In the years 1960 through 1966, vagotomy and pyloroplasty was performed for duodenal ulceration in 182 patients. Elective operation was performed in 84% of cases, and intractability of symptoms was the major indication. For emergency operations, bleeding was the main indication. Eight of 182 patients died in the postoperative period (operative mortality 4.4%--3.3% elective, 10.3% emergency). Follow-up of 1 to 17 years has been obtained in 154 of 182 patients (96% of these from 5 to 17 years). Bilateral truncal vagotomy performed in 13 rats reduced weight gain by 26% compared to controls, but also produced gastric distension. Addition of pyloroplasty was found to facilitate gastric drainage in vagotomized rats, and both procedures were then performed in 57 rats of both sexes and varying initial weights. When fed ad libitum, vagotomized animals maintained a weight 14-30% less than controls over periods of 30-300 days. Within the recent 7 years in the clinic 400 operations of trunk vagotomy with drainage of the stomach were performed for complicated ulcers of the duodenum, 110 of them - in perforating ulcer. The postoperative mortality was 1.8%. The immediate and late results were studied within the terms from 6 months to 6 years. Good results were obtained in 79.9%, satisfactory - in 17.6%. Poor issues were noted in 1 patient, who was reoperated upon gastric resection being performed.

With reference to the case-histories of nine patients with necrosis of the gastric wall and to other data culled from the literature, the hypothesis is advanced that occasionally the gastric ulcer may be the result of ischemia of the lesser curvature caused by the devascularization which is part of the HSV procedure, and that such an ulcer and necrosis of the gastric wall are both consequences of ischemia, differing only in degree.
