Eye injuries in patients with major trauma.

BACKGROUND: A study was performed to determine the type and frequency of ocular injuries in patients with major trauma. METHODS: All patients with ocular and adnexal injuries (n = 178) among 1,119 patients admitted with major trauma (Injury Severity Score >15) to the Royal Prince Alfred Hospital from July 1990 to December 1997 were analyzed. RESULTS: Sixteen percent of the major trauma cohort had ocular or orbital trauma. Fifty-five percent of patients with injuries involving the face had ocular or orbital injuries. A range of ocular injuries was seen. Analysis of the major trauma cohort sho

Refractive extended depth-of-focus lens design based on periodic power profile for presbyopia correction.

PURPOSE: Limitations of existing diffractive multifocal designs for presbyopia correction include discrete foci and photic phenomena such as halos and glare. This study aimed to explore a methodology for developing refractive extended depth-of-focus (EDoF) lenses based on a periodic power profile. METHODS: The proposed design technique employed an optical power profile that periodically alternated between far, intermediate and near distances across the pupil radius. To evaluate the lens designs, optical bench testing was conducted. The impact on visual performance was assessed using a spatial

How to assess eye movements clinically.

Eye movements serve vision in orienting gaze toward an object of interest in order to place its image simultaneously on both foveas and in stabilizing gaze relative to the environment in order to maintain fixation on the object of interest, even in the case of body displacement. Disorders of eye movements can interfere with ocular alignment and/or monocular motility, and result in diplopia, which is the most common symptom. Eye movement disorders can also interfere with binocular motility without ocular misalignment and result in gaze palsy. Finally, disorders of eye movement can interfere wit

The use of progressive addition lenses to improve the daily visual function of children receiving topical atropine treatment.

PURPOSE: To evaluate the changes in visual function when progressive addition lenses (PAL) are added in children using topical atropine as a myopia control therapy. Daily visual complaints and the determination of their near correction were studied. METHODS: Forty children aged 7-12 years were recruited. Distance and near visual acuity, accommodative lag, heterophoria, near point of convergence and stereopsis were examined, and a questionnaire of daily visual complaints was administered. RESULTS: Significant differences in visual functions were found after the near correction was prescribed.

Ocular lateral deviation with brief removal of visual fixation differentiates central from peripheral vestibular syndrome.

OBJECTIVE: Ocular lateral deviation (OLD) is a conjugate, ipsilesional, horizontal ocular deviation associated with brief (3-5 s) closing of the eyes, commonly linked to the lateral medullary syndrome (LMS). There is limited information regarding OLD in patients with the acute vestibular syndrome (AVS). In one case series 40 years ago OLD was suggested to be a central sign. Recently, horizontal ocular deviation on imaging (RadOLD) was frequently associated with anterior circulation stroke and horizontal gaze palsy. Similarly, RadOLD has been associated with posterior circulation stroke, e.g.,

Deficient vergence prism adaptation in subjects with decompensated heterophoria.

Vergence prism adaptability was evaluated in subjects with high symptomatic and asymptomatic heterophoria and compared to individuals with a heterophoria within normal range (the control group). A computer haploscope was used to measure phoria values and changes in the eye position after introducing 6 prism diopters base out in front of the right eye. Phoria values were measured with a nonius paradigm every minute for a period of 10 minutes during adaptation. The results showed that subjects with symptomatic heterophoria are characterized by a lower rate of prism adaptation and adapted to a sm

Induced visual fading of complex images.

Visual stimuli fade from awareness under retinal stabilization or careful fixation, a phenomenon documented by Troxler more than 200 years ago. Research on visual fading during normal visual fixation typically has been restricted to discrete, simple, low-contrast shapes presented peripherally against a uniform or textured background. In four experiments, we document a striking new visual fading effect in which entire photographs of scenes fade to a uniform luminance and hue during normal visual fixation. Critically, this "scene fading" can be induced almost instantaneously by some types of vis

The underachieving child.

Visual factors in specific learning difficulties (SpLD) are reviewed. People with SpLD fail to achieve at a level that is commensurate with their intelligence. The commonest SpLD is dyslexia, which usually results from phonological processing/decoding deficits. Additionally, there are several optometric correlates of SpLD which may, in some cases, contribute to the learning difficulty. These correlates include binocular instability and a low amplitude of accommodation. Some people with reading difficulties and perceptual distortions/eyestrain can be helped by individually prescribed coloured f

The linearity of proximally induced accommodation and vergence.

This study sought to determine the linearity and amplitude of both proximally induced accommodation (PIA) and vergence (PIV) by comparing the open-loop accommodation and vergence responses, respectively, to individual stimuli located at viewing distances ranging from 0.20 m to 1,500 m. For the measurement of PIA, the vergence and accommodation loops were opened by subjects (N = 10) monocularly viewing the target through a 0.5-mm pinhole, while accommodation was assessed concurrently using an infrared optometer. In the assessment of PIV, the accommodation loop was opened either by subjects (N =

Characteristics of convergence insufficiency.

Convergence insufficiency (CI) is a well-known syndrome of binocular visual dysfunction. In a review of 58 papers, considerable variation was noted in the criteria used to define the condition. Symptoms and decreased positive fusional vergences (both at the nearpoint) were the only criteria named in more than one-half of the studies. An extended nearpoint of convergence (NPC) and increased exophoria at near were criteria in about one-third of the papers. Examination of data in the reviewed papers shows that although considerable variability was noted, the distance exodeviation, distance negati

Adaptation to lens-induced heterophorias.

In previous studies we have shown that subjects with normal binocular vision adapt to prism-induced heterophorias. In this study we have continued the work by investigating whether the oculomotor system can also adapt to lens-induced heterophorias. It was found that subjects gradually adapted to the esophoria induced when viewing a distance chart through negative lenses. Adaptation also occurred to lens-induced esophoria and exophoria at near, while at distance, where the visual feedback was unaltered, the phoria remained virtually constant. These findings demonstrate the flexibility of the oc

National survey of paediatric vision screening programs across Canada: Identifying major gaps and call to action.

OBJECTIVE: Paediatric vision screening programs identify children with ocular abnormalities who would benefit from treatment by an eye care professional. A questionnaire was conducted to assess existence and uptake of school-based vision screening programs across Canada. A supplementary questionnaire was distributed among Ontario's public health units to determine implementation of government mandated vision screening for senior kindergarten children. METHODS: Chief Medical Officers of Health for each province and territory, and Ontario's thirty-four public health units were sent a questionna

Distance vergence adaptation is abnormal in subjects with convergence insufficiency.

It is well known that in convergence insufficiency (CI) prism adaptation is reduced in response to base-out (BO) prisms at near. There have also been some suggestions in the literature that adaptation is reduced at other distances as well. The present data show that in CI adaptation is not only reduced in response to BO at near, but also in response to base-in (BI) at near and for both BI and BO at distance. This raises the interesting question whether distance adaptation becomes reduced because of reduced near adaptation or whether these subjects have a generally reduced horizontal adaptation

Analysis of tonic and accommodative vergence disorders of binocular vision.

Tonic and accommodative vergence disorders of binocular vision are analyzed with separate methods. The center of symmetry and the associated phoria of forced duction fixation disparity curves are two criteria used for prismatic correction of tonic vergence disorders. The associated phoria is corrected physiologically when patients who have tonic disorders are able to develop prism adaptation with orthoptic exercises. Graphical methods are used to analyze accommodative disorders resulting from mutual interactions between accommodation and convergence. Graphic analysis illustrates that patients
