Oral manifestations of gastrointestinal diseases in children. Part 3: Ulcerative colitis and gastro-oesophageal reflux disease.

Alterations of the oral cavity are common in children: 22% of children aged less than 4 years and 44% of those aged more than 12 develop dental erosion, 9-95% of children in Europe and in North America develop gingivitis, with adolescents showing a prevalence of more than 60% [Italian Ministry of Health, Guidelines 2013]. Alterations within the oral cavity can be the first sign of systemic diseases and may thus allow for an early diagnosis and treatment. In particular, being the oral cavity a part of the gastrointestinal system, oral alterations can be an expression of a gastrointestinal disea

Awareness and attitudes related to dental erosive wear among 18-yr-old adolescents in Oslo, Norway.

The aim of the present study was to describe awareness and attitudes related to dental erosive wear among 18-yr-old adolescents in Oslo and to explore attitudinal differences between participants with and without the condition. All 18-yr-old subjects scheduled for their routine examination at the Public Dental Service clinics during 2008 (n = 3,206) were invited, and 1,456 agreed to participate (a response rate of 45%). The data were collected using structured questionnaires and by clinical examination of the participants. Dental erosive wear was assessed using a pictorial manual - the Visual

Saliva: a powerful diagnostic tool for minimal intervention dentistry.

UNLABELLED: Saliva plays a vital role in oral health as patients strive to maintain a healthy dentition throughout their lives. It is natures primary defense mechanism for the oral environment, and is particularly important for protecting exposed tooth surfaces. While internal protection for dentin comes from odontoblasts and the dental pulp, the body's external protection for enamel comes from saliva. The noninvasive nature of salivary testing has made it an effective alternative to blood and urine testing and home testing kits have made it possible for people to monitor their own health usin

Dental erosion and upper gastrointestinal disorders.

The orodental status, particularly dental erosions, of 109 patients with upper gastrointestinal symptoms was examined. In 44 patients, the underlying pathosis was associated with increased acid output in the stomach (reflux esophagitis or duodenal ulcer), while in 48 patients who underwent cholecystectomy, the duodenogastric reflux was alkaline. In 17 patients with gastric ulcer, the gastric secretion was usually normal. The diagnoses were made with gastroscopy. Seven patients with dental erosion were found, and they all came from the group of 35 dental patients with reflux esophagitis or duod

Rumination Syndrome and Dental Erosions in Children.

Rumination syndrome is the effortless regurgitation of recently ingested food with subsequent reswallowing or spitting out. Dental erosion (DE) affects 2% to 5% of the population. DE is defined as loss of tooth structure by a chemical process that does not involve bacteria. Our objective was to compare the frequency of DE among children with rumination syndrome with healthy controls. We enrolled 30 patients 4 to 21 years of age diagnosed with rumination syndrome, and 30 age- and sex-matched healthy control subjects. Patients were evaluated by pediatric dentists for presence of DE with Taji et

[Dental erosion in children and adolescents: gastroenterologic background].

Reflux of acidic gastric secretions into the oral cavity may cause significant dental erosion. This gastro-oesophageal reflux has been shown to occur in children as well as in adults, and is often not accompanied by typical reflux symptoms. When symptoms are present the children may complain of heartburn, food regurgitation and epigastric pain. Current investigation involves ambulatory 24-hour oesophageal pH-measurements. In the case of exaggerated reflux, the main therapeutic option is medical acid suppression for several years. The pathogenesis, the diagnostic and therapeutic procedures as w

Are sugars-free medicines more erosive than sugars-containing medicines? An in vitro study of paediatric medicines with prolonged oral clearance used regularly and long-term by children.

OBJECTIVE: The reduced use of sugars-containing (SC) liquid medicines has increased the use of other dose forms, potentially resulting in more widespread dental effects, including tooth wear. The aim of this study was to assess the erosive potential of 97 paediatric medicines in vitro. METHODS: The study took the form of in vitro measurement of endogenous pH and titratable acidity (mmol). Endogenous pH was measured using a pH meter, followed by titration to pH 7.0 with 0.1-M NaOH. RESULTS: Overall, 55 (57%) formulations had an endogenous pH of < 5.5. The mean (+/- SD) endogenous pH and titra

An In Vitro Analysis of Erosive and Cariogenic Potential of Commonly Prescribed Antiasthmatic and Antiepileptic Liquid Medications on Permanent Teeth: An SEM-EDX Study.

BACKGROUND: The commonest multifactorial disease affecting mankind is dental caries. Oral pediatric liquid medications (PLMs) are the best line of treatment for young children, as they are available in the form of syrups, suspensions, and solutions. Extended use of liquid medicines has a deleterious effect on dentition in chronic asthmatic patients, sensitive respiratory problems, and epileptic patients, along with frequent sudden conditions such as sinusitis, allergic rhinitis, tonsillitis, and otitis media, and has a deleterious effect on the dentition of young adults. Improving Oral Health with Fluoride-Free Calcium-Phosphate-Based Biomimetic Toothpastes: An Update of the Clinical Evidence.

As the demand for clinically effective fluoride-free oral care products for consumers increases, it is important to document which types of toothpastes have been shown in clinical studies to be effective in improving oral health. In this review, we included different indications, i.e., caries prevention, improving periodontal health, reducing dentin hypersensitivity, protecting against dental erosion, and safely improving tooth whitening in defining what constitutes improvement in oral health. While there are several professional and consumer fluoride-containing formulations fortified with cal

Optimizing smile composition and esthetics with resin composites and other conservative esthetic procedures.

Numerous esthetic deficiencies may be present in natural, restored, or orthodontically enhanced smiles. The most frequent defects are transposed anterior teeth following aplasia, existing or remaining diastemas, form abnormalities and discolorations, abrasion, erosion, and dysplasia. Conservative treatment modalities such as enamel recontouring, bleaching, microabrasion, and resin composites have the potential to correct or improve esthetic problems. These treatments deserve more attention because they have tremendously improved in practicability, efficiency, and predictability. The search for

Morphological study of remineralization of the eroded enamel lesions by tyrosine-rich amelogenin peptide.

BACKGROUND: Tyrosine-rich amelogenin peptide (TRAP) is the main amelogenin digestion product in the developmental enamel matrix. It has been shown to promote remineralization of demineralized enamel in our previous study. However, direct evidence of the effect of TRAP on the morphology and nanostructure of crystal growth on an enamel surface has not been reported. This study aimed to examine the effect of TRAP on the morphology of calcium phosphate crystals grown on early enamel erosion using a pH-cycling model. METHODS: Eroded lesions were produced in human premolars by 30-second immersion i

Efficient prosthodontic treatment in a young patient with long-standing bulimia nervosa: A clinical report.

Time-saving, efficient dental treatment is essential for patients in poor dental condition. This clinical report describes a systematic technique for restoration of the visibly-destroyed dentition of a long-term bulimia nervosa patient, including occlusal vertical dimension increase, with composite resin core foundations and prosthetic rehabilitation with ceramic crowns, in only a few treatment sessions. The efficiency of this procedure is gained from composite resin core restorations that establish the new occlusal vertical dimension (OVD) and replace and form the foundation for the subsequen

Gastroesophageal reflux in children and its relationship to erosion of primary and permanent teeth.

Gastroesophageal reflux (GOR) is often a self-limiting condition but nevertheless is a common cause of morbidity in childhood. Studies of adult patients with hiatus hernias and GOR have suggested that there is an increased incidence of dental erosion in these individuals. The aim of this study was to investigate the relationship between dental erosion and GOR in children. Fifty-three children aged 2 to 16 yr (mean 4.9 yr) with moderate to severe GOR, defined by pH monitoring, were examined for dental erosion. A questionnaire investigating dietary habits, other relevant medical conditions, and

Odontogenic Cutaneous Fistula: First Dermoscopic Description and a Practical Management Framework.

Odontogenic cutaneous fistula (OCF) is an uncommon manifestation of dental infection that is often underrecognized because of its variable morphology and the absence of obvious dental symptoms. Misdiagnosis may lead to unnecessary tests and treatments, whereas a permanent cure depends on the elimination of the offending tooth. We report the case of a 15-year-old female who presented with progressive swelling of the right cheek and fever. Ultrasound revealed a 21.8-cc abscess, which was managed with surgical drainage and a five-day course of amoxicillin-clavulanate. Dermoscopy showed radiating
