Vascular malformations of the brain.

Vascular malformations of the brain are a heterogeneous group of disorders involving the blood vessels of the brain. These entities are relatively rare; therefore, most primary care physicians are somewhat unfamiliar with their clinical implications and management. Vascular malformations of the brain range from the completely benign capillary telangiectasia to the potentially fatal arterio-venous malformation. This article reviews the distinct natural histories, etiologies and treatments of four major types of malformations: capillary telangiectasia, developmental venous anomaly, cavernous mal

Creutzfeldt-Jakob Disease in the hospital setting: a case report and review.

Rapidly progressive dementia is a neurological condition that results in subacute deterioration in cognitive, behavioral and motor function. The most serious diagnosis for a patient with rapidly progressive dementia is Creutzfeldt-Jakob Disease (CJD), a prion-related illness that typically results in death within one year. However, there are numerous autoimmune, infectious and toxic-metabolic causes of rapidly progressive dementia that are potentially reversible with treatment. Thus, the differential diagnosis for a rapidly progressive dementia is critically important. In this article, the aut

The elimination/challenge diet.

Elimination diets can be both a diagnostic tool and a therapeutic intervention for people with a suspected food sensitivity or allergy. They are clinically relevant not only for patients with functional gastrointestinal disorders but also for those with conditions where symptoms are refractory and a diagnosis is elusive. Elimination diets can help a physician make a diagnosis or identify an underlying cause of symptoms. The physician and team treating the patient can then use that information to recommend appropriate dietary and lifestyle changes as well as judicious drug therapy. This article

Meniere's disease: new concepts, new treatments.

Our understanding of Meniere's disease is being revamped. For decades, the condition was thought to be caused by excessive fluid retention (hydrops) in the endolymphatic spaces of the inner ear, which led to tears or ruptures of the membranous structures that affect hearing and balance. More recently, research has shown that hydrops is not always associated with Meniere's and ought not to be considered the ultimate cause of its symptoms. New theories center on the fact that Meniere's disease may not have a single cause but may well be a common endpoint of a variety of anatomic or physiologic v
