Bilateral cataracts in a three-year-old with deficiency of adenosine deaminase 2 (DADA2), hyperferritinemia, and prolonged steroid use.

BACKGROUND: Deficiency of adenosine deaminase 2 (DADA2) is a rare autosomal recessive autoinflammatory disorder associated with systemic vasculitis and bone marrow failure. Reported ophthalmic findings in DADA2 include optic neuritis, retinal artery occlusion, uveitis, and optic atrophy. We report the case of a child found to have bilateral cataracts. CASE REPORT: A three-year-old recent immigrant from Mexico with a diagnosis of DADA2 and transfusion-dependent anemia was referred to ophthalmology to screen for deferasirox-associated retinopathy in the setting of hemochromatosis. He was incide…

Evaluation of retinal and peripapillary vessel density and subfoveal choroidal thickness changes in patients undergoing cardiopulmonary bypass: An OCTA study.

PURPOSE: We aimed to investigate hemodynamic changes in the retina, peripapillary area, and choroid in patients undergoing coronary artery bypass grafting (CABG). METHODS: This prospective case series included 35 eyes of 35 patients. Vessel density (VD) of superficial capillary plexus, deep capillary plexus, radial peripapillary capillary, foveal avascular zone area, and subfoveal choroidal thickness changes were evaluated using optical coherence tomography angiography (OCTA) 1 day before and 2 weeks after surgery. The correlation of these parameters with aortic cross-clamp (ACC) duration and

Orbital emphysema as a result of chest tube placement for recurrent pneumothorax.

Orbital emphysema refers to the presence of air or gas within the orbital soft tissue space. Although orbital fracture due to trauma is the most common cause, iatrogenic orbital emphysema from chest tube insertion is a rare but potentially serious complication from the treatment of pneumothorax. Symptoms can range from mild crepitus of the periorbital region to permanent vision loss secondary to central retinal artery occlusion or optic neuropathy, a result of orbital compartment syndrome. Management is dependent on the presence of orbital signs and vision loss. Herein, we present a rare case …

Systemic Bartonellosis Manifesting With Endocarditis and Membranoproliferative Glomerulonephritis.

Cat scratch disease caused by Bartonella species is mostly benign and self-limiting condition. Systemic infection is uncommon in immunocompe … alve vegetations despite multiple negative blood cultures. The left eye blindness was attributed to retinal artery occlusion from septic valvular embolus. Kidney biopsy showed membranoproliferative glomerulonephritis pattern of injury with "full house" pattern on immunofluorescent staining with subendothelial deposits on electron microscopy. Markedly elevated Ig …

The Enigma of Orbital Compartment Syndrome After Lumbar Spine Surgery in the Prone Position: Case Report and Literature Review.

BACKGROUND: Perioperative visual loss after spinal surgery is a devastating complication for the patient and the surgical team. Two known major causes are ischemic optic neuropathy and central retinal artery occlusion (CRAO). Traditional understanding of CRAO has been consistently related to occurrence of periocular trauma and signs of increased intraorbital pressure in addition to visual loss. However, such orbital signs are not a feature of any common perioperative visual loss syndrome. CASE DESCRIPTION: A 55-year-old woman underwent prolonged lumbar decompression and fusion for spinal sten…

Optical Coherence Tomography in an Infant with Walker-Warburg Syndrome.

PURPOSE: Walker-Warburg syndrome (WWS) is a type of congenital muscular dystrophy (CMD) characterised by severe brain malformation, lissencephaly, and congenital eye abnormalities. Despite the coexistence of various eye abnormalities, results from optical coherence tomography (OCT) in WWS have not previously been reported. We herein report specific OCT findings in an infant with WWS. PATIENTS AND METHODS: The patient was a 14-day-old boy delivered by caesarean section at 38 weeks and 4 days of gestation and with a birth weight of 2,543 g. A cranial MRI showed lissencephaly, hydrocephalus, an

Recurrence of Susac Syndrome following 23 Years of Remission.

Susac syndrome is an autoimmune microangiopathy affecting the brain, retina and inner ear (cochlea and semicircular canals), leading to encephalopathy, branch retinal artery occlusions (BRAOs) and asymmetric neurosensory hearing loss, respectively. The natural history and long-term prognosis are variable as the disease has been shown to be monophasic and self-limiting, polycyclic or chronic continuous. We describe a 35-year-old woman who presented with a sudden hearing loss in the left ear in the 37th week of her second pregnancy. She subsequently developed BRAO in the right eye 2.5 months aft…

Coronary artery disease in Alström syndrome.

Alström syndrome (ALMS) is a rare autosomal recessive condition, caused by mutations in the ALMS1 gene located on the short arm of chromosome 2. This gene codes for a protein linked with the centrosome, whose precise function is unknown. This condition was first described by Alström in 1959. ALMS is a multisystem condition that is characterised by childhood onset of blindness secondary to rod-cone retinal degeneration and dilated cardiomyopathy with heart failure, which often presents in infanthood and may recur later in life. Metabolic abnormalities including hypertriglyceridemia, liver steat

Ocular motor nerve palsies: implications for diagnosis and mechanisms of repair.

Measurements of the dynamics of the eyes in ocular motor nerve palsies may aid diagnosis, characterize peripheral and central palsies, and reveal adaptive properties of the brain. Saccadic and vestibulo-ocular reflex (VOR) functions of patients with peripheral and central sixth, and peripheral third and fourth nerve palsies were studied by three dimensional magnetic field search coil oculography. Combined third and fourth cranial nerve microvascular ischaemic palsy in diabetes mellitus produced low ratios of intorsion to adduction amplitudes. Presumed isolated third nerve palsy caused higher r

The pupil.

PURPOSE OF REVIEW: To give an overview on topics of pupillary function selected according to their relevance in clinical neurology. Mainly publications of the last 3 years have been considered. RECENT FINDINGS: The discovery of photosensitive retinal ganglion cells not serving vision, but serving circadian rhythm and the pupil initiated many studies. There is evidence that pupil reaction to light might be preserved even if all rods and cones are lost. Spontaneous pupillary contractions in darkness occur in sleepy subjects and have been used to quantify the state of alertness. Apraclonidine ey

Role of sildenafil in neurological disorders.

Sildenafil, a phosphodiesterase-5 inhibitor commonly used for erectile dysfunction, may also have a beneficial therapeutic effect in the tre … sient global amnesia, nonarteritic anterior ischemic optic neuropathy, macular degeneration, branch retinal artery occlusion, and ocular muscle palsies. Thus, preclinical and very limited clinical data suggest that sildenafil may have therapeutic potential in selected neurological disorders. However, numerous reports are available regarding neurological adverse …

[The significance of concurrent vertebrobasilar insufficiency in postanesthesia recovery].

A study of 149 patients was carried out, who had cervical osteochondrosis, by using the stage-based narcosis and general intravenous anesthesia. The parameters of the peripheral velocity of the bloodstream in the direct brain venous sine were examined by transcranial dopplerography. Preoperatively, the patients were shared between 3 groups with due respect to clinical and neurological symptoms as well as to X-ray-registered changes in the cervical spine. While evaluating the intracranial pressure, the cerebral hemodynamics was examined by ophthalmodynamometry procedures (diastolic pressure in

Orbital venous-lymphatic malformations (lymphangiomas) mimicking cavernous hemangiomas.

PURPOSE: To illustrate that orbital venous-lymphatic malformations (lymphangiomas) may rarely simulate cavernous hemangiomas. METHODS: Retr … section was difficult in all patients because of dense adhesions and, in one case, led to a central retinal artery occlusion. The histology was characteristic of orbital venous-lymphatic malformations in all five cases. CONCLUSIONS: Deep orbital venous-lymphatic malformations presenting in adulthood may be rarely confused with cavernous hemangiomas. In doubtful …

Transient ischaemic attacks: evaluation and management.

Transient ischaemic attacks (TIAs) are temporary focal cerebral or retinal deficits that resolve within 24 hours. Attention should be given to the tempo and localisation of the clinical syndrome, as multiple and hemispheric TIAs are associated with the greatest incidence of early stroke. Evaluation of TIAs depends on the clinical symptoms, physical examination and investigations. Attention should be given to clinical evidence of generalised atherosclerotic disease, as death due to the complications of ischaemic heart disease is the commonest outcome in patients with TIA. Early attention should

[Evolution of symptomatic carotid stenoses under medical treatment].

Data from the 2 major studies (ECST and NASCET) carried out to evaluate surgery of symptomatic carotid artery stenosis provided more information on the evolution (during medical treatment) of these lesions than all previous work. The essential risk, that of a cerebral infarction homolateral to the stenosis, is closely related to the degree of stenosis; for occlusions greater than 70% it was 5% annually in the ECST and 13% in the NASCET, more severe occlusions being treated in the latter study. Inversely, it was only 0.5% annually for stenoses involving less than 30% (ECST). For occlusions of b

Ultrasound and Clinical Predictors of Recurrent Ischemia in Symptomatic Internal Carotid Artery Occlusion.

BACKGROUND AND PURPOSE: Occlusion of the internal carotid artery puts patients at risk of recurrent ischemic events because of hemodynamic compromise. Our goal was to characterize clinical and duplex parameters indicating patients at risk of recurrent ischemia. METHODS: We retrospectively identified patients with symptomatic internal carotid artery occlusion. Clinical characteristics and ultrasound parameters, including collateral networks, were analyzed. Predictors for recurrent ipsilateral ischemia were investigated by Cox regression analysis. RESULTS: Of 68 patients, at least 1 recurrent

Influence of oxygen free radicals on the tone of ciliary arteries: a model of vasospasms of ocular vasculature.

BACKGROUND: Altered regulation of vascular tone and particularly vasospasms are thought to be a risk factor for the progression of primary open angle glaucoma (POAG). Apoptosis of retinal ganglion cells and possibly vascular tone regulation in glaucoma might be caused by oxidative stress. The aim of the present study was to investigate the influence of oxygen free radicals on the tone of ciliary arteries. METHODS: The experiments were carried out with fresh ring preparations from porcine ciliary arteries obtained from a slaughterhouse. The preparations were placed in a self-designed myograph
