Late complications of vertebral artery dissection in children: pseudoaneurysm, thrombosis, and recurrent stroke.

Craniocervical arterial dissection is an important cause of childhood arterial ischemic stroke, accounting for 7.5% to 20% of cases. Significant neurologic morbidity and mortality may result and recurrence risk may be higher than in adults. However, the natural history and long-term outcome of pediatric dissection are poorly studied. We report 3 cases of extracranial vertebral artery dissection with complications including pseudoaneurysm formation, recurrent stroke, and late spontaneous thrombosis of the dissected artery. These cases illustrate the dynamic processes involved in vascular injury…

Traumatic carotid and vertebral artery dissection in a professional jockey: a cautionary tale.

Jockeys accept bony fractures and soft tissue injuries as occupational hazards. An average National Hunt jockey falls once in ten races with an injury rate of 4.25%. Head injury is a common cause of morbidity and the benefit of helmets is well recognized. Neck injuries are also common and usually musculoskeletal. Although rare, trauma to the neck arteries may go unnoticed yet have catastrophic consequences. Internal tears can allow arterial blood to dissect the layers of the arterial wall and obstruct the lumen. Severe obstruction may lead to cerebral ischaemia and infarction. An appreciation

Vertebral artery dissection as the initial manifestation of primary antiphospholipid syndrome in a man.

Antiphospholipid syndrome (APS) is a prothrombotic autoimmune disorder that is associated with arterial and venous thrombosis. Cervical artery dissection is an uncommon cause of stroke and has rarely been reported with APS. We report the case of a 46-year-old man who presented with acute dizziness, vertigo, neck pain, and headache after jujitsu training. Neuroimaging showed right vertebral artery dissection from V1 to V4, and magnetic resonance imaging revealed a right cerebellar infarction. Evaluation revealed persistently elevated lupus anticoagulant titers on repeat testing at 12-week inter…

Bilateral Vertebral Artery Dissections Associated With Postpartum Preeclampsia: A Case Report.

Postpartum vertebral artery dissections (VADs) are infrequently reported; although data are limited, several case reports describe VAD occurring in the setting of postpartum preeclampsia. We present a 39-year-old G3P3003 (P1 term stillbirth) who developed bilateral VADs in the setting of postpartum preeclampsia and a persistent, nonfocal occipital headache. Six days after an uncomplicated cesarean delivery, the patient developed an unrelenting occipital headache accompanied by severe-range blood pressures. She met diagnostic criteria for postpartum preeclampsia. Despite treatment with magnesiu…

Angiographic characteristics of ruptured versus unruptured vertebral artery dissecting aneurysm.

OBJECTIVE: Vertebral artery dissecting aneurysm (VADA) is a rare and critical disease. VADA rupture can cause subarachnoid hemorrhage which is a major complication of VADA due to their high rebleeding rate and poor outcome. In the present study, ruptured and unruptured VADAs were compared by analyzing angiographic findings to determine useful predisposing factors for VADA rupture for appropriate treatment selection. METHODS: Subjects with VADA treated during a 10-year period were retrospectively identified. The 57 cases diagnosed with VADA were divided into ruptured (n=15) and unruptured (n=4

Cervical Cord Infarction Caused by Dissection of the Intracranial Segment of the Vertebral Artery.

Cervical cord infarction is uncommon but has been increasingly reported as a complication of vertebral artery dissection (VAD). A 54-year-old woman presented with neck pain and neurological deficit following sudden neck movement. Radiological findings suggested cervical cord infarction in the anterior spinal artery territory at the C5-C6 vertebral level and dissection of the intracranial segment of the right vertebral artery. Cervical cord infarction due to VAD is usually caused by dissection of its extracranial segment. The present case indicates that dissection of the intracranial segment of…

Postpartum spontaneous coronary, vertebral, and mesenteric artery dissections: a case report.

BACKGROUND: Spontaneous coronary artery dissection is a rare cause of myocardial infarction that must always be considered on a clinician's differential diagnosis, particularly in patients <50-years old with a paucity of typical vascular risk factors. CASE PRESENTATION: We describe a case of a 33-year-old white woman, 3 weeks postpartum, presenting with retrosternal chest and back pain, neck pain and stiffness, and intermittent headaches. Subsequent workup revealed concurrent spontaneous dissections in three separate medium-sized arterial beds. CONCLUSIONS: She was successfully managed in a

Treatment of a dissecting vertebral artery aneurysm with angioplasty and the pipeline embolisation device.

A 49-year-old woman was admitted with Hunt and Hess grade 1 subarachnoid haemorrhage. A cerebral aneurysm of the intracranial left vertebral artery (VA) distal to the left posterior inferior cerebellar artery (PICA) was found and treated using parent vessel occlusion with coils. The PICA was preserved. Angiography at three time points during her hospitalisation showed a normal right VA. Eight months later, angiography demonstrated a new 14 mm right VA dissecting aneurysm with 90% outflow stenosis and recurrence of the left VA aneurysm distal to the coils. A Marksman microcatheter would not tra

Endovascular management of vertebral artery dissecting aneurysms: review of 25 patients.

AIM: Management of Vertebral Artery (VA) dissections remains controversial. The clinical and angiographic variables of VA dissections were evaluated to demonstrate the safety and efficacy of endovascular intervention in treatment of VA dissecting aneurysms. MATERIAL AND METHODS: 25 patients with 27 VAdissecting aneurysms were treated with endovascular intervention during the last 10 years.17 patients were admitted with subarachnoid hemorrhage. 23 aneurysms treated using destructive endovascular trapping, while reconstructive techniques were used in 3 aneurysms treated with stent-assisted coil

Spontaneous dissection of the carotid and vertebral arteries: the 10-year UCSD experience.

The etiology of spontaneous dissection of the carotid and vertebral arteries without antecedent trauma remains unclear. The goal of this 10-year review was to examine factors regarding presentation, diagnosis, treatment, and outcome for all patients at our institution who were diagnosed with spontaneous carotid dissections (SCD) or spontaneous vertebral dissections (SVD) with no prior trauma history. A retrospective chart analysis was performed involving all discharges from UCSD Medical Center from 1995 to 2005. Patients were selected for inclusion based on the diagnosis of carotid or vertebra

Lethal vertebral artery dissection in pregnancy: a case report and review of the literature.

Subarachnoid hemorrhage represents a rare event in pregnancy with a high mortality rate. We present the case of a 39-year-old pregnant woman who developed right vertebral artery dissection with subsequent massive subarachnoid hemorrhage with fatal outcome. The macroscopic and microscopic autopsy findings are described. A review of the literature with a discussion of the varied predisposing factors for vertebral artery dissection and subarachnoid hemorrhage and the rarity of these events in pregnancy is provided.

Dissection of the V4 segment of the vertebral artery: clinicoradiologic manifestations and endovascular treatment.

Intracranial vertebral artery (VA) dissection has three clinical presentations: ischemia, hemorrhage, and mass effect. Imaging findings of intracranial VA dissections vary according to clinical presentation. Irregular stenosis or occlusion of the VA is the most common finding in patients with posterior fossa infarction, whereas a dissecting aneurysm is the main feature in those with acute subarachnoid hemorrhage. A chronic, giant, dissecting aneurysm can cause mass effect on the brain stem or cranial nerves, as well as distal embolism. Magnetic resonance imaging is useful for detection of intr

Prediction of rebleeding from angiographic features in vertebral artery dissecting aneurysms.

To identify patient characteristics and angiographic features that predict high risk for rebleeding in vertebral artery (VA) dissecting aneurysms. We analyzed 62 patients treated for subarachnoid hemorrhage (SAH) from VA dissecting aneurysms (male: female, 46:16; mean age, 51.7 +/- 8 years). Univariate and multivariate stepwise logistic regression analyses were performed to assess relationships between rebleeding rate and age, gender, history of hypertension, sidedness of the aneurysm, angiographic configuration, and location relative to the origin of the posterior inferior cerebellar artery (

Stent-within-a-stent technique for the treatment of dissecting vertebral artery aneurysms.

Subarachnoid hemorrhage from dissecting vertebral artery aneurysms is a well-known phenomenon. With the advent of navigable intracranial stents, these aneurysms are now amenable to endovascular stent placement. However, immediate aneurysm obliteration is often not accomplished, and current covered stents have poor short-term patency rates. We report a series of three patients with intradural dissecting vertebral artery aneurysms treated with a novel endovascular stent-within-a-stent construct using currently available stent technology.
