The syndrome of paroxysmal positional vertigo. A review.

Paroxysmal positional vertigo is one of the most common types of vertigo. When there are no other neurologic abnormalities and the vertigo occurs only with changes in head position and is associated with a direction-fixed fatiguing nystagmus that has a latency of onset and a brief duration, the term "syndrome of paroxysmal positional vertigo" applies. This syndrome is usually idiopathic and is rarely seen in association with central nervous system disease. The diagnosis usually can be made on the basis of a careful history and physical examination without costly laboratory procedures. Habituat…

[Differential diagnosis of tinnitus and vertigo. A review].

Tinnitus and vertigo, two common neurological complaints, often challenge the physician's ability with respect to possible etiology. Objective tinnitus can result from an abnormally patent eustachian tube, from tetanic contractions of the muscles of the soft palate, or from vascular abnormalities within the head or neck. Subjective tinnitus refers to lesions involving the external ear canal, tympanic membrane, ossicles, cochlea, auditory nerve, brainstem, and cortex. As many as 50% of patients with tinnitus do not exhibit associated hearing loss; in these patients, the cause of the tinnitus is…

Hearing loss with delayed onset of vertigo.

The association between the delayed onset of episodic true vertigo and previous sensorineural hearing loss has been previously described. We have reviewed our series of sixty consecutive labyrinthectomies to determine the prevalence of this disorder in this population, and found a variety of etiologic agents of sensorineural hearing loss with subsequent delayed vertigo. Relief from vertigo following surgery has confirmed that the previously deafened ear has been the cause of the vertigo. Case histories are presented and the literature is reviewed.

A study of vertigo and dizziness in the premenstrual period.

Twenty-five patients with nausea, vomiting and vertigo or dizziness in the premenstrual period were examined. In the week before menstruation, four of them had spontaneous nystagmus, six had positional nystagmus, and nine of them showed abnormal caloric responses. Most of them showed no abnormality during the week of menstruation. The caloric-induced responses were 'labyrinthine' in type in seven cases, and 'central' in type in two cases, thus denoting that cerebral oedema is not the only factor which may produce vertigo in the premenstrual period; as suggested by the present study, labyrinthi…

Analysis of surgical procedures in patients with vertigo.

The majority of patients with vertigo secondary to inner ear problems can be cured by surgery (80%). The results after neurectomy procedures such as cochleovestibular neurectomy or vestibular neurectomy appear to be much better (85%) than after the endolymphatic subarachnoid shunt procedure (70%). If hearing is worth saving, a conservative procedure should be used first in an effort to preserve the patient's hearing. If this fails, a cochleovestibular neurectomy will usually resolve the problem. At present we are using the retrolabyrinthine vestibular neurectomy as a primary procedure for Meni…

Transdermal scopolamine in the treatment of acute vertigo.

A double-blind, parallel investigation was undertaken to evaluate the efficacy of transdermally delivered scopolamine hydrobromide in acute vertigo as compared to meclizine hydrochloride and placebo. Statistical analysis of subjective patient responses and clinical observations suggested that both scopolamine and meclizine are more effective than placebo. Both have significant, but different, side effects.

Sudden deafness and vertigo in children and juveniles.

The 17 cases of sudden deafness in children and juveniles treated in the HNO-Universitäts-Klinik Erlangen between August 1977 and December 1980 are analyzed and correlations of symptoms and possible etiologies are discussed. There is no unique clinical picture. Vascular reasons do not play an important role contrary to adult patients. Unexpectedly viral infections are not of great importance. Young patients who had a sensorineural hearing loss prior to the sudden deafness seem to have a bad prognosis for therapy. Vestibular involvement seems to be of no meaning for a successful therapy in chil
