Challenges in Diagnosis and Management of Infantile Deep Neck Abscess: A Case Report and Literature Review.

A deep neck abscess is a relatively rare bacterial infection in infants that can rapidly progress to life-threatening complications. Mostly, the patients present with fever and neck pain. Some may present with dysphonia, sore throat, torticollis, trismus, or neck swelling. Early diagnosis and management can prevent life-threatening complications, such as airway obstruction, mediastinitis, and tracheitis. In this report, we present a case of a six-month-old infant presenting with retropharyngeal and parapharyngeal abscesses with prompt diagnosis leading to complete recovery of the patient.

Oral Complications from Oropharyngeal Cancer Therapy.

Post-oropharyngeal cancer treatment complications include a multitude of oral side effects that impact overall survival and quality of life. These include acute and chronic conditions affecting the oral cavity and head and neck, such as mucositis, infection, xerostomia, dysgeusia, radiation caries, osteonecrosis, and trismus. This review will summarize the most common oral complications from oropharyngeal cancer therapy. The authors would like to point out that the literature cited frequently combines oropharyngeal and head and neck cancer results. If recommendations are made strictly related …

Peritonsillar abscess due to temporomandibular joint septic arthritis: an uncommon cause of a common disease.

Peritonsillar abscess is a common presentation of complicated acute tonsillitis. The temporomandibular joint septic arthritis is an uncommon clinical entity while its complications are further rare. We present a case of a man in his late 30s who presented with a painful swelling in the left preauricular region along with increasing trismus. A diagnosis of peritonsillar abscess due to septic arthritis was made based on clinical history and examination. Patient recovered fully after antibiotic and abscess drainage. This is the first case report of temporomandibular septic arthritis leading to pe…

Use of the TotalTrack VLM for emergent endotracheal intubation in predicted difficult airway with obstruction by expanding space-occupying lesions and reduced interincisor opening.

Acute cervical pathology may lead to serious airway compromise resulting from anatomical distortion secondary to obstruction or deviation of the upper airway, scenarios where any airway device can be fallible. Passage of a fiberoptic bronchoscopy to expose the glottis may be very difficult and tracheostomy may be impractical or risky in advanced cases. We present the use of the TotalTrack VLM to secure the airway for emergent surgery in 2 uncooperative patients with difficult airway due to Ludwig's angina and neck hematoma, respectively, both accompanied by trismus.

PERIOPERATIVE CARE OF A CHILD WITH CRISPONI SYNDROME.

Crisponi syndrome is an autosomal recessive disorder characterized by intermittent episodes of muscular contraction of the facial muscles with trismus and excessive salivation simulating a tetanic spasm. These episodes occur in response to tactile stimulation or during crying. Associated physical and constitutional findings include characteristic facial anomalies, camptodactyly, intermittent hyperthermia, and feeding difficulties. We present a 15-month-old girl who required anesthetic care during laparoscopic fundoplication and gastric tube insertion. The perioperative implications of the diso…

[Metoclopramide intolerance and oral manifestations].

Metoclopramide is a widely used product, which can accidentally cause acute dyskinesia, of which several forms may be observed, including dysarthria, tetany, spasmophilia, false cerebral vascular insult, trismus, hysteriform spell.... In all those cases, one should think of giving an antiemetic and of properly orienting history-taking, thus avoiding a longer stay in hospital that would not be justified. This is demonstrated by two patients recently admitted into the department of maxillofacial surgery and stomatology with maxillofacial signs related with the adverse effects of this kind of dru…

Cephalic tetanus progressing to generalized tetanus after facial trauma in an unvaccinated elderly patient: A case report.

A 67-year-old unvaccinated male patient was analyzed. Clinical manifestations, laboratory and imaging findings, treatment, and disease progression were evaluated using hospital records. Eight days after facial trauma from a bicycle accident, the patient developed progressive trismus, dysphagia, neck stiffness, and respiratory discomfort. He was hospitalized on day 10 post-injury. Initial treatment included 500 IU of tetanus immunoglobulin. Two days later, his condition deteriorated with progression to generalized tetanus, requiring ICU admission. Additional 2000 IU of tetanus immunoglobulin, s…

Tongue piercing. The new "rusty nail"?

BACKGROUND: Cephalic tetanus is a rare form of the tetanus caused primarily by wounds or other infectious processes involving the head and neck. This condition frequently progresses to the generalized form of tetanus with the attendant risks and complications. METHODS: A case report of a young female who developed an unusual form of tetanus after a tongue piercing is presented here. We discuss this disorder as it applies to the contemporary caregiver with a focus on its presentation and recognition. RESULTS: A delay in diagnosis of 13 days from presentation occurred. The patient had a slow,

Lockjaw secondary to skull base osteochondroma: CT findings.

Osteochondromas arising from the long bones, pelvis, or scapulae are common. However, osteochondromas originating from the base of the skull are extremely unusual. Although these tumors are histologically benign, intracranial extension and their close proximity to the cranial nerves may require complex surgery. We present a case in which CT clearly delineated an osteochondroma arising from the base of the skull. The tumor had created a fusion with the ipsilateral coronoid process of the mandible, thus causing lockjaw.

[Analysis on deformity of disk associated with internal derangement of TMJ].

Internal derangement of temporomandibular joint has been studied mainly arthrographically about disk position, but it was reported that morphologic alterations and histo-chemical changes of disk were frequently observed in joints with internal derangement. The aim of this investigation was to study the relationship between deformity of disk and the kind of internal derangement of TMJ (anterior disk displacement with or without reduction). We analysed arthrographically the configuration of anterior displaced disk according to the criteria of Westesson. Deformity of disk was seen in 35 joints in

Clinical, tomographic, and histological analysis of post-extraction dental sockets filled with particulate dentin or blood clot: pilot study of a randomized clinical trial.

OBJECTIVE: This preliminary study aims to compare the clinical, radiological, and histological outcomes of third molar socket repairs after dental extraction with and without the use of particulate dentin graft. MATERIALS AND METHODS: Ten patients requiring bilateral surgical extraction of impacted lower third molars were selected. One side received a blood clot (control side) and the other an autologous particulate dentin graft (experimental side). Postoperative evaluations at 7 and 21 days assessed pain, edema, trismus, suture dehiscence, and wound edge coaptation. At 120 days, CBCT was use…

Tetanus presenting primarily as periocular discomfort in a patient who underwent surgery for sleep apnea: A case report and brief literature review.

RATIONALE: Tetanus is caused by a neurotoxin (tetanospasmin) secreted by a spore forming gram-positive, anaerobic rod-shaped motile bacillus, Clostridium tetani. The most common symptoms of tetanus are trismus (100%), dysphagia (70.5%), dysarthria (35.2%), and neck stiffness (29.4%). Respiratory failure, laryngeal spasm, seizure, chest pain, nausea/vomiting, opisthotonus, back pain, and rigid abdominal wall can also be observed during progression of the disease. However, there has been no report of periocular discomfort as an initial manifestation after endoscopic sleep surgery in a patient wi…

Abdominal pain with rigidity secondary to the anti-emetic drug metoclopramide.

We report a case of abdominal pain with rigidity, mimicking an acute abdomen, caused by metoclopramide, a common anti-emetic drug. Extrapyramidal symptoms are commonly reported side-effects of this medication. They generally include involuntary movements of limbs, torticollis, oculogyric crisis, rhythmic protrusion of tongue, trismus, or dystonic reactions resembling tetanus, etc. Abdominal rigidity due to this medication, resembling an acute abdomen, has not been reported previously. This case report illustrates the importance of considering medication side-effects when evaluating a patient w…
