Discordance Between Subjective Visual Vertical and Vestibular Evoked Myogenic Potentials in Stroke.

BACKGROUND: While there has been a significant correlation between subjective visual vertical (SVV) and ocular vestibular myogenic potentials (oVEMPs) tests in acute vestibular neuritis (AVN), the association between these two tests in central vestibular lesions remains uncertain. METHODS: We prospectively recruited 58 patients with acute stroke involving the posterior circulation territory (PCS) between 2019 and 2022. For comparison, 32 patients with AVN served as a control. We evaluated sustained utricular function through SVV tests and transient utricular function through bone-conducted (B

Eye-centered representation of optic flow tuning in the ventral intraparietal area.

Reference frames are important for understanding sensory processing in the cortex. Previous work showed that vestibular heading signals in the ventral intraparietal area (VIP) are represented in body-centered coordinates. In contrast, vestibular heading tuning in the medial superior temporal area (MSTd) is approximately head centered. We considered the hypothesis that visual heading signals (from optic flow) in VIP might also be transformed into a body-centered representation, unlike visual heading tuning in MSTd, which is approximately eye centered. We distinguished among eye-centered, head-c

Acute unilateral peripheral vestibulopathy in neurosyphilis.

INTRODUCTION: Neurosyphilis producing basal meningitis presenting as sequential transient cranial nerve palsies was well recognized before the antibiotic era. OBJECTIVE: To report two patients presenting with acute unilateral peripheral vestibulopathy due to syphilitic basal meningitis. RESULTS: In Case 1 basal meningitis occurred early in the secondary phase of the infection, in Case 2 in the late latent phase. The diagnosis was not made immediately in either case; in Case 1 after previous presentation with increasing hearing loss and then with facial palsy and then a subsequent presentatio

Vestibular neuritis: etiopathogenesis.

Vestibular neuritis presents as sudden unilateral vertigo in the absence of hearing loss or neurologic involvement and is thought to be due to neurotropic viruses. Its morbidity is unknown and it affects both sexes equally, with the highest incidence at 40-50 years of age. The etiology of this condition has been ascribed to viral, bacterial and protozoan infections, as well as allergic and auto-immune causes. Inflammation of the vestibular nerve is followed by demyelination and loss of function, which is not always reversible. Higher plasma fibrinogen and CRP levels in the acute phase, longer

Timing of Delamination of Inner Ear Neurons Specifies Their Topography and Target Innervation.

The neurons of the inner ear delaminate from a sensorineurogenic epithelium in the ventral part of the otocyst. Delaminated neuroblasts then condense to form the acoustico-vestibular ganglion (AVG). As they differentiate, the neurons connect mechanosensory hair cells (HCs) of the inner ear with their targets in the hindbrain in a precise topographical manner. However, it is unclear how or when positional identities of neurons are specified within the AVG, such that topographical information from HCs is maintained into the auditory centers of the brain. Here, we find that the time of delaminati

Cellular basis of vestibular compensation: analysis and modelling of the role of the commissural inhibitory system.

In this study we used a cellular network model of the brainstem vestibulo-ocular reflex (VOR) pathways to investigate the role of the vestibular commissural system in "vestibular compensation", the behavioural recovery that takes place after unilateral labyrinthectomy (UL). The network was initialized on the basis of mathematical analysis and trial simulations to generate a VOR response with a physiologically realistic gain and time constant. The effects of a selective decrease in the strength of commissural inhibitory input to the ipsi-lesional medial vestibular nucleus (MVN) neurones, withou

The nucleus of the optic tract. Its function in gaze stabilization and control of visual-vestibular interaction.

1. Electrical stimulation of the nucleus of the optic tract (NOT) induced nystagmus and after-nystagmus with ipsilateral slow phases. The velocity characteristics of the nystagmus were similar to those of the slow component of optokinetic nystagmus (OKN) and to optokinetic after-nystagmus (OKAN), both of which are produced by velocity storage in the vestibular system. When NOT was destroyed, these components disappeared. This indicates that velocity storage is activated from the visual system through NOT. 2. Velocity storage produces compensatory eye-in-head and head-on-body movements through

Diverse spatial reference frames of vestibular signals in parietal cortex.

Reference frames are important for understanding how sensory cues from different modalities are coordinated to guide behavior, and the parietal cortex is critical to these functions. We compare reference frames of vestibular self-motion signals in the ventral intraparietal area (VIP), parietoinsular vestibular cortex (PIVC), and dorsal medial superior temporal area (MSTd). Vestibular heading tuning in VIP is invariant to changes in both eye and head positions, indicating a body (or world)-centered reference frame. Vestibular signals in PIVC have reference frames that are intermediate between h

Noradrenergic pathways involved in the development of vertigo and dizziness--a review.

In this study, vestibular caloric stimulation (CS) inhibited noradrenergic (NA) neurons of the locus coeruleus (LC) in rats. The vestibular input can be modified by the ventrolateral medulla (VLM), which then inhibits the LC neuronal activity via GABAA receptors. Clinically, CS induces vertigo in humans. Thus, LC-NA inhibition may be involved in the development of vertigo. Moreover, it is speculated that Sopite syndrome, one of the major symptom complexes of motion sickness, is also evoked by LC-NA inhibition. The central LC-NA neuronal system may participate in vertigo and motion sickness ind

Functional brain imaging: a window into the visuo-vestibular systems.

PURPOSE OF REVIEW: Advances have been made in identifying how areas involved in processing vestibular, ocular motor, and visual information are represented in the human cortex as well as the cortical interaction between these systems in healthy subjects. RECENT FINDINGS: While we know how some vestibular and ocular motor disorders modify visuo-vestibular interaction by changing the 'normal' cortical activation-deactivation patterns, it is still early days in functional magnetic resonance imaging studies of patients with specific disorders. Findings from current brain imaging studies of severa
