INTRODUCTION: Blood pressure (BP) control is assessed annually from patients on Renal Replacement Therapy at renal centres in England, Wales and Northern Ireland by the UK Renal Registry.
METHODS: Patients alive and receiving RRT on 31st December 2007 with a BP reading in either the fourth or third quarter of 2007 were included. Summary statistics were calculated for each renal centre, nation and renal disease category. Linear regression analyses were performed for prevalent patients between 2000 and 2007.
RESULTS: Significantly more haemodialysis patients achieved the BP standard (44.6% pre

The bioactivity of glomerular VEGF (or activity of available VEGF) is critical to the physiological maintenance of the glomerular filtration barrier. Disturbances in glomerular VEGF expression have been linked to numerous glomerulopathies, highlighting its importance in disease progression within the kidney. However, the changes in expression are not consistent between conditions; enhanced expression sometimes appears to have a renoprotective effect, yet at other times it appears destructive. Also, the level of expression can change with the progression of disease. This review focuses on how o

BACKGROUND: Microalbuminuria (MA) is associated with increased risk of cardiovascular and possibly chronic kidney disease (CKD). Obesity has been linked to MA, though the prevalence of MA in overweight groups is not well documented. This population study with an overrepresentation of individuals with BMI >25 (calculated as kg/m2) investigates the prevalence of MA in different BMI categories, and the relationship between MA and BMI.
METHODS: Data from two cross-sectional epidemiological studies in the City of Sheffield were combined to produce a cohort of non-diabetic, non-CKD subjects over th

AIMS: To describe the demographics of the paediatric RRT population in the UK and analyse changes in demographics with time.
METHODS: Extraction and analysis of data from the UK paediatric Renal Registry.
RESULTS: The UK paediatric established renal failure (ERF) population in April 2008 was 875 patients. The prevalence under the age of 16 years was 55 per million age related population (pmp) and the incidence 7.92 pmp. The incidence and prevalence for South Asian and Other ethnic groups were 3 times that of the White and Black populations. Renal dysplasia was the most common cause of ERF ac

Toll-like receptors (TLRs) play a central role in the response of both the innate and the adaptive immune system to microbial ligands. There is also evidence that they are stimulated by endogenous ligands. In this review, I discuss evidence that they are important in renal disease. This discussion considers the role of both endogenous and microbial ligands, and also the contribution of TLRs present on leucocytes and on intrinsic renal cells. There is strong evidence of a role for TLR2 and TLR4 in renal ischaemia-reperfusion injury, with the effects probably mediated by endogenous ligands. In s

BACKGROUND/AIMS: Although a low level of high-density lipoprotein cholesterol (HDL-C) is an independent risk factor for atherosclerotic heart disease, the mechanism of HDL-C abnormality in hemodialysis (HD) as well as peritoneal dialysis (PD) patients is not fully understood. The purpose of this study was to investigate the relationship of physical activity with HDL-C subfractions and lecithin:cholesterol acyltransferase activity in HD and PD patients.
METHODS: Thirty-five HD and 26 PD patients were studied. Physical activity was estimated as the average number of steps taken per day over 7 d

Hepatitis C virus (HCV) infection is the most common blood-borne viral infection in haemodialysis. It causes significant morbidity and long-term mortality. Practice of universal precautions has been reported to be sufficient to prevent HCV seroconversion in dialysis units. However, the seroconversion rate remains very high in many dialysis units. A previous study from 1995 to 1998 at our own hospital without isolation showed that nosocomial transmission is the major cause of HCV seroconversion. The present study was therefore conducted with the aim to study the impact of isolation on HCV seroc

BACKGROUND/AIMS: In end-stage renal disease (ESRD), hyperhomocysteinemia is a common finding associated with increased cardiovascular risk. However, the pathogenic role of homocysteine is still unclear. In vitro studies show that thiol redox status affects endothelial cell functions. We therefore investigated the possible association between homocysteinemia and plasma thiol redox status in ESRD patients.
METHODS: Total plasma homocysteine (Hcy), cysteine (Cys) and free thiols (SH) were measured both before and after a dialytic session in 54 ESRD patients receiving (n = 15) or not receiving (n

AIM: To examine whether treatment of secondary hyperparathyroidism with paricalcitol provides benefits to arteries in uremic rats.
METHODS: 5/6-nephrectomized rats were treated (NX+Pari) or not treated (NX) with paricalcitol (200 ng/kg, thrice weekly) for 12 weeks. Aortic histology and isolated segments of the main and 2nd-order mesenteric arterial branches were studied.
RESULTS: Creatinine clearance was reduced by 54-61%, plasma phosphate increased 2.1- to 2.5-fold, and blood pressure by 40 mm Hg in both NX groups. PTH increased 13-fold in NX and 5-fold in NX+Pari rats. Calcification in aor

BACKGROUND: Cardiovascular disease remains the most common cause of mortality in end-stage kidney failure patients with diabetes. To improve blood pressure control, and reduce cardiovascular risk, in 2002 the UK Renal Association Standards' Committee introduced pre- and post-dialysis target blood pressures of <140/90 and <130/80 mm Hg, respectively.
METHODS: We audited blood pressure control and symptomatic intradialytic hypotension during 1 week in the eleven renal centres in the Greater London Urban Area in 2,193 patients, capturing 6,579 haemodialysis sessions.
RESULTS: Although 73.9% of

BACKGROUND: Progressive proteinuria and glomerulosclerosis characterize chronic allograft nephropathy. However, the causes are not fully elucidated. Podocytes function to prevent proteinuria; injury to this glomerular cell leads to glomerulosclerosis. The potential role of podocytes in the failing transplanted kidney is unknown. A rat model of kidney transplantation, characterized by proteinuria and glomerulosclerosis, was utilized to examine the potential role of podocytes.
METHODS: Archival tissue was examined from allografts (Dark Agouti kidneys transplanted into operationally tolerant Alb

BACKGROUND: The antiproliferative immunosuppressant everolimus adversely affects the acute reversible anti-Thy1 nephritis model. We hypothesized that everolimus treatment started after the acute proliferative phase could even be beneficial in the chronic anti-Thy1 nephritis model in the rat.
METHODS: Chronic anti-Thy1 nephritis was induced by injection of the monoclonal antibody 1-22-3 in 20 male Sprague-Dawley rats 7 days after uninephrectomy. Two weeks after disease induction, rats were randomly treated with either everolimus or vehicle for 14 weeks. Changes in progression of renal disease

BACKGROUND/AIMS: We have previously shown that treatment of spontaneously hypertensive rats (SHR) with an angiotensin receptor blocker (ARB) during the 'critical period' from age 3 to 10 weeks confers protection against L-NAME-induced renal injury later in life. The aim of this study was to examine the effects of transient prepubertal exposure to ARB on the development of nephropathy in streptozotocin-induced diabetic SHR and to compare the results with other antihypertensive agents including a mineralocorticoid receptor antagonist (MR-ant).
METHODS: Male SHR (n = 43) were transiently treated

BACKGROUND: Early diagnosis of proximal tubular lesions can be achieved by detecting abnormal levels of low molecular weight proteins in urine.
MATERIAL AND METHODS: 100 cases of glomerulopathies were retrospectively studied to establish the profile of urinary levels of retinol-binding protein (urRBP) and their correlation with histological markers of tubulointerstitial lesions in renal biopsies. The histological study included staining with picrosirius red.
RESULTS: Nonproliferative glomerulopathies, male sex, white race and young adults were predominant. The chance of abnormal urRBP occurr

Autosomal-dominant polycystic kidney disease (ADPKD) is one of the most common monogenic diseases. It is characterized by a substantial variability in the severity of renal phenotype, primarily assessed by the age at end-stage renal disease (ESRD). The role of modifier genes has been shown in various hereditary diseases, including ADPKD. The gene coding for the endothelial nitric oxide synthase (NOS3) is considered to have a modifier effect on the severity of ADPKD, even if there are studies among different populations that have shown contradictory results. In this study, we investigated the i

BACKGROUND: Oxidative modification of low-density lipoprotein (LDL) has been demonstrated in patients with end-stage renal disease, where it is associated with oxidative stress and plays a key role in the pathogenesis of atherosclerosis. In this context, the generation of minimally oxidized LDL, also called electronegative LDL [LDL(-)], has been associated with active disease, and is a detectable sign of atherogenic tendencies. The purpose of this study was to evaluate serum LDL(-) levels and anti-LDL(-) IgG autoantibodies in end-stage renal disease patients on dialysis, comparing patients on

BACKGROUND/AIM: Chronic glomerulonephritides associated with psoriasis vulgaris have been reported in the literature. However, because of the limited number of cases and the lack of specific histological findings, the existence of psoriatic nephropathy remains controversial. This study was performed to investigate the clinical and morphological characteristics of renal involvement in psoriasis vulgaris.
METHODS: We studied retrospectively 11 patients with psoriasis vulgaris. All patients had nephritic urinary sediments and underwent percutaneous renal biopsy between January 1997 and December
