The causes of granulomatous hepatitis are often difficult to determine and standard diagnosis methods for tuberculous hepatitis lack sensitivity, which led us to evaluate the contribution of mycobacteria serology. The case is reported of a woman in whom a diagnosis of extrapulmonary sarcoidosis had been made on two occasions (1985, 1987) in the presence of granulomatous hepatitis. Liver histology remained identical in 1991 but the existence of positive mycobacteria serology led to the start of antituberculous treatment. The satisfactory response in terms of clinical, laboratory and histologica

Tuberculosis, caused by Mycobacterium tuberculosis is a common infection both in immunocompromised and normal hosts. Its clinical manifestation can by divided as pulmonary and extrapulmonary form. Pyomyositis caused by M. tuberculosis is extremely rare. The authors report 2 patients, one with underlying dermatomyositis, and the other with polymyositis. The diagnosis was delayed according to nonspecific symptoms and masking effect of steroid therapy, which led to complications. Microscopy and culture of the pus confirmed the diagnosis. Surgical drainage was done and antituberculous therapy was

Tuberculosis of the flexor tendon sheaths in western industrial countries is rare. The author presents a case with an exceptional course and an unexpected outcome. A sixty-five year old woman suffered from tuberculosis of the flexor tendon sheath of her little finger. Appropriate tuberculostatic treatment was impossible due to damage of liver cells by chronic hepatitis. An open wound with positive cultures of mycobacterium tuberculosis persisted over eight months despite local and systemic treatment. It was only after an incidental streptococcal infection of the wound--resembling an erysipelas

Isoniazid (INH) is recommended for tuberculosis prophylaxis in non-liver transplant recipients. However, there is a great reluctance to prescribe this agent for liver transplant candidates and recipients due to the risk of precipitating further hepatic decompensation. We analyzed the records of liver transplantation candidates undergoing a purified protein derivative (PPD) test (tuberculosis skin test) between 2008 and 2010. Patients with no respiratory symptoms, PPD test > 10 mm, and normal chest radiography were diagnosed as latent tuberculosis and prescribed INH (300 mg) per day for 6 month

The treatment of 77 pulmonary tuberculosis patients with zixoryn in the course of chemotherapy resulted in a lower incidence of toxic and allergic reactions and acute hepatitis, and a faster normalization of altered biochemical indices as compared to controls. The rate of cavity closure and sputum conversion was higher than in the controls in addition to a shorter period of in-patient treatment. It is concluded that the liver protector is highly effective in patients with tuberculosis. Besides, it is advisable to use it in the patients with hepatobiliary abnormalities for preventive purposes.

Intramedullary tuberculoma (IMT) is rare and usually indistinguishable from spinal cord tumors. Thus, the diagnosis of an IMT is challenging. Our case deals with an unusual presentation of a 55-year-old Asian man who had presented with lower limb weakness which was found to be caused by the dissemination of tuberculosis (TB) resulting in an IMT, a rare complication of tuberculosis. The patient also had a concurrent incidental hepatitis B infection. The treatment of IMT is anti-tuberculous medication. This case highlights the significance of the prompt diagnosis of an IMT, urgent intervention p

BACKGROUND: Hepatobiliary tuberculosis includes miliary, tuberculous hepatitis or localized forms. The localised form is extremely uncommon and can mimic malignancy. Still rarer is its presentation as sclerosing cholangitis.
CASE PRESENTATION: A 50 year male presented with acute onset jaundice, significant weight loss and elevated liver enzymes with clinico-radiological suspicion of cholangiocarcinoma. A left hepatectomy was done and dilated bile ducts filled with caseous necrotic material were seen intra-operatively. Histopathology suggested localized hepatobiliary tuberculosis with features

Decreasing of number of cases as well as incidence rate of hepatitis type B and type A, and increasing of pertussis, leptospirosis, encephalitis and some other diseases was noted in Poland in 1997. The biggest percentage of deaths was caused by tuberculosis--43.1%, sepsis--over 21.9% and hepatitis--10.6%. Introduction of ICD-10 as well as strikes of health workers in Poland in 1997 caused undernotification especially of deaths.

BACKGROUND: Anti-tuberculosis drugs (ATD) induced DRESS syndrome is rarely reported, and its diagnosis and management are very challenging.
CASE PRESENTATION: A 33-year-old woman presented with fever, maculopapular rashes, hypereosinophilia, and hepatic involvement, which occurred 4 weeks after a fixed-dose combination of first-line ATD containing rifampicin, isoniazid, pyrazinamide, and ethambutol. The patient's condition improved after the withdrawal of the drugs and administration of systemic steroids. Furthermore, active pulmonary tuberculosis was treated with second-line ATD containing s

Hepatic granulomas of infectious origin are due to several causes. Bacterial and rickettsial infections are the most frequent (mainly tuberculosis, infections due to atypical mycobacteria, Q fever). However parasitic and fungal infections may also be involved. Histological characteristics of the granuloma are sometimes suggestive of the cause, but the aetiological diagnosis often results from a careful confrontation of clinical, biological, and histological data. One should always have in mind the possibility of tuberculosis, where an effective treatment must be given as early as possible. The

Granulomatous hepatitis is not a uniform disease entity but a generic histopathologic diagnosis. The present review of aetiology and epidemiology of granulomas in the liver, shows that in the past tuberculosis and sarcoidosis were the most frequent but there remained a large percentage of "undiagnosis" cases; at the present drug-induced granulomatous hepatitis are frequently reported and "undiagnosed cases" are rare. Analysis of the hystological features of the most common granulomas is presented.

Although all European countries have vaccination policies for children, there are no comprehensive studies of pediatric vaccination programs in Europe. We studied vaccination programs for children in Europe. Vaccinations against diphtheria, tetanus, pertussis, poliomyelitis, Haemophilus influenzae type b, hepatitis B, measles, mumps, rubella, and influenza existed in 42 countries, against human papilloma virus in 41 countries, and against pneumococcus in 40 countries. In addition, the following vaccinations existed: against tuberculosis (35 countries), hepatitis A (33), meningococcus A, C, W,

BACKGROUND: Patients with suspected tuberculosis are often overtreated with antituberculosis drugs. We evaluated the diagnostic value of the focused assessment with sonography for HIV-associated tuberculosis (FASH) in rural Tanzania.
METHODS: In a prospective cohort study, the frequency of FASH signs was compared between patients with confirmed tuberculosis and those without tuberculosis. Clinical and laboratory examination, chest x-ray, Xpert MTB/RIF assay, and culture from sputum, sterile body fluids, lymph node aspirates, and Xpert MTB/RIF urine assay was done.
RESULTS: Of 191 analyzed pa

PURPOSE: To clarify the incidence and clinical significance of anti-tuberculosis drug-induced liver injury.
SUBJECTS AND METHODS: Questionnaire was sent out by mail to 114 hospitals, to ask whether there were patient(s) from 1994 to 2003 with liver injury induced by anti-tuberculosis drugs with alanine aminotransferase (ALT) or aspartate aminotransferase (AST) level of more than 1000 IU/l and/or total bilirubin level of more than 2 mg/dl. As for the cases of severe hepatic injury, their backgrounds and clinical courses were investigated.
RESULTS: Seventy cases were reported from 24 out of 68

BACKGROUND: In this study, renal transplant recipients with tuberculosis of different organs, were retrospectively analysed with respect to prevalence, outcome and drug toxicity.
PATIENTS AND METHODS: In 520 patients, 22 (4.2%) tuberculosis of various organs was diagnosed. The time interval between transplantation and diagnosis of tuberculosis was 44.4 +/- 33.5 (range 3-111) months. In 18 (82%) of the patients, tuberculosis was detected after the first year of transplantation. The most common form was pleuro/pulmonary tuberculosis (54%), and other localizations included jejunum, liver, bone,

BACKGROUND: Perinatal tuberculosis is acquired during birth or during the early neonatal period. Although its incidence is unknown, a search was conducted in Medline and 28 cases were found of perinatal tuberculosis reported from 1983 to 2011. Diagnosis of this disease is important due to having nonspecific symptoms that are mistaken for other infectious diseases. The disease has a high mortality rate (60%); therefore, it requires prompt diagnostic suspicion by the medical staff to prevent a fatal outcome.
CASE REPORT: We describe the case of a 3-month-old male whose 29-year-old mother died o

Between 1983 and 1989, we cared for 56 patients with tuberculosis and human immunodeficiency virus (HIV) infection. In 37 patients (66%), tuberculosis occurred before any other AIDS-defining disease (group 1); in 10 (18%) it occurred during the same month as another AIDS-defining disease (group 2); and in 9 (16%), after the diagnosis of AIDS (group 3). Tuberculosis was entirely pulmonary in 14 patients (25%), entirely extrapulmonary in 9 (16%), and both pulmonary and extrapulmonary in 33 (59%). The frequency of extrapulmonary involvement was similar in patients from group 1 and from groups 2 a

In Denmark, about one third of notifiable diseases are imported, but these have had only a limited spread within the Danish community. For example, imported strains of Mycobacterium tuberculosis remain confined to the immigrant population, and the high prevalence of chronic hepatitis B among certain groups of immigrants has not resulted in a general increase in the incidence of hepatitis B. Exceptions are imported cases of hepatitis A in children, which have resulted in outbreaks in the indigenous Danish population, and the import of drug-resistant bacteria, which often pose a serious challeng
