Skeletal and dental pathology of free-ranging mountain gorillas.

The mountain gorillas of the central Virungas have been the subject of field study for the last 30 years; however, our understanding of morbidity and mortality in these apes is limited. This paper describes pathological conditions of the skeleton and dentition of these animals and evaluates lesions in relation to behavioral and environmental data. The skeletal remains of 31 mountain gorillas from the Karisoke Research Center were examined for enamel wear, carious lesions, abscesses, periodontal disease, antemortem tooth loss, trauma, inflammation, arthritis, neoplasia, and developmental anomal

Fixed prosthodontic rehabilitation in a wear patient with Fabry's disease.

Fabry's disease is an uncommon X-linked metabolic disorder that leads to abnormal accumulation of glycosphingolipids in the body resulting in a variety of systemic disorders. Few reports have addressed dental findings and management of these patients. This clinical report describes the fixed prosthodontic rehabilitation of an adult male patient with Fabry's disease, who presented with generalized severe wear of the dentition. In addition to numerous systemic morbidities, the patient also presented with intraoral angiokeratomas, telangiactasias, anterior diastemata, bimaxillary prognathism, and

Contemporary materials for the prevention of occlusal carious lesions.

Prevention of occlusal caries has been a longstanding challenge in dentistry, evolving appropriately from invasive mechanical techniques toward increasingly adhesive and bioactive strategies. Early attempts, such as the application of silver nitrate and preventive restorations, were limited by questionable efficacy and unnecessary removal of sound tooth structure. The advent of adhesive dentistry and the acid-etching technique enabled the development of resin-based sealants, providing a minimally invasive and effective method for fissure protection. Over time, sealant materials have evolved th

Temporomandibular Junction Disorders.

Temporomandibular junction disorders (TMD) are a common problem for patients presenting to the primary care office. Symptoms may be acute or chronic. Patients may report a variety of complaints such as: painful clicking at the joint, difficulty opening the mouth or chewing, tenderness in the muscles around the joint, headaches, or tinnitus. Physical examination findings vary and may include palpable tenderness or spasm of the pterygoid muscles, palpable or audible clicking at the joint, wear and tear of tooth enamel, or dental malocclusion. Most TMDs respond well to conservative therapy, but s

[Reduction of trigeminal neuralgia complaints after recovery of the vertical dimension].

A 56-year-old man was referred by his dentist to an orthodontist as part of an interdisciplinary treatment plan for a complete dental rehabilitation. The patient presented with a mutilated dentition with a restricted envelope of function and generalized severe tooth wear with loss of vertical dimension. The patient showed a history of multiple endodontically treated and subsequently extracted elements associated with unexplained symptoms until a neurologist diagnosed trigeminal neuralgia. The orthodontic treatment in which the restricted envelope of function was eliminated and the vertical dim…

Neurologic Disorders and Cavovarus Deformity.

A cavovarus deformity results from muscle imbalances in the foot. There are several etiologies of a cavovarus foot including congenital, neurologic, post-traumatic, and idiopathic. Charcot-Marie-Tooth disease is a common genetic cause of cavovarus foot. History, physical examination, and imaging help determine appropriate treatment. The deformity can be flexible or rigid and can present in children or adults, thus treatment should be individualized to the patient. Non-operative management includes shoe wear modification, physical therapy, and bracing. Operative management consists of soft tiss

Effects of unilateral upper incisor extraction on facial growth of young rats.

The effects of unilateral extraction of the upper incisor (one tooth) on facial growth of young rats were studied. The remaining upper incisor showed deviation towards the opposite side, and there was regular dental wear of all the incisors. The effects were evident only in the mid-facial area. Cephalometric measurements showed significant shortening with deviation of the incisor bone on the extraction side. There was no significant disturbance of the growth of the rest of the maxilla and mandible. These results indicate that upper incisor extractions have a localized influence, leading to imp

Mesiodistal tip expression of mandibular anterior teeth in patients with mandibular incisor extraction treated with Invisalign aligners.

INTRODUCTION: Extraction of a single mandibular incisor may be prescribed in conjunction with clear aligner orthodontic treatment. The accuracy of therapeutic mesiodistal tooth positioning for the remaining mandibular anterior teeth for such patients has not been comprehensively evaluated. This retrospective study compared the predicted and achieved mesiodistal tooth movements in the mandibular anterior region for patients treated with Invisalign (Align Technology, Santa Clara, Calif), along with the influence of aligner attachments and the frequency of aligner change. METHODS: A sample of 83

Risk factors for anterior disc displacement with reduction and intermittent locking in adolescents.

AIMS: To test the hypothesis that oral parafunctions and symptomatic temporomandibulair joint (TMJ) hypermobility are risk factors in adolescents for both anterior disc displacement with reduction (ADDR) and intermittent locking. METHODS: Participants were two hundred sixty 12- to 16-year-old adolescents (52.3% female) visiting a university clinic for regular dental care. ADDR and symptomatic TMJ hypermobility were diagnosed using a structured clinical examination. During the anamnesis, reports of intermittent locking and of several parafunctions were noted, eg, nocturnal tooth grinding, diur

Direct vs indirect restorations for diastema closure: determining the suitable approach.

Determining the appropriate technique for diastema closure is challenging, and the decision must be evidence based. The objective of these case reports is to describe different approaches to diastema closure using direct and indirect techniques, focusing on the characteristics of the patient and clinical requirements to guide treatment. In the first case, a 16-year-old patient had multiple diastemas in the maxillary anterior dentition. The clinical evaluation revealed microdontia of the lateral incisors and malpositioned teeth. The treatment included tooth whitening and placement of composite

Oral health in children with chronic kidney disease.

The review summarizes the current understanding of dental health in children with chronic kidney disease (CKD). Oral conditions associated with CKD and its medical and surgical management have been described in cohort studies. Children with CKD may present with severe developmental defects of enamel (DDE) including discoloration, pitting, and reduced hardness leading to extensive tooth wear with normal function. The alkaline oral pH resulting from the uremia of CKD inhibits cariogenic bacteria, reduces dental caries risk, and increases accumulation of dental calculus. The malnutrition, acidosi…

Reliability of two clinical scoring systems for dental erosive wear.

The aim of the study was to evaluate and compare two dental erosive wear scoring systems, the Visual Erosion Dental Examination (VEDE) and Basic Erosive Wear Examination (BEWE). Seventy-four tooth surfaces (photographs) and 562 surfaces (in participants) were scored by 5 (photographs) or 3 (in participants) clinicians using both scoring systems. The surfaces in the photographs were scored twice. The level of agreement was measured by weighted kappa (kappa(w)). Inter- and intraexaminer agreement showed small variations between the examiners for both systems when scoring the photographs. Slightl
