We present a case of 23-year-old student misdiagnosed for two months. Radiological finding showed a pneumonial infiltrate of left lung lower lobe. Antibiotical therapy was not resulting in a radiological regression. Biopsy of the lung infiltrate by transthoracic computed tomography guided histology needle, showed granulomatous inflammation with necrosis. Bronchial aspirate received by bronchoscopy was positive in culture on Mycobacterium tuberculosis. After 6 months of antituberculotic therapy advance the complete regression of lung infiltrate. Tuberculosis of lower lung lobe is difficult to d

Over the past decade, tuberculosis has been recognized worldwide as a public health problem of increasing proportions. We report a patient who presented with epistaxis and generalized petechiae. The diagnosis was immune thrombocytopenia and the patient was treated with intravenous immunoglobulin and pulse steroid. The bleeding continued and thrombocyte level was low despite therapy. Chest X-ray and a computed tomography scan of the thorax showed right upper lobe opacities and bilateral interstitial infiltrates. The patient also had a history of close contact with an individual with active tube

We present an unusual pediatric patient with severe allergic bronchopulmonary aspergillosis that masqueraded as pulmonary tuberculosis. After antituberculous therapy, the patient's pulmonary disease flared, prompting an aggressive diagnostic evaluation. Radiographic findings included nodular pulmonary densities with superimposed infiltrates. Lung biopsy showed evidence of acute and chronic inflammation with necrotizing granulomas, giant cells, tissue eosinophilia and rare hyphal elements.

UNLABELLED: We investigated the usefulness of 99mTc-methoxyisobutylisonitrile scintigraphy in patients with known or suspected pulmonary tuberculosis (PTB) in comparison with radiological and bacteriological findings.
METHODS: Thirty-six patients aged 13-59 yr were scanned 15 and 60 min after intravenous injection of 370 MBq (10 mCi) 99mTc-methoxyisobutylisonitrile. Twenty-four patients had active PTB proven by chest radiograph and sputum examinations, two had miliary tuberculosis and ten were suspected of having relapsed PTB with negative sputum examinations and indeterminate chest radiograp

Neuromyelitis optica and acute necrotic myelopathy occur in association with pulmonary tuberculosis. We studied 8 patients with either neuromyelitis optica (6), acute myelopathy (1), or acute optic neuropathy (1) in close temporal association with pulmonary tuberculosis, but with no evidence for CNS tuberculosis. Neurologic symptoms preceded the use of antituberculosis medication in 5 patients. Different patients showed similar clinical features, suggesting a consistent disease pattern. Autopsy examination (1 patient) revealed extensive spinal cord and optic nerve demyelination. We identified

DLCO, DL/VA, Dm and Vc, lung volumes, capacities and values of a curve representing flux-forced expiration volume relationships were investigated in 159 patients with focal, infiltrative, disseminated and fibrocavernous tuberculosis. A decrease in DLCO was observed in half of the patients with disseminated and fibrocavernous tuberculosis and in less than one/fifth of the patients with focal and infiltrative tuberculosis. The leading factor of a decrease in DLCO in the patients with disseminated and fibrocavernous tuberculosis was the reduction of the lung respiratory surface resulting from a d

Multifocal tuberculosis is characterized by the presence of large multifocal tuberculosis areas in the same or different adjacent or distant organs. Primary lesions are usually in the lungs in the majority of patients. Difficulty in confirming multifocal tuberculosis and consideration of other diseases may lead to a delay in diagnosis and thus in initiating treatment. Bone and joint involvement in tuberculosis is uncommon. While osteoarticular tuberculosis most commonly occurs in the vertebral column, less frequently affected sites are hip, knee, and sacroiliac joints. The following is a fasci

Primary pulmonary leiomyosarcoma (PPL) is an exceedingly rare malignancy, accounting for fewer than 0.5% of all lung tumours. Its occurrence alongside disseminated tuberculosis (TB) complicated by constrictive pericarditis has not previously been described. We report a 66-year-old HIV-negative male from rural South Africa presenting with progressive dyspnoea, found to have a right upper lobe nodule that enlarged despite adequate anti-TB therapy. Computed tomography (CT)-guided biopsy demonstrated spindle cell morphology. Cardiac MRI and right and left heart catheterisation confirmed constricti
