Involvement of the right heart-pulmonary circulation system in systemic sclerosis is a typical feature, with critical prognostic implications. Pulmonary hypertension may occur in association with interstitial lung disease or as a result of an isolated pulmonary vascular disease that may affect both the precapillary arterioles and the postcapillary venules, as well as a consequence of left heart involvement. These apparently different phenotypes often underlie a significant pathophysiologic overlap, which makes the diagnosis and management of these patients highly complex and uncertain.

Liver cysts are a well-recognized feature of autosomal dominant polycystic kidney disease (ADPKD) and occur in 77% of patients more than 60 years old. Serious sequelae, however, are rare, the two most common complications being pain and cyst infections. Portal hypertension has been reported in ADPKD due to the rare presence of congenital hepatic fibrosis. We report a case of ADPKD in a patient who had portal hypertension due to distortion of portal vein and venules by extensive hepatic cysts.

Ethyl alcohol produced graded contractile responses in rat cerebral arterioles and venules in vivo and in isolated canine basilar and middle cerebral arteries at a concentration range (10 to 500 milligrams per deciliter) which parallels that needed for its graded effects of euphoria, mental haziness, muscular incoordination, stupor, and coma in humans. Two specific calcium antagonists, nimodipine and verapamil, prevented or reversed the alcohol-induced cerebrovasospasm and thus may prove valuable in treating the hypertension and stroke observed in heavy users of alcohol.

We present a 3-year-old child with Budd Chiari syndrome having idiopathic complete occlusion of all three major hepatic veins and small hepatic venules. Adequate antegrade flow in right hepatic vein was established by transfemoral balloon angioplasty followed by stenting of the same. Long term antiplatelet therapy was instituted. Medium term follow up reveals satisfactory antegrade flow and regression of symptoms. This strategy highlights an effective nonsurgical approach of restoring physiological pattern of hepatic sinusoidal blood flow in Budd Chiari syndrome.

Steroid responsive encephalopathy associated with autoimmune thyroiditis is a well-recognized complication of autoimmune thyroid disease. However, the characterization of the histopathological features of steroid responsive encephalopathy associated with autoimmune thyroiditis are limited to six cases. Reported pathological features include vasculitis involving venules and arterioles, lymphocytic perivascular cuffs and microglial activation. We report a case of SREAT with biopsy proven (on two occasions) primary CNS demyelination and radiological evidence of steroid responsiveness, identifying…

Ocular fundus photographs of 100 eyes were obtained from an institutionalized population of patients with Down's Syndrome (66 males, 34 females). The vascularity of the optic nerve head was assessed in each photograph by counting the number of arterioles, venules, and fine vessels crossing the disc margin. The results of this analysis were compared to similar data obtained from 100 ocular fundus photographs of normal patients and show that there are more large vessels crossing the disc margin in those with Down's Syndrome.

L-selectin is a cell surface glycoprotein expressed on most leukocyte subsets that mediates leukocyte interaction with ligands on lymphoid tissue high endothelial venule cells as well as with ligands on activated endothelium at sites of inflammation in nonlymphoid organs. Similar to two other members of the selectin family, L-selectin behaves as a lectin, recognizing carbohydrate ligands in a calcium-dependent fashion. Recent in vivo studies reveal the importance of L-selectin expression in both normal and pathologic conditions.

A fatal case of pulmonary venoocclusive disease in a 4-year-old boy is reported, with emphasis on the radiologic features of this condition. The disease is characterized by fibrous intimal obliteration of the lumina of small pulmonary veins and venules leading to severe secondary pulmonary arterial hypertension. The clinical picture is usually indistinguishable from that of other forms of pulmonary arterial hypertnesion, but radiologic features of postcapillary pulmonary hypertension, in conjunction with a normal venoocclusive disease. The diagnosis is definitively established by lung biopsy.

The first description of polyarteritis nodosa (PAN) was in 1852 by Karl Rokitansky, a pathologist at the University of Vienna. The initial report describes a 23-year-old man who had a 5-day history of fever and diarrhea. Since then, the definition of PAN has evolved. The currently accepted definition of PAN comes from the 2012 Chapel Hill Conference, which classified PAN as a necrotizing arteritis not associated with antineutrophil cytoplasmic antibodies of medium or small arteries without glomerulonephritis or vasculitis in arterioles, capillaries, or venules.

Wegener's granulomatosis is characterized by necrotizing granulomatous inflammation and necrotizing vasculitis affecting predominantly small arteries, arterioles, capillaries, and venules. In contrast to the well-described pulmonary parenchymal involvement of Wegener's granulomatosis, the lower airway (tracheobronchial) disease manifestations are less well recognized by clinicians. Consequently, mild disease of the airways is easily missed. There is a relative paucity of published information on various tracheobronchial manifestations of Wegener's granulomatosis. This article provides a compre…

Reconstructive microsurgery is now in a new stage of supermicrosurgery. With this technique, very tiny (0.3mm) vascular anastomoses are possible. In this paper, we describe two cases of successful fingertip replantations employing arteriole (terminal branch of digital artery) anastomoses, the arteriole graft being obtained from the same fingertip defect, reverse arteriole flow to subdermal venule, and delayed venular drainage for venous congestion. These atypical tiny vascular anastomoses were successfully carried out under digital block.

Retinal neovascularization, a rare complication of ocular toxoplasmosis, is a source of vitreous hemorrhage. We examined three patients (all women, 70, 36, and 19 years old) with inactive toxoplasmosis scars associated with occlusion of a retinal arteriole or venule passing through the scar, capillary nonperfusion, and adjacent retinal neovascularization. In two patients photocoagulation of the ischemic areas produced a cessation of hemorrhage and the disappearance of neovascularization. In the third case, the neovascular frond was avulsed from its supplying retinal arteriole without treatment

Microscopic polyangiitis (MPA) is a systemic disease described as necrotizing vasculitis that affects capillaries, venules and arterioles. Hormone-based immunosuppression therapy is the common treatment, but with severe side effects. Hence, non-hormone or low-hormone therapies may be another option. Here we report a case of a MPA patient, who was first diagnosed MPA at the age of 56, experienced a number of recurrences from 63 to 70 that were successfully controlled by Traditional Chinese Medicine (TCM) alone or with low-dose hormone.

The histopathology of Gottron's papules has not been often reported. Morphological descriptions of these lesions are mainly focused on changes of the epidermis and interface, and on the nature of the inflammatory infiltrate. Dilated vascular vessels of Gottron's papules are rarely mentioned in the literature. We studied these vessels in a biopsy specimen obtained from Gottron's papule in a 65-year-old woman with D2-40 antibody. Although most of the vessels were identified as blood vessels (venules and capillaries), some were demonstrated to be dilated lymphatic vessels.

Diseases that cause hyperviscosity of the blood can result in otologic symptoms--especially vertigo. These symptoms are predominantly produced by peripheral vestibular involvement. The pathophysiology probably involves vascular obstruction in the venules. Specific changes in the microvasculature are presumed to be comparable to those that occur with hyperviscosity disorders in the retina. Maintenance of a normal blood viscosity will prevent damage to the ear, as well as other organs. Additionally, reduction of the blood viscosity can greatly improve otologic symptoms. Case studies and a review…

We describe two patients who experienced small-vessel vasculitic syndromes after influenza immunization. Their illnesses were characterized by fever, arthralgias, and myalgias; uveitis and optic neuritis occurred in one patient, while the other had palpable purpura, which histologically was proved to be a cutaneous necrotizing venulitis. No viral or bacterial infection, medications, or underlying systemic rheumatic disease could be implicated. To our knowledge, small-vessel vasculitis occurring after influenza vaccination has not been previously reported.

BACKGROUND: Indocyanine green (ICG) lymphography gained widespread acceptance as a gold standard in lymphatic surgery, although this imaging modality has several limitations. Ultrasound is progressively emerging as a powerful tool in LVA preoperative planning, as it expands the available knowledge for both lymphatic channels and recipient venules. The purpose of this study is to evaluate the accuracy of ultrahigh frequency ultrasound (UHFUS) in detecting the lymphatic vessels and in evaluating their degeneration status, including wall and lumen characteristics, compared with direct histology o…
