Paediatric varicocele is a well known entity but its effect on adult infertility has not been adequately clarified. Since measurement of testicular volume is currently the best method of estimating the male reproductive potential, 945 boys aged between 13 and 18 years were examined with regard to testicular volume and the incidence of varicocele. Pregnancy rates in 651 subfertile couples in which the man had a varicocele were analysed by life table methods and were not found to be significantly different before and after testicular vein ligation performed in 283 patients. Estimated proportions of couples conceiving were roughly 30% by one year and 45% by two years in both groups. The operation was also not associated with improvement in results of semen analysis. These findings suggest that testicular vein ligation for varicoceles does not improve fertility.

In cases of varicocele the reverse venous blood flow can be detected by the use of the Doppler-stethoscope in a simple way. Applied to the basic examination of fertility patients this method has proved to be a valuable non-invasive screening in the evaluation of varicoceles including cases with minor clinical symptoms. A number of reports have shown that serotonin has a detrimental effect on spermatogenesis and androgen synthesis. Serotonin has 2 mechanisms of action on the testicle: 1) vasoconstrictive changes and 2) direct alterations of testicular metabolism and endocrine activity. Parital obstruction of left renal vein leads to production of varicocele in a fairly short period, i.e.; even within a month. Collateral venous channels play an important role in the formation and degree of varicocele. Varicocele has got a depressive effect on the spermatogenesis on the side of varicocele as well as on opposite testes. Leydig cells and interstitial tissue flourish at the expense of gonadal elements. The factors affecting the spermatogenesis are different from those of other tissues of the testes.

Plasma testosterone concentration was decreased in 10 patients combining varicocele with sexual inadequacy (mean 346.2 ng/100 ml) against normal concentration observed in 23 men with varicocele without sexual disturbances (mean 567.8 ng/100 ml) and in 31 patients with pure psychogenic impotence (mean 581.6 ng/100 ml). In this study the pathophysiological connections in varicocele genesis are discussed, as well as its effect upon spermiogenesis and the causal factors. In 73 patients the high retroperitoneal ligature of the testicular vein was performed. Indication was given either on the basis of subjective complaints or on the proof of subfertility.
