Single posterior approach for circumferential debridement and anterior reconstruction using fibular allograft in patients with skipped multifocal spinal tuberculosis.

BACKGROUND: Skipped multifocal spinal tuberculosis (TB) is an atypical presentation of spinal TB. Surgical treatment for these unusual cases remains a challenge for spine surgeons. In our institute, we used single-stage circumferential debridement and anterior reconstruction with fibular allograft followed by posterior instrumentation through posterior-only approach for these patients. This study aimed to determine the efficacy and feasibility of this technique. METHODS: Twelve patients with skipped multifocal spinal TB who received our treatment method from January 2012 to June 2020 were enr

Late-Onset Paraplegia in Old Healed Spinal Tuberculosis Due to Traumatic Fracture of Fusion Mass - A Rare Case Report.

The natural healing of spinal tuberculosis occurs by spontaneous fusion of vertebral bodies with or without kyphotic deformity. Late-onset paraplegia secondary to the fracture of fusion mass in tuberculosis is one of the rare conditions which have not been extensively reported. A 56-year-old male patient sustained road traffic accident was diagnosed with a fracture of fusion mass in already healed tuberculosis. He was presented with weakness in both the lower limbs with ASIA-C grading of spinal cord injury. He was treated with posterior instrumented stabilization and decompression. The patient

Cervical spinal tuberculosis with tuberculous otitis media masquerading as otitis externa malignans in an elderly diabetic patient: case report.

Extrapulmonary manifestation of tuberculosis (Tb), a medieval disease, occurs in every part of the body with varying degree of frequency but commonly in the pleural and lymph nodes. When it occurs in bones thoracolumbar vertebrae is the usual site of involvement. Other bones are less involved hence seldomly reported. This is true for other organs and parts of the body including larynx, pharynx nose and the middle ear. More importantly, isolated extrapulmonary tuberculosis without pulmonary involvement is uncommon. The case of a 65 year old Nigerian trader who presented with headache, chronic e

Comparison of clinical efficacy and surgical safety among three bone graft modalities in spinal tuberculosis: a network meta-analysis.

BACKGROUND: Autogenous granular bone graft (AG), autogenous massive bone graft (AM), and titanium mesh bone graft (TM) are the three commonly utilized bone implant methods for spinal tuberculosis. However, the gold standard is still controversial. Therefore, this study aimed to compare the clinical efficacy and surgical safety of three primary bone graft modalities. METHODS: For systematic literature review, several databases, including PubMed, Embase, and Web of Science, were searched up to December 2022. Stata (version 14.0) was employed for data analysis. RESULTS: Our network meta-analysi

The impact of miR-221-3p modification by the m6A demethylase ALKBH5 on the inflammatory response in spinal tuberculosis and its diagnostic significance.

BACKGROUND: The interplay between N6-methyladenosine (m6A) modification and microRNAs (miRNAs) in the pathogenesis of spinal tuberculosis (STB) remains incompletely understood. This study aimed to investigate the regulatory mechanism involving the m6A demethylase ALKBH5 and specific miRNAs in STB. METHODS: miRNA sequencing was conducted to identify differentially expressed miRNAs in STB tissues. The expression levels of miR-221-3p and ALKBH5 were validated via qRT-PCR in tissue samples and peripheral blood from 50 STB patients and healthy controls. The diagnostic potential of these biomarkers

Unmasking the great imitators-noninfectious conditions masquerading as spinal tuberculosis in a developing country: A single-center case series analysis.

BACKGROUND: The diagnosis of spinal tuberculosis often relies on clinical, radiological, and laboratory findings, particularly in resource-constrained settings. However, numerous noninfectious conditions exhibit similar clinical and radiological features to spinal tuberculosis, leading to potential misdiagnosis in the absence of microbiological or histopathological confirmation. This study aims to present a case series from a developing country, highlighting noninfectious conditions that mimic spinal tuberculosis. METHODS: A retrospective analysis was conducted on hospital records and imaging

A predictive model for early clinical diagnosis of spinal tuberculosis based on conventional laboratory indices: A multicenter real-world study.

BACKGROUND: Early diagnosis of spinal tuberculosis (STB) remains challenging. The aim of this study was to develop a predictive model for the early diagnosis of STB based on conventional laboratory indicators. METHOD: The clinical data of patients with suspected STB in four hospitals were included, and variables were screened by Lasso regression. Eighty-five percent of the cases in the dataset were randomly selected as the training set, and the other 15% were selected as the validation set. The diagnostic prediction model was established by logistic regression in the training set, and the nom

Esophageal perforation following pedicle screw placement for the treatment of upper thoracic spinal tuberculosis: a case report and review of the literature.

BACKGROUND: The technique of posterior pedicle screw fixation has already been widely applied in the treatment of upper thoracic spinal tuberculosis. However, lesions of tuberculosis directly invade the vertebrae and surrounding soft tissues, which increases the risk of esophageal perforation induced by the posterior pedicle screw placement. Herein, we report the first case of esophageal perforation following pedicle screw placement in the upper thoracic spinal tuberculosis, and describe the underlying causes, as well as the treatment and prognosis. CASE PRESENTATION: A 48-year-old female pat

A novel clinical classification of spinal tuberculosis for decision-making guidelines from Chinese spine surgeons: a multicenter retrospective cohort study.

BACKGROUND: Spinal tuberculosis (TB) presents substantial therapeutic challenges as it can lead to severe structural and neurological complications. However, evidence-based therapeutic decision-making is hindered by the limitations of existing classification systems, which lack anatomical specificity, quantifiable parameters, and reliability. METHODS: The classification system proposed herein categorizes spinal TB into four types based on clinical manifestations and radiologic features: type I (mild): single-segment involvement, with <50% vertebral collapse and preserved stability; type II (m
