A Rare Case of Multidrug-Resistant Tuberculosis Affecting the Pleura.

The majority of cases with tuberculous pleuritis have negative acid-fast bacilli (AFB) on smear microscopy, making the diagnosis difficult. This case report is based on the successful diagnosis and management of an extra-pulmonary (EP) multidrug-resistant tuberculosis (MDR-TB) patient with a history of lymphoma. Initial tests revealed a right-sided pleural effusion and thickening of the pleura. The closed pleural biopsy, pleural fluid histopathology, culture, and drug sensitivity testing (DST) report revealed Mycobacterium tuberculosis with isoniazid and rifampicin resistance. Based on the DST

Empyema Necessitans Due to Tuberculosis in a Patient With Poorly Controlled Diabetes Mellitus Type 2: A Case Report.

Empyema necessitans (EN) is a rare condition characterized by the accumulation of pus in the extra-thoracic soft tissue due to decompression of intrathoracic empyema by extending through the parietal pleura and chest wall usually by Mycobacterium tuberculosis. This is a report of a patient with poorly controlled diabetes mellitus type 2 presenting with high-grade fever, productive cough, dyspnoea and a left-sided pleuritic chest pain complicated by a left pleural effusion. It further extended to extra-thoracic soft tissue space without fistulation to the external environment. Non-specific sign

Cat scratch disease and lymph node tuberculosis in a colon patient with cancer.

A 71-year-old man operated for a sigmoid tumour remained in the surveillance after adjuvant chemotherapy. After 3 years, a left axillary lymph node was visible on CT scan. The biopsy revealed a necrotising and abscessed granulomatous lymphadenitis, suggestive of cat scratch disease. The patient confirmed having been scratched by a cat and the serology for Bartonella henselae was IgM+/IgG-. Direct and culture examinations for tuberculosis were negative. The patient was treated for cat scratch disease. One year later, the CT scan showed increased left axillary lymph nodes and a left pleural effu

Significance of Pleural Fluid Neutrophil-Lymphocyte Ratio and Pleural Fluid Protein as Additional Biomarkers in Differentiating Tubercular Pleural Effusions From Non-tubercular Effusions.

BACKGROUND: Tubercular pleural effusion (TPE) remains a significant diagnostic challenge in tuberculosis-endemic regions. Current diagnostic methods have substantial limitations, necessitating the investigation of complementary biomarkers. OBJECTIVES: The primary objective of this study was to evaluate the diagnostic utility of pleural fluid neutrophil-lymphocyte ratio (NLR) and pleural fluid protein as adjunctive biomarkers, in combination with adenosine deaminase (ADA), for differentiating TPEs from non-tubercular effusions (NTEs). The secondary objective was to assess their potential clini

Exudative pleural effusion: effectiveness of pleural fluid analysis and pleural biopsy.

The study compared pleural fluid analysis and pleural biopsy in the diagnosis of 100 patients with exudative pleural effusion (PE) in Babol, Islamic Republic of Iran. Tuberculous pleurisy and malignant pleural effusion were confirmed by the identification of acid-fast bacilli from body fluids or tumour cells from tissue specimens. Malignant diseases and tuberculosis were the causes of exudative PE in 43% and 33% of patients respectively. The diagnostic sensitivity of pleural biopsy in patients with tuberculous PE and malignant PE was 70% and 54%, and the diagnostic sensitivity of pleural fluid

[Two cases of tuberculosis after treatment of rheumatoid arthritis with infliximab].

We report two cases of tuberculosis (TB) after treatment with infliximab (IFX) for rheumatoid arthritis (RA). The first case, a 69-year-old woman with RA, developed miliary TB with acute respiratory distress syndrome 21 months after initiation of IFX therapy. Sputum samples revealed smears and cultures positive for Mycobacterium tuberculosis and also positive polymerase chain reaction for TB (PCR-TB); in addition urine samples were smear-negative and culture-positive for TB. She was treated with corticosteroid pulse therapy and anti-tuberculosis drugs, and recovered. The second case, a 51-year

A Complex Case: Tubercular Pleural Effusion, Infarct, and Multisystem Involvement.

Tuberculous pleural effusion, the second most common type of tuberculosis, poses diagnostic and management challenges, especially in patients with multiple comorbidities. A 58-year-old female with a history of poorly controlled diabetes mellitus, hypertension, hypothyroidism, coronary artery disease post-angioplasty, and stable chronic kidney disease presented with fever with chills, reduced appetite, dyspnea, and dysuria. A chest X-ray showed a blunted right costophrenic angle and ultrasonography revealed a moderate pleural effusion on the right side. Pleural fluid analysis confirmed tubercul

[Case of pubic tuberculous osteomyelitis and pericarditis during anti-tuberculosis chemotherapy].

56 year-old man was referred to the department of orthopedics in our hospital for further investigation on right inguinal pain. The patient was initially diagnosed as bacterial myelitis in right pubic bone and was treated with antibiotics. Since his symptom did not improve, the curettage was performed. Histological examination of the pubic bone obtained during the operation showed epithelioid cell granulomas with caseous necrosis, supporting the diagnosis of tuberculous osteomyelitis. Chest X-ray film revealed small nodular lesions in both upper lung fields. Sputum was positive for acid-fast b

[Role of biomarkers in making the diagnosis of tuberculous pleurisy].

Although tuberculosis is one of the most common causes of pleural effusion, diagnosis of tuberculous pleuritis still remains a challenge. This is due to paucity of M. tuberculosis organisms in pleural effusion which results in a relatively low sensitivity of the routinely used diagnostic methods. Thus, different biomarkers in pleural effusion have been extensively studied in order to improve the diagnostic accuracy. Pleural fluid deaminase adenosine activity (ADA) and interferon gamma (IFN-γ) concentration have been shown to be the most reliable and cost-effective markers of tuberculous pleuri

Multiple tuberculous nodules with metachronous changes: a case report.

BACKGROUND: Spontaneous regression of lesions occurs in non-infectious benign diseases, such as sarcoidosis, as well as in infectious diseases, such as tuberculosis. Lung cancer and malignant lymphoma, on the other hand, rarely follow a similar course. We report a rare case of lung tuberculosis that presented with multiple nodules with metachronous changes in size. CASE PRESENTATION: We describe the case of a 50-year-old immunocompetent Japanese man with pulmonary tuberculosis in the form of multifocal nodules. He came to our hospital because of a chest X-ray abnormality. During the course of

Case Report: Macrophage activation syndrome due to multifocal tuberculosis in an immunocompromised patient.

Macrophage Activation Syndrome (MAS) is a serious and life-threatening complication defined by excessive immune activation. While it's commonly associated with rheumatic diseases, infections can also trigger MAS, with tuberculosis being a rare but significant cause. This case report discusses a rare occurrence of Macrophage Activation Syndrome (MAS) caused by multifocal tuberculosis in an immunocompromised patient with Crohn's disease receiving immunosuppressive treatment. The patient is a 26-year-old woman with Crohn's disease who is being treated with azathioprine. She arrived at the hospita

Recurrent non-traumatic idiopathic chylothorax: a diagnostic dilemma with therapeutic challenge.

Chylothorax is an uncommon pleural effusion characterized by the presence of chyle in the pleural space. Malignancy, trauma, and infections such as tuberculosis and filariasis can result in chylothorax. The chyle is an odourless and white liquid; however, around 50% of chylothorax is not milky (serosanguineous) and 10% of chylothorax is idiopathic which can pose a diagnostic dilemma. We propose a systematic approach which includes pleural fluid triglycerides and cholesterol for all undiagnosed pleural effusion. The case highlights the diagnostic dilemma as well as therapeutic challenges.

[Thoracoscopy in chronic pleural effusion (author's transl)].

123 patients with chronic pleurisy were investigated by thoracoscopy. The endoscopic biopsy is positive in 16 out of 17 cases of mesothelioma (96%), in 57 out of 72 cases of pleural metastasis (79%), and in 6 cases out of 7 pleural tuberculosis. Begnin pleural asbestosis was confirmed in 5. Endoscopic biopsy is thus more efficient than results obtained with pleural needle biopsy and exudative cytology. Thoracoscopy can be performed without any serious complications, this is not always true with open thoracotomy.

[Adenocarcinoma of the lung associated with acute promyelocytic leukemia and miliary tuberculosis].

A 44-year-old man was admitted to the hospital because of acute promyelocytic leukemia. A nodular shadow (2.5 cm in diameter) was seen in the right upper lung field on a chest roentgenogram. During the administration of combination chemotherapy for the leukemia, diffuse granular shadow appeared in all lung fields. Transbronchial lung biopsy findings indicated tuberculosis. The patient was given streptomycin, isoniazid, rifampicin and ethambutol, but the nodular show enlarged and a pleural effusion appeared on the right side. Cytologic examination of the pleural effusion revealed adenocarcinoma

Impact of HIV infection on tuberculous pleural effusion.

The nature of tuberculosis (TB), being one of the most common opportunistic infections, is different among HIV-infected patients than HIV-negative patients. A retrospective study was conducted on HIV-positive and HIV-negative patients with new TB pleural effusion who were admitted to the National Research Institute of Tuberculosis and Lung Diseases in Tehran, Iran from 2005 to 2012. The two groups were compared with respect to clinical, imaging, mycobacteriologic and histopathologic characteristics of TB pleural effusion. In all, 42 HIV-positive and 132 HIV-negative cases of TB pleural effusio

Multiple pleural nodules without effusion--a rare presentation of tuberculous pleurisy.

We report a rare case of tuberculous pleurisy presenting with multiple pleural nodules without associated effusion or parenchymal lung lesions. A 62-year-old man had multiple discrete pleural nodules in the right hemithorax on chest radiography without any clinical symptoms. Thoracoscopic biopsy of the pleural nodules revealed a caseous granuloma with acid-fast bacilli. The patient received antituberculous therapy, with resolution of tuberculomas on chest film within 2 months. To our knowledge, only two similar cases have been previously reported in the English literature, and our observation
