Tractional detachment of the macula following branch retinal vein occlusion.

In two patients, a detachment of the macula occurred following a branch retinal vein occlusion. Vitreoretinal relationships were investigated and documented by biomicroscopic photography. Both eyes showed partial posterior vitreous detachment with dense adhesions to a preretinal membrane at the macula. Vitreous contraction and traction appeared to be responsible for the detachment.

[Comminuted fractures of the lower end of the radius. Treatment by traction-suspension].

Treatment of comminuted fractures of the distal radius bone by the traction-suspension technique is both simple and innocuous. The authors describe this old and satisfactory technique, slightly modified: a transmetacarpal pin is introduced and suspended to a giving set stand, and a counter-traction of 3 to 4 kg is exerted by means of a transulnar pin. After remodelling of the bone under general anaesthesia and radiographic control, the pins are sunk in plaster to be removed 6 to 8 weeks later. Excellent, often unexpected clinical and radiological results can be obtained, and complex traumatic …

The use of modified Neufeld traction in the management of femoral fractures in polytrauma.

The choice of treatment modalities for the fractured femur in the polytrauma patient is a continuing dilemma. Prevention and treatment of respiratory complications requires maximum patient mobility. Conventional traction methods restrict turning in bed and preclude moving the patient out of bed. Immediate open reduction results in additional blood loss, tissue trauma, and a higher rate of infection and nonunion. Immediate closed intramedullary nailing is an excellent method but requires special equipment and surgical expertise. To meet the mobilization requirements of good pulmonary care and a…

Auto-traction for treatment of lumbago-sciatica. A multicentre controlled investigation.

In this controlled prospective study of the Auto-traction method for the treatment of lumbago-sciatica, 82 patients were randomly allocated to either treatment with Auto-traction for up to three 1-hour sessions in 1 week, or they were given a corset and advised to rest. The orthopaedic surgeons participating in the study worked at six different hospitals and all had limited experience of the Auto-traction method obtained during a 1-week course. All patients were clinically evaluated by an independent observer who also performed the follow-up examinations 1 and 3 weeks after the treatment sessi…

[Prosthesis control by traction instead of myoelectricity in upper arm amputations].

This paper is concerned with the provision of an upper limb prosthesis in the presence of an axillaris nerve paralysis and additional injuries such as clavicular and scapular fractures, dislocation of the shoulder and major psychological changes. Such paralysis and injuries exclude the use of a myoelectric prosthesis. The shoulder muscles were trained to provide traction forces which controlled an MGH-S hand (VIENNATONE).

Posterior scleral contraction to treat myopic traction maculopathy at different stages.

OBJECTIVE: We aimed to evaluate anatomical and functional outcomes and determine posterior scleral contraction (PSC) timing in treating myopic traction maculopathy (MTM). METHODS: This is a retrospective study of MTM patients undergoing PSC with genipin cross-linked strip at a single hospital site. Differences in demographic and clinical characteristics were compared among three groups defined by the MTM staging system. All patients were followed up for at least one year postoperatively. The best-corrected visual acuity (BCVA), macular hole diameter, axial length (AL), optical coherence tomog…

Idiopathic Vitreomacular Traction Managed with Initial Observation: Clinical Course and Outcomes.

OBJECTIVE: To review the clinical course and outcomes of patients with idiopathic vitreomacular traction (VMT) managed initially by observation. DESIGN: Retrospective chart review including patients with idiopathic VMT based on clinical symptoms and findings on OCT between January 1, 2015, and February 15, 2021. SUBJECTS: The study included 436 eyes of 317 patients with a mean age of 72.2 years ± 8.9 at initial visit and mean follow-up time of 34 months ± 19.2. METHODS: Vitreomacular traction severity grade was ascribed to each patient using previously published grading criteria. Grade 1 de…

Traction therapy in lumbar disc hernias: A finite element analysis study.

OBJECTIVES: This study aims to evaluate the results of lumbar traction treatment at different traction angles and different traction forces using the finite element analysis (FEA). MATERIALS AND METHODS: Computed tomography (CT) images of a healthy 35-year-old male patient who had no history of trauma or fracture were modeled in three-dimensional (3D) with Mimics® software for the lumbosacral spine model. Ligaments and discs were created on the 3D spine model in the SolidWorks® program. The obtained model was sent to the ANSYS version 18 software, and analyses were done non-linearly. All anal…

Neuromuscular lordoscoliosis: an unusual response to post-operative halo-gravity traction.

PURPOSE: To report the results of prolonged post-operative halo-gravity traction in a patient in whom the surgery had to be interrupted unexpectedly and for whom subsequently specific clinical circumstances contraindicated completion of the surgical procedure. METHODS: The patient was a 15-year-old male with severe cervico-dorsolumbar lordoscoliosis who was being studied for associated diffuse axonal injury. He performed halo-gravity traction for 12 weeks. Subsequent surgical management consisted of occipito-lumbar posterior instrumented fusion. During the surgical approach, electrocardiograp…

Mesenteric traction syndrome in pigs: A single-blinded, randomized controlled trial.

BACKGROUND: Mesenteric traction syndrome is commonly observed in patients undergoing upper abdominal surgery and is associated with severe postoperative complications. A triad of hypotension, tachycardia, and facial flushing seems provoked by prostacyclin (PGI2) release from the gut in response to mesenteric traction. The administration of nonsteroidal anti-inflammatory drugs (NSAID) inhibits PGI2 release, stabilizing the hemodynamic response. Here, we examined the effect of mesenteric traction on splanchnic blood flow in pigs randomized to NSAID or placebo treatment. MATERIALS AND METHODS: T…

Uniportal thoracoscopic surgical management using a suture traction for primary pneumothorax.

BACKGROUND: We devised a new strategy using suture traction to facilitate the management of primary spontaneous pneumothorax during uniportal thoracoscopy. To test its validity, we compared the outcomes of our modified technique with those of conventional three-port thoracoscopy. METHODS: This retrospective study included all 43 consecutive patients with primary spontaneous pneumothorax undergoing thoracoscopy between January 2017 and December 2019. They were divided within two groups: uniportal thoracoscopy using suture traction (n = 21) and conventional 3-port thoracoscopy (n = 22). Postope…

Novel probabilistic model of core vitreous traction using microsurgical vitrectomy tools.

PURPOSE: Traction exerted on the vitreous base during vitrectomy poses a risk for retinal tears. We aimed to quantify core vitreous traction during vitrectomy using spring return and pneumatic cutters. METHODS: Juvenile porcine vitreous was vacuum held in a vitreous bath while traction was measured using precision force gauge during vitrectomy. The parameters included were aspiration rate, cut-rate, cutter size, and machine types. RESULTS: An empirical probabilistic model was developed. The traction was proportional to the aspiration rate but insignificantly dependent on the cut-rate. The tr…

OBJECTIVE: Correction of the caudal septum deviation is the most difficult part of the septoplasty and a common cause of revision septoplasty. The purpose of this study was to present authors' preliminary results in the treatment of patients with caudal septal deviation using the septal cartilage traction suture technique. STUDY DESIGN: Prospective, single center, observational study.
