Endoscopic lysis of anterior glottic webs and silicone keel placement.

OBJECTIVES: Acquired anterior glottic webs occur most commonly after endoscopic resection of laryngeal papilloma involving the anterior vocal folds. Treatment of anterior glottic webs has included a tracheotomy with laryngofissure and placement of a laryngeal stent or keel. We have used an endoscopic technique of web lysis and placement of a laryngeal keel without tracheotomy over the past 7 years. METHODS: A retrospective chart review was conducted of all cases of endoscopic web lysis and keel placement performed by the senior author (S.A.B.). RESULTS: Over the past 7 years, 10 patients und

Electrophysiologic monitoring of the recurrent laryngeal nerves may not predict bilateral vocal fold immobility after thyroid surgery.

Two cases of bilateral vocal fold immobility (VFI) after identification and preservation of the recurrent laryngeal nerves (RLNs) required tracheotomy until vocal fold recovery. The first patient underwent thyroid surgery without preoperative or postoperative evaluation of the vocal folds, administration of postoperative intravenous steroids, or electrophysiologic monitoring of the RLNs, whereas the second patient underwent a thyroid procedure in which all of the aforementioned were executed. Preoperative and postoperative clinical evaluation of the RLNs is strongly suggested in patients under

[Complete disruption of cervical trachea, whose distal airways were migrated into the mediastinum].

The first case was a 55-year-old man, who suffered by a rope while driving his motor bicycle. On 7th day after injury, tracheotomy was scheduled due to progressive dyspnea. Following intubation of a endotracheal tube, his trachea was ruptured. The second case was a 16-year-old man, who was stabbed his trachea with a sword by his mother. His trachea completely separated following coughing during the examination of bronchoscopy. For 2 cases, we immediately excised their necks for tracheotomy but couldn't find their distal portion of trachea, because they were migrated into the mediastinum. We in

[Laryngotracheobronchial involvement in relapsing polychondritis].

OBJECTIVE: To investigate the clinical cause, treatments and prognosis of laryngotracheobronchial involvement in relapsing polychondritis (RP). METHODS: Thirteen patients of RP with laryngotracheobronchial involvement were review retrospectively. RESULTS: From 1983 to 1998, 20 patients with RP were managed in this hospital. Of them, 13 (65%) involving the laryngotracheobronchial region were observed. Respiratory tract lesions might be diffuse or localized and occurred in larynx (46%), trachea (69%) and bronchus (69%). Corticosteroids, immunosuppressive agents and dapsone were used in these p

Very Late-Onset Myasthenia Gravis Presenting With Dysphagia and Gradual Decrease in Laryngeal Elevation During Repeated Swallowing: A Case Report.

Very late-onset myasthenia gravis (VLOMG) is rare myasthenia gravis (MG) that begins after the age of 65 years. Here, we describe a 72-year-old patient who presented with dysphagia. Upper gastrointestinal radiography revealed delayed initiation of swallowing in the pharynx and inadequate opening of the upper esophageal sphincter. Notably, there was a gradual decrease in laryngeal elevation, which strongly suggested MG. Additionally, the frequency of swallowing was reduced, and anti-acetylcholine receptor (anti-AChR) antibodies were positive. Due to type I respiratory failure, the patient requi

Rehabilitation protocol of dysphagia after subtotal reconstructive laryngectomy.

Dysphagia is a constant complication of subtotal reconstructive laryngectomy, due to modifications in the anatomy and in sensitivity of the larynx and pharynx. The reduced sphincteric activity of the larynx can enhance aspiration with a higher risk of pneumonia. In our opinion, the presence of the tracheotomy tube in the first weeks after surgery interferes with proper mobility of the laryngo-tracheal axis during swallowing, as it anchors the trachea to the skin. We have conducted swallowing rehabilitation, without the tracheotomy tube, ready to aspirate eventual saliva or food debris dropping

High pulmonary risk scoliosis surgery: role of noninvasive ventilation and related techniques.

OBJECTIVE: There is little awareness among surgeons of the potential for noninvasive mechanical ventilation as an alternative to prolonged endotracheal intubation or tracheostomy for patients with neuromuscular scoliosis and ventilatory failure. These methods have not been reported for the perioperative management of scoliosis correction in patients with an inability to sustain their alveolar ventilation. METHODS: Five children with flaccid scoliosis secondary to muscular dystrophy or spinal muscular atrophy who had very high pulmonary risk were preoperatively trained in the use of noninvasiv

Bilateral vocal fold paralysis and adhesion in anterior spinal artery syndrome.

The purpose of this report is to present a rare case of anterior spinal artery syndrome (ASAS) in which there proved to be a combined lesion of paralysis and adhesion. A 26-year-old woman with a history of ASAS complained of difficulty of tracheal decannulation. In 1988, she was intubated and underwent tracheotomy because of respiratory muscle weakness, and she was decannulated in 1990. In 1998, she had cesarean delivery under general anesthesia, and postdelivery dyspnea necessitated tracheotomy again. On her first visit to us, endoscopic examination revealed bilateral vocal fold immobility at

A functional approach to lateral fixation in bilateral abductor cord paralysis.

A modification of the Clerf arytenoidopexy treatment for bilateral vocal cord paralysis is described; it consists of an external approach with complete unilateral division of the posterior cricoarytenoid and interarytenoid muscles. The entire arytenoid is next mobilized and tilted laterally, then fixed into position along the posterior margin of the thyroid cartilage by two or three permanent retention sutures. Careful placement of these sutures renders the laryngeal mucosa maximally undisturbed and is largely responsible for the predictable results achieved with this technique. During the pas

Conservative and surgical treatment of acute posttraumatic tracheobronchial injuries.

BACKGROUND: Acute posttraumatic tracheobronchial lesions are rare events associated with significant morbidity and mortality. They are caused by blunt and penetrating trauma, or they are iatrogenic, appearing after intubation or tracheotomy. Although surgery has traditionally been considered the treatment of choice for these injuries, recent reports show that conservative treatment can be effective in selected patients. The aim of this study was to evaluate the role of surgical and conservative management of these lesions, differentiated on the basis of clinical and endoscopic criteria. METHO

Risk factors analysis and prediction model construction of LRTI in head and neck cancer patients with tracheostomy based on subglottic sputum aspiration volume.

OBJECTIVE: Most head and neck cancer (HNC) patients had postoperative aspiration and even lower respiratory tract infections (LRTI). This study aimed to investigate the association between subglottic sputum aspiration volume (SSAV) and the onset of LRTI in HNC patients with tracheostomy. We further sought to identify independent risk factors and construct a predictive model for postoperative LRTI in this patient population. METHODS: This study retrospectively enrolled 235 HNC patients with intraoperative tracheotomy from June 2018 to November 2022. Subglottic sputum aspiration volume (SSAV) a

Predictors of pharyngeal electrical stimulation treatment success in tracheotomised stroke patients with dysphagia: Secondary analysis from PHADER cohort study.

Pharyngeal electrical stimulation (PES) has emerged as a promising intervention for neurogenic dysphagia, with potential benefits in reducing dysphagia severity in stroke patients. PES may facilitate decannulation in tracheotomised stroke patients with dysphagia, yet the predictive factors for treatment success have not been investigated in detail. This study used data from the PHAryngeal electrical stimulation for treatment of neurogenic Dysphagia European Registry (PHADER) study to identify predictive factors for PES treatment success among patients with post stroke dysphagia who required me

Laser arytenoidectomy in the management of bilateral vocal cord paralysis in children.

OBJECTIVE: To analyse the efficacy of CO(2) laser arytenoidectomy in the management of bilateral vocal cord paralysis in children. METHODS: Retrospective series of 17 patients who underwent laser arytenoidectomy for bilateral vocal cord between 1995 and 2008 in a tertiary care institution. All patients had bilateral laryngeal paralysis, in isolation (n=5) or associated with concomitant airway conditions (n=12). All cases had anterior prolapse of the arytenoids with partial obstruction of the airway on inspiration. 12/17 patients (70.5%) were tracheotomy-dependant, 2/17 were in-extubatable, an

Case series: Montgomery T-tube placement for subglottic tracheal stenosis: a report of 3 cases.

RATIONALE: Subglottic tracheal stenosis is stenosis of the trachea between the vocal cords and the lower margin of the cricoid cartilage. The Montgomery T-tube is used as a tracheostomy tube and a combined tracheal stent to avoid postoperative tracheal stenosis. PATIENT CONCERNS: Because the stenosis is close to the glottis, surgical treatment is complex, and many complications may arise. DIAGNOSES: Subglottic tracheal stenosis. INTERVENTIONS: The patients underwent endotracheal intubation or tracheotomy because of acute pancreatitis, laryngeal malignancy, or cerebral hemorrhage after endot

Endoscopic laser medial arytenoidectomy for treatment of bilateral vocal fold paralysis.

Endoscopic laser medial arytenoidectomy for bilateral vocal fold paralysis has the advantage of preserving the structure and the position of the vocal fold, contrary to a transverse cordotomy or total arytenoidectomy. Our objective was to evaluate the functional results of this procedure. This is a prospective non-randomized study. Twenty patients were included: five patients had a tracheotomy preoperatively and 15 patients had dyspnea on exertion. Acoustic voice measurements, spirometric parameters and the voice handicap index 120 (VHI), were evaluated 1 week before surgery and 3 months after
