Trauma and addiction experiences of African American women.

Substance abuse affects African American women at an increasingly alarming rate. The interaction of substance abuse and traumatic events requires exploration. For many African American women, early life trauma plays a critical role in how and when they fall victim to the ravages of substance abuse. This article about a phenomenological study of 15 African American women uses intense narratives to disclose experiences of incest, rape, abuse, and other horrors that led to drug and alcohol use to extinguish pain. The themes include family history of substance abuse, lack of a caring childhood env

Nursing care of posttraumatic stress disorder after anesthesia awareness.

Anesthesia awareness can cause adverse psychological symptoms in patients after surgery. Although rare, posttraumatic stress disorder (PTSD) has been documented in surgical patients who have a history of previous trauma or maladaptive coping patterns. Plastic surgical nurses should assess all patients for anesthesia awareness postoperatively, recognize the diagnostic criteria of PTSD, and offer support of psychiatric referrals if indicated.

Applying an Abolitionist Framework to Understand and Address Mental Health Concerns Among Transgender and Gender-Diverse Populations.

This article explores the mental health concerns of transgender and gender diverse (TGD) individuals within a mutual aid and harm reduction framework. Despite historical strides toward equity and care in the public health sector, TGD people face structural discrimination, polyvictimization, and worsening political hostility that heighten mental health disparities. The authors critique medical and nursing practices shaped by carceral logic and call for new approaches rooted in equity and care. Clinical recommendations emphasize affirming, trauma-informed, and community-based care, while highlig

A Model to Predict Birth Stress in Adolescents Within 72 Hours of Childbirth.

An organizing framework for understanding adolescent birth stress in immediate postpartum does not exist. Researchers evaluated adolescent birth stress within 72 hours postpartum via a modification of Slade's conceptual model of risk factors for posttraumatic stress (PTS). Birth stress was defined by negative birth appraisal and subjective distress. Precipitating factors pain management, partner presence, and delivery type, plus maintaining factor infant complications, predicted negative birth appraisal. Predisposing factors depression and prior trauma predicted subjective distress. Findings s

Psychiatric and Behavioral Characteristics Among Male Adolescents With Sexually Acting Out Behaviors Admitted to a Residential Treatment Facility.

One quarter of sexual offenses in the criminal justice system are committed by children and adolescents. Mental health conditions, trauma history, and relationship to the victim may play a role in sexually acting out behaviors (SAB). A retroactive chart review was performed to discover commonalities among 109 adolescent males with SAB admitted to a residential treatment facility. Variables of interest included prior mental health diagnoses, trauma history, full scale intelligence quotient (IQ), relationship to the victim, sex of victim, history of multiple offenses, and history of sexually act

Abdominal compartment syndrome in the pediatric patient with blunt trauma.

Abdominal compartment syndrome in the trauma patient is usually associated as a post surgical complication or as a consequence of full thickness burns to the trunk and abdomen. It is not widely recognized in patients who have received massive fluid resuscitation in the absence of abdominal pathology. This paper will present a case study of a pediatric patient who was the victim of a motor vehicle collision. This 4-year-old boy was hemodynamically unstable initially and received massive fluid resuscitation in the emergency department and pediatric intensive care unit. An emergency laparotomy re

Post-operative sleep disturbance: causes, factors and effects on outcome.

Post-operative sleep disturbance, with suppression of rapid eye movement sleep and slow wave sleep followed by a subsequent rebound, seems to be related to the magnitude of trauma and thereby to the surgical stress response. In this context, cortisol, autonomic stimulation, and certain cytokines may lead to abnormal sleep. Furthermore, the environment, pain and the administration of analgesics seem to be important factors in the precipitation of sleep abnormalities. Post-operative sleep disturbance may contribute to the development of episodic hypoxaemia, haemodynamic instability and altered m

Delay in diagnosis of urethral perforation due to catheterisation in a person with cervical spinal cord injury. Importance of (1) imaging studies done promptly for detection and documentation of urethral trauma, (2) Urethrotech catheterisation device to minimise urethral trauma and achieve successful catheterisation: A case report.

INTRODUCTION AND IMPORTANCE: Spinal cord injury patients are at risk for urethral trauma during catheterisation. We report a patient in whom urethral perforation due to catheterisation was not recognised for ∼ four days. CASE PRESENTATION: Following a routine catheter change by community nurses in a male person with tetraplegia, the catheter drained only a small amount of urine. Therefore, the patient attended the hospital where the staff removed the catheter but could not place a new catheter. The doctor tried different catheters and on fifth attempt, inserted a catheter. The patient continu

Causes of death following discharge after trauma in North Carolina.

BACKGROUND: While a "fourth peak" of delayed trauma mortality has been described, limited data describe the causes of death (CODs) for patients in the years following an injury. This study investigates the difference in COD statewide for patients with and without a recent trauma admission. METHODS: This retrospective cohort study compared COD for trauma and nontrauma patients in North Carolina. Death certificates in NC's death registry were matched with the NC trauma registry between January 2013 and December 2018 using matching on name and date of birth. Patients who died during the index tr

Mechanisms of injury in wartime.

A significant number of wounded servicemembers are returning from the conflicts in Iraq and Afghanistan. As the U.S. government finds itself with more wounded servicemembers than its systems can handle, the wounded are beginning to use private rehabilitation facilities. Mechanisms of injury in war are unlike those of most injuries encountered in civilian life. Rehabilitation nurses in both military and private rehabilitation facilities can benefit from learning about the mechanisms of injury in war to better help their patients and anticipate potential and hidden complications. This article re

A mixed psychiatric and somatic care unit for trauma patients: 10 years of experience in an urban level I trauma center in the Netherlands.

BACKGROUND: A medical-psychiatric unit (MPU) is a special ward where staff is trained in caring for patients with psychiatric or behavioural problems that need hospitalisation for physical health problems. It is well known that these patients are at higher risk of complications and have a longer length of stay resulting in higher costs than patients without psychiatric comorbidity. The objective of this study was to analyse the trauma patient population of the first 10 years of existence of the MPU in a level I trauma center. PATIENTS AND METHODS: A retrospective analysis was performed in 2-y

Female Genital Mutilation and Cutting: Essentials for Pediatric Nurse Practitioners.

Female genital mutilation and cutting (FGM/C) is a serious global public health problem, violating the basic human rights of girls and women. FGM/C is a form of child maltreatment. According to the World Health Organization 200 million girls and women have undergone FGM/C in thirty countries in Africa, the Middle East, and Asia. FGM/C is also a concern in the United States (U.S.). Approximately 513,000 girls and women living in the U.S. are at risk for FGM/C or its consequences, representing a 4-fold increase within a decade. FGM/C places victims at risk for significant physical and psychologi

A scoping review of effective health practices for the treatment of birth trauma.

BACKGROUND: There is currently no consensus on the most effective health practices to manage or reduce the effects of birth trauma (BT) and childbirth-related posttraumatic stress disorder (CB-PTSD). AIM: The aim was to map the current literature on effective health practices for BT/CB-PTSD, identify key elements (the what, when and how) important for effective health practices, and highlight gaps in maternity care. METHODS: A systematic search was conducted across key nursing, allied, and medical databases (MEDLINE, Scopus, PubMed) for key terms related to (1) birth trauma and (2) intervent

Prediction of Alcohol Withdrawal Severity Scale (PAWSS) for patients cared for in an emergency department/Level I Trauma Center: An evidence-based practice change.

BACKGROUND: Patients with alcohol use disorder (AUD) often come to the Emergency Department (ED) for unrelated complaints but can still experience withdrawal. The purpose of this Evidence-Based Practice (EBP) project was to detect the potential for alcohol withdrawal in the ED using the Prediction of Alcohol Withdrawal Severity Scale (PAWSS) and then provide necessary treatment to prevent negative patient outcomes (i.e., alcohol withdrawal-related seizures, admissions to the intensive care unit (ICU), and prolonged stays in the ED). METHODS: This EBP project was implemented at a Level I Traum

The challenges of managing dysphagia in brain-injured patients.

Dysphagia, or the inability to swallow normally, is a feature of a number of neurological conditions. It is found in both paediatric and adult populations, but the scope of this article is limited to the adult neurogenic population. The normal swallow is a complex and highly coordinated activity, any part of which may be disturbed by neurological illness or injury. Assessment of dysphagia is normally undertaken by speech and language therapists in conjunction with the multidisciplinary team. A bedside screening assessment may be augmented by instrumental assessment, such as videofluoroscopy, i

Using non-verbal communication to support de-escalation in patients experiencing acute psychosis in the emergency department.

Emergency nurses are frequently the first point of contact for patients presenting to the emergency department (ED) with the symptoms of acute psychosis. Altered perception, fear and disorganised thinking can impair communication and precipitate distress, which may lead to agitation or aggressive behaviour, placing patients and staff at increased risk of harm. In these circumstances, verbal reasoning alone may be ineffective. Evidence indicates that non-verbal communication, such as body posture, eye-level alignment, proximity and tone of voice, can have a pivotal role in these situations by p

Human trafficking: what psychiatric nurses should know to help children and adolescents.

Psychiatric nurses are in key positions to identify and stop human trafficking, as well as aid its survivors. The combination of emotional trauma, sexual violence, and physical injuries experienced by these victims leads to high rates of posttraumatic stress disorder, depression, and anxiety. To detect human trafficking, it is important to identify the salient risk factors of homelessness and runaway history. This article offers key questions to help identify victims, as well as web-based resources.

Prevention of hypothermia in trauma victims - the HYPOTRAUM 2 study.

INTRODUCTION: Hypothermia is common in trauma patients. It contributes to increasing mortality rate. Hypothermia is multifactorial, favoured by exposure to cold, severity of the patient's state and interventions such as infusion of fluids at room temperature. AIM: To demonstrate that specific management of hypothermia (or of the risk of hypothermia) increases the number of trauma patients arriving at the hospital with a temperature >35°C. DESIGN: This is a prospective, multicentre, open-label, pragmatic, cluster randomized clinical trial of an expected 1,200 trauma patients included by 12 ou

Characteristics and demography of low energy fall injuries in patients > 60 years of age: a population-based analysis over a decade with focus on undertriage.

BACKGROUND: An increasing group of elderly patients is admitted after low energy falls. Several studies have shown that this patient group tends to be severely injured and is often undertriaged. METHODS: Patients > 60 years with low energy fall (< 1 m) as mechanism of injury were identified from the Stavanger University Hospital trauma registry. The study period was between 01.01.11 and 31.12.20. Patient and injury variables as well as clinical outcome were described. Undertriage was defined as patients with a major trauma, i.e., Injury Severity Score (ISS) > 15, without trauma team activatio
