Clinical and Electrophysiologic Features of Ulnar Neuropathy at the Wrist: Analysis of 150 Consecutive Cases.

PURPOSE: To report clinical and electrodiagnostic (EDX) findings in ulnar neuropathy at the wrist (UNW). METHODS: This is a monocentric study. We collected demographic and clinical data, history, symptom type, neurologic examination findings, EDX results, as well as causes and risk factors of UNW. RESULTS: We enrolled 150 consecutive cases. Based on EDX findings, the most common UNW pattern involved the ulnar nerve at the entrance of Guyon canal, prior to its bifurcation into sensory and motor branches (44% of cases). We identified atypical topographic lesions involving the superficial senso

Orthodromic sensory action potentials from palmar stimulation in the diagnosis of carpal tunnel syndrome.

It is known that the major slowing of nerve conduction in the carpal tunnel syndrome occurs in the palm to wrist segment. A technique exploiting this fact and easily applicable in the routine laboratory would be useful in diagnosing early examples of the condition. To this end, orthodromic sensory potentials from stimulation of digits II and V, and of the palmar branches of the median and ulnar nerves were recorded at the wrist in 53 healthy hands, 72 hands with a tentative diagnosis of carpal tunnel syndrome and in 20 hands of patients with unrelated neurological conditions. Using conventiona

Entrapment Neuropathies of the Upper Extremity.

Upper extremity entrapment neuropathies are common and can cause pain, sensory loss, and muscle weakness that lead to functional disability. In this article, the authors review common entrapment neuropathies of the upper extremities, including median neuropathy at the wrist (carpal tunnel syndrome), ulnar neuropathy at the elbow, and radial neuropathy. The authors discuss the pathophysiology of nerve compression and typical etiologies, as well as strategies for differentiating between common mimics such as cervical radiculopathy and for selecting between various treatment modalities.

Peripheral nerve entrapment, occupation-related syndromes and sports injuries, and bursitis.

A growing interest in nonarticular rheumatic conditions has been shown recently by the large number of articles published on the subject. In this review, three nerve entrapment syndromes and problems of four tendons of the ankle region are covered. Suprascapular neuropathy is underrecognized and, in many cases, may be treated conservatively. Entrapment of the radial and ulnar nerves may be caused by a variety of conditions, some of which are unusual. Tenosynovitis and rupture of the tendons of the ankle region, including the posterior tibial, anterior tibial, peroneus longus, and Achilles tend

Nerve compression by pacinian corpuscles.

Compression of nerves of the hand by hyperplasia of pacinian corpuscles is a rare occurrence. Six cases with previous history of trauma in the hand in four of them have been reported in the literature. We present two cases of nerve compression of the two varieties reported, both with previous histories of trauma. In these two cases, surgical excision of the tumor determined the disappearance of clinical symptoms.

Presence of multiple tendinous insertions of palmaris longus: a unique variation of a retrogressive muscle.

BACKGROUND: Palmaris longus, being the most variable muscle of the upper extremity is generally considered important morphologically rather than functionally. The palmar aponeurosis represents the distal part of the tendon of Palmaris longus. CASE DETAILS: We report here a unique case of palmaris longus presenting variation distally as its tendon divides to gain multiple attachments with the fascia covering the abductor pollicis brevis, flexor retinaculum and the tendon of flexor carpi ulnaris. In addition, it also continues as palmar aponeurosis as its normal course. CONCLUSION: Presence of

Sports-related injuries of the wrist: an approach to MRI interpretation.

Sports-related injuries to the wrist range from minor sprains to severe soft tissue disruption that can pose a risk to the normal function of the upper extremity. It is important to identify the specific nature of such injuries so as to establish an accurate diagnosis and deliver appropriate treatment. MRI of the wrist has greatly benefited from the use of dedicated surface coils, which allow fine depiction of soft tissue and cartilaginous structures. A review of the normal anatomy, MR interpretation pitfalls, and most common abnormalities of the tendons, ligaments, triangular fibrocartilage c

Early detection of median nerve compression by Electroneurography can improve outcome in children with Mucopolysaccharidoses.

BACKGROUND: Carpal tunnel syndrome (CTS) is a common complication of the mucopolysaccharidoses. In severe or attenuated mucopolysaccharidoses patients, clinical symptoms of CTS usually appear at a late stage of median nerve compression. Relying on CTS symptoms is often too late and there is a risk of axonal damage and further irreversible sequelae. Electroneurography is a powerful technique to detect the initial preclinical signs of median nerve compression. In a retrospective series of 13 children with mucopolysaccharidoses (10 Hunter, one Hurler-Scheie and 2 Hurler children), we describe the

Kienböck's Disease: A Rare Case Report and Review of Management.

Kienböck's disease (KD) is a rare condition characterized by avascular necrosis of the lunate, ultimately causing progressive wrist dysfunction and pain. Here, we present a case of a 26-year-old woman with persistent left wrist pain refractory to conservative treatment. Physical examination revealed tenderness over the lunate with diminished sensation in the ulnar and median nerve distribution. However, nerve conduction studies showed normal nerve function, ruling out carpal tunnel syndrome. Various imaging modalities revealed lunate sclerosis, edema, and ulnar negative variance, strongly indi

[Nerve entrapment syndrome of the elbow and forearm].

In this study we will discuss entrapment of the median, ulnair, radial and lateral antebrachial nerves of the elbow and the forearm. Compression of the nerves may occur when they traverse a tunnel and an incompatibility exists between the diameter of the tunnel and its contents (e.g. nerves, tendons,...). However, at the elbow and the forearm the nerves are also exposed to particularly dynamic compressions. This is due to anatomical relationships changing between the nerve and its surrounding muscles, tendons and aponevroses during the motion of flexion-extension of the elbow and the prono-sup

[The terminal latency quotient in patients with compressive syndromes].

In our investigation 119 patients suffering from carpal tunnel syndrome, and 45 suffering from cubital tunnel syndrome have been analyzed. The above mentioned syndromes belong to compressive mononeuropathies and are the result of the narrowing of the anatomical channel, through which the nerve passes, causing direct or indirect pressure on the nerve and difficulties in the flow of particles and nerve impulses towards the periphery. The aim of our work was to study the influence of the compressive lesion, primarily on the distal motor and sensory latencies. To achieve a better precision, the di

Evaluation of thermography in the diagnosis of selected entrapment neuropathies.

We studied 20 normal subjects, 22 patients with carpal tunnel syndrome, and 15 with ulnar neuropathy at the elbow to compare the diagnostic accuracy of infrared thermography with that of conventional electrodiagnostic studies. We found abnormal thermograms in 55% of patients with carpal tunnel syndrome and 47% with ulnar neuropathy, using 2.5 SD from the normal mean as criteria for abnormality. The abnormalities consisted of either an increase in interside temperature difference in the fingers and hands or an alteration of the normal thenar-hypothenar temperature gradient in the fingers. The s

The Unpredictable Ulnar Nerve-Ulnar Nerve Entrapment from Anatomical, Pathophysiological, and Biopsychosocial Aspects.

Peripheral nerves consist of delicate structures, including a rich microvascular system, that protect and nourish axons and associated Schwann cells. Nerves are sensitive to internal and external trauma, such as compression and stretching. Ulnar nerve entrapment, the second most prevalent nerve entrapment disorder after carpal tunnel syndrome, appears frequently at the elbow. Although often idiopathic, known risk factors, including obesity, smoking, diabetes, and vibration exposure, occur. It exists in all adult ages (mean age 40-50 years), but seldom affects individuals in their adolescence o

[A Case of Guyon's Canal Syndrome Associated with Cubital Tunnel Syndrome].

A 51-year-old man had a 1-year history of numbness on the ulnar side of the 4th finger, the 5th finger, and the ulnar side of the forearm, and weakness of the right hand. The Spurling sign was negative, and cervical radiography and magnetic resonance imaging revealed no abnormality. The Tinel-like signs at the Guyon's canal and cubital tunnel were positive. The diagnosis upon electrophysiological examination was cubital tunnel syndrome and Guyon's canal syndrome, but the possibility of Guyon's canal syndrome was high. Based on the presence of numbness in the forearm, the Tinel-like sign at the

[True neurogenic outlet syndrome: report of 2 cases].

True neurogenic thoracic outlet syndrome is caused by compression of the lower trunk of the brachial plexus usually by a cervical rib, fibrous band or an elongated transverse process of C7. We describe two cases of female patients (23 and 19 years old) with pain in the right superior limb and progressive muscular weakness and atrophy of the intrinsic muscles of hand. Electrodiagnostic studies showed reduced amplitude of compound muscle action potential of median nerve and decreased amplitude of ulnar sensory nerve action potential. Motor and sensory nerve conduction velocities were normal in b
