A Nosocomial Cluster of Tigecycline- and Vancomycin-Resistant Enterococcus faecium Isolates and the Impact of rpsJ and tet(M) Mutations on Tigecycline Resistance.

Tigecycline-resistant enterococci are only rarely detected worldwide. In 2017, the National Reference Centre for Staphylococci and Enterococci noticed a nosocomial cluster of tigecycline- and vancomycin-resistant Enterococcus faecium (TVRE) in a hospital of tertiary care in Northern Germany. Nineteen E. faecium isolates were analyzed by means of antimicrobial susceptibility testing and pulsed-field gel electrophoresis. A subset of isolates was subjected to whole-genome sequencing. The genetic basis of tigecycline resistance was assessed by ResFinder and by comparative analyses to known tetracy

In vivo development of daptomycin resistance in vancomycin-susceptible methicillin-resistant Staphylococcus aureus severe infections previously treated with glycopeptides.

Our aim was to describe the clinical and microbiological features of four cases of severe vancomycin-susceptible methicillin-resistant Staphylococcus aureus (MRSA) infections in which the vancomycin non-susceptibility development and daptomycin resistance occurred under therapy with teicoplanin (three cases) and daptomycin switched to vancomycin (one case). Clinical data were retrospectively reviewed. On nine clinical epidemiologically unrelated daptomycin-susceptible (DAP-S) and daptomycin-resistant (DAP-R) MRSA, we performed: (i) DAP-VAN-TEC-CFX-RIF minimum inhibitory concentrations (MICs);

Vancomycin or Daptomycin Plus a β-Lactam Versus Vancomycin or Daptomycin Alone for Methicillin-Resistant Staphylococcus aureus Bloodstream Infections: A Systematic Review and Meta-Analysis.

Aims: Several in vitro and in vivo studies demonstrated that adding a β-lactam to vancomycin (VAN) or daptomycin (DAP) can provide synergy against methicillin-resistant Staphylococcus aureus (MRSA). However, the results from clinical studies were controversial. The objective of this systematic review and meta-analysis was to compare the efficacy and safety of using VAN or DAP plus a β-lactam (combination therapy) and using VAN or DAP alone (monotherapy) in MRSA bloodstream infections. Methods: We included randomized controlled trials and observational studies evaluating whether combination the

Comparative outcomes of daptomycin and linezolid in vancomycin-resistant enterococcal intra-abdominal infections with bacteraemia.

OBJECTIVE: We set out to see if daptomycin or linezolid (8-12 mg/kg) is effective for vancomycin-resistant enterococci (VRE) intra-abdominal infections (IAIs) because of its efficacy for VRE bloodstream infections (BSIs). METHODS: We conducted a multicentre observational study. Between April 2010 and October 2024, we screened all patients with VRE-BSIs and included those with a documented VRE-IAI treated with daptomycin or linezolid. Limiting enrolment to patients with concurrent bloodstream infections reduced misclassification bias. The primary outcome was 28‑day in‑hospital mortality. RESU

Zanthoxylum beecheyanum enhances vancomycin activity and inhibits biofilm formation in resistant staphylococcus aureus.

The increasing prevalence of multidrug-resistant Staphylococcus aureus continues to compromise the effectiveness of conventional antimicrobial therapy and highlights the need for novel adjunctive treatment strategies. Despite the traditional medicinal use of Zanthoxylum species, the antibacterial potential of Zanthoxylum beecheyanum remains largely unexplored. Here, we characterized the chemical composition of Zanthoxylum beecheyanum essential oil (ZBEO) and evaluated its antibacterial activity against a panel of clinically relevant S. aureus strains, its ability to potentiate vancomycin activ

Vancomycin-resistant enterococcal infections in liver transplant recipients: Clinical impact and therapeutic approaches.

Vancomycin-resistant Enterococci (VRE) are an important cause of infection in liver transplant recipients and are associated with significant morbidity and mortality. Liver transplant candidates and recipients share various risk factors for VRE infections, and colonization has been shown to be associated with increased risk for infection. With limited antimicrobial options and data available for guidance, treatment can be challenging. This review discusses the epidemiology of VRE infections in patients with end-stage liver disease and after liver transplant, as well as therapeutic approaches.

Virulence signature of the endemic vancomycin-resistant Enterococcus faecium clones in Denmark, 2015-2023.

Since 2012, Denmark has seen a significant rise in vancomycin-resistant Enterococcus faecium (VREfm) cases, mirroring trends in other countries, though exceptions occurred during the coronavirus disease 2019 (COVID-19) lockdown and between 2022 and 2023. This rise has been accompanied by ongoing changes in the endemic VREfm clones, reflecting the species' high genetic plasticity. VREfm rapidly acquires plasmids and mobile genetic elements, enriching it with putative virulence markers (PVMs), including surface proteins, pili, and factors encoding biofilm production and adhesion. Impact of Rapid Molecular Diagnostic Testing on Outcomes of Patients With Vancomycin-Resistant Enterococcal Bacteremia.

BACKGROUND: Vancomycin-resistant enterococci (VRE) bacteremia is associated with substantial mortality. Rapid molecular diagnostic testing (RMDT) that detects enterococci and vancomycin resistance genes from positive blood culture broths may lead to earlier appropriate antimicrobial therapy for VRE bacteremia, which could improve clinical outcomes. METHODS: We conducted a retrospective cohort study of patients with VRE bacteremia from 2010 to 2019. RMDT that detects enterococci and vancomycin resistance genes from positive blood cultures was implemented in October 2014. We compared time to ac…

Proven anti-virulence therapies in combating methicillin- and vancomycin-resistant Staphylococcus aureus infections.

INTRODUCTION: Despite years of efforts to develop new antibiotics for eradicating multidrug-resistant (MDR) and multi-virulent Methicillin-Resistant Staphylococcus aureus (MRSA) and Vancomycin-Resistant Staphylococcus aureus (VRSA) infections, treatment failures and poor prognoses in most cases have been common. Therefore, there is an urgent need for new therapeutic approaches targeting virulence arrays. Our aim is to discover new anti-virulence therapies targeting MRSA and VRSA virulence arrays. METHODOLOGY: We employed phenotypic, molecular docking, and genetic studies to screen for anti-vi

Effect of gallium nitrate on the antibacterial activity of vancomycin in methicillin-sensitive and resistant Staphylococcus aureus.

The extension of multidrug-resistant strains of Staphylococcus aureus (S. aureus) is one of the main health challenges in the world, which requires serious solutions to deal with it. Combination therapies using conventional antibiotics and new antibacterial compounds that target different bacterial pathways are effective methods against resistant bacterial infections. Gallium is an iron-like metal that competes with iron for uptake into bacteria and has the potential to disrupt iron-dependent vital processes in bacteria. In this study, we explored the antibacterial effects of gallium nitrate (
