Femoral lengthening using limb reconstruction system augmented with only one antegrade flexible nail in children: early results.

Several techniques have been described for decreasing the duration of external fixator use, augmenting stability, and minimizing complications. The purpose of the present study was to evaluate the clinical results and complications of femoral lengthening procedures using the Limb Reconstruction System (LRS) in combination with a single antegrade flexible intramedullary nail (FIN). Femoral lengthening with LRS and FIN was applied to 14 patients (aged 6-16 years) between 2017 and 2021. The etiology was a congenital femoral deficiency in 12 patients and post-traumatic growth arrest in two. A sing

Dual modulation of bone formation and resorption with zoledronic acid-loaded biodegradable magnesium alloy implants improves osteoporotic fracture healing: An in vitro and in vivo study.

UNLABELLED: Osteoporotic fracture (OPF) remains a major clinical challenge for skeletal regeneration. Impaired osteogenesis and excessive remodeling result in prolonged and poor quality of fracture healing. To augment bone formation and inhibit excessive resorption simultaneously, we constructed a biodegradable magnesium-based implant integrated with the anti-catabolic drug zoledronic acid (ZA); this implant exhibits controllable, sustained release of magnesium degradation products and ZA in vitro. The extracts greatly stimulate the osteogenic differentiation of rat-bone marrow-derived mesench

[Technic and results of clavicular medullary wiring].

Pseudo-arthrosis was recorded from 13 per cent of patients whose clavicular fractures had been treated by wire fixation. Too short fragmental splinting, postoperative diastasis, use of too thin wires (2 mm in diameter) or intricate shapes of fracture (fragmental) were causes of poor results. Neither parallel nor sagittal skin incision was cosmetically acceptable. Kirschner's wire fixation, when applied to short fractures in the central third of the clavicula--has proved to be a simple and promising method of stabilisation.

Finite-element modelling of femoral shaft fracture fixation techniques post total hip arthroplasty.

The presence of a femoral prosthesis superior to a shaft fracture severely complicates fixation and treatment. This study uses two-dimensional, multithickness, plane stress finite-element models of a femur with prosthesis to investigate the stresses developed with the application of three popular fixation techniques: revision to a long stem prosthesis, lateral plating with a cortical bone allograft strut and cerclage wires, and custom plate application with proximal Parham band fixation with distal cortical screws (Ogden plate). The plate and bone contact as well as the fracture site contact w

Cost-effectiveness and quality-of-life outcomes of proximal humerus fracture treatment in adults aged 65 years and older in the United States.

BACKGROUND: Proximal humerus fractures (PHFs) are the third most common fragility fracture in older adults, and their rising incidence in the United States contributes substantially to health care spending. Although several randomized trials have evaluated outcomes across treatment strategies, the optimal management strategy remains debated. Few studies have compared all five common PHF treatment options using an integrated cost effectiveness framework. METHODS: A cost-utility analysis was performed to compare nonoperative management, intramedullary nailing, open reduction internal fixation,

Surgical load in major fractures - results of a survey on the optimal quantification and timing of surgery in polytraumatized patients.

PURPOSE: It is known that the magnitude of surgery and timing of surgical procedures represents a crucial step of care in polytraumatized patients. In contrast, it is not clear which specific factors are most critical when evaluating the surgical load (physiologic burden to the patient incurred by surgical procedures). Additionally, there is a dearth of evidence for which body region and surgical procedures are associated with high surgical burden. The aim of this study was to identify key factors and quantify the surgical load for different types of fracture fixation in multiple anatomic regi

The accumulation of technical errors exponentially increases the risk of screw cut-out in femoral intramedullary nailing.

PURPOSE: Proximal femur fractures primarily affect the elderly, with significant morbidity, mortality, and socioeconomic impact. The main complication of short trochanteric intramedullary nailing is the cut-out of the cervicocapital screw through the femoral head. The objective of this study was to analyze the influence of technical errors in short trochanteric intramedullary nailing for the treatment of trochanteric femur fractures on the mechanical failure of osteosynthesis. METHODS: A total of 540 patients who underwent surgery for a trochanteric femur fracture using short trochanteric int

Comparison of fluoroscopic versus electromagnetic distal locking in humeral shaft osteosynthesis using intramedullary nail: a retrospective study.

PURPOSE: Intramedullary nailing is a standard method for osteosynthesis of diaphyseal humeral fractures. Distal locking, however, typically requires fluoroscopic guidance, which increases radiation exposure. This study aimed to compare standard fluoroscopy-guided distal locking with the Ezy-Aim system (Austofix, Australia) based on electromagnetic pulse detection from the perspectives of surgical times, radiation exposure, and complications. METHODS: A retrospective study included 58 patients treated for diaphyseal humeral fractures (AO/OTA 12A, 12B) between 2019 and 2023. Group A (n = 28) re

Impact of two different intramedullary nail fixations performed through the knee joint on knee osteoarthritis: A retrospective radiological analysis.

Although retrograde femoral (RGF) and suprapatellar approach tibial (SPT) nails performed through the knee joint have shown successful outcomes, their effects on knee osteoarthritis remain unclear. This study aimed to evaluate the radiological effects of RGF and SPT nailing on knee osteoarthritis progression. Between January 2014 and February 2024, 35 patients who underwent RGF (Group 1) or SPT (Group 2) nailing met the study criteria. We retrospectively reviewed digital archives and radiological images and recorded demographic data, trauma type, comorbidities, complications, fracture healing

Retrograde tibial intramedullary nailing for distal tibial fractures: outcomes and complications.

BACKGROUND: Retrograde tibial nailing (RTN) is a relatively new, minimally invasive technique for managing distal tibial fractures. Studies have demonstrated that RTN provides superior biomechanical stability compared to locking plates and yields favorable clinical outcomes. However, clinical data on RTN remain limited, particularly regarding its complication profile. This study aimed to evaluate the clinical efficacy of RTN in distal tibial fracture treatment, with a focus on identifying and analyzing associated complications. METHODS: A retrospective review was conducted on patients who und

Effect of Preoperative Nutritional Status on Postoperative Functional Recovery of Hip Joint in Elderly Patients with Intertrochanteric Fractures.

OBJECTIVE: The state of nutrition of senior patients with intertrochanteric fracture of femur before operation affects the patients' tolerance to the operation, the body recovery, healing of the wound and clinical prognosis. For these patients, the poor state of nutrition may lengthen the time of being in hospital, leading to poor hip recovery and clinical outcome. But currently, the relationship between functional recovery of hip joint after operation of intertrochanteric fracture in elderly patients and camp condition has not been reported. To investigate the effect of preoperative nutritiona

Minimally Displaced Humeral Lateral Condyle Fractures: Immobilize or Operate When Stability Is Unclear?

BACKGROUND: Either casting or in situ fixation (to prevent displacement) are recommended for minimally displaced pediatric lateral condyle fractures of indeterminate stability with ≤2 mm lateral displacement and narrowed fracture extension to the epiphyseal articular cartilage. This study compares casting only, acute prophylactic in situ pinning, and surgery if casting fails due to displacement. METHODS: In total, 738 fractures at a level 1 trauma center between 2008 and 2014 were reviewed. All fractures were assigned a Song classification and Song 2 fractures followed to union were analyzed.
