67%)

Changes in thyroid hormones by treatment with aspirin and prednisolone in subacute thyroiditis with hyperthyroidism.

Thyroid functions were studied in 11 patients with subacute thyroiditis accompanied by signs and symptoms of hyperthyroidism, and were compared with 13 patients with untreated thyrotoxicosis in which serum T4 was elevated to the identical level. Serum T3 was also elevated in subacute thyroiditis but to a significantly lower extent than in thyrotoxicosis. Therefore the ratio of T4/T3 was significantly higher in subacute thyroiditis than in thyrotoxicosis. Although duration of thyroid swelling was shorter in the group treated by prednisolone than by aspirin, the accelerated ESR, thyroid tenderne

Lymphocyte transformation in response to human thyroid extract in patients with subacute thyroiditis.

Seven patients with subacute thyroiditis were studied for evidence of cell-mediated immunity (CMI) to human thyroid extract, as judged from lymphocyte transformation responses, measured by the increase in labeled thymidine uptake in vitro. Significant transformation was observed in lymphocytes from five of the seven patients with active subacute thyroiditis, and in the lymphocytes from only three of 15 patients with Graves disease (x2, P EQUALS 0.036). Repeated studies of lymphocyte transformation were done in four of the patients with subacute thyroiditis, three of whom initially showed abnor

Incomplete thyrotroph suppression determined by third generation thyrotropin assay in subacute thyroiditis compared to silent thyroiditis or hyperthyroid Graves' disease.

Serum TSH concentrations were determined by both second and third generation assays in three types of thyrotoxicosis associated with subacute thyroiditis, silent thyroiditis, and hyperthyroid Graves' disease at the time of each patient's initial visit to the clinic. Serum TSH concentrations as measured by the second generation assay with an analytical sensitivity of 0.04 mU/L were below the detection limit in every patient. In contrast, serum TSH concentrations as measured by the third generation assay with an analytical sensitivity of 0.009 mU/L were below the detection limit in 18 of 21 (86%
