The Pathobiology of Behavioral Changes in Multiple System Atrophy: An Update.

While cognitive impairment, which was previously considered a red flag against the clinical diagnosis of multiple system atrophy (MSA), is a common symptom of this rare neurodegenerative disorder, behavioral disorders are reported in 30 to 70% of MSA patients. They include anxiety, apathy, impaired attention, compulsive and REM sleep behavior disorders (RBD), and these conditions, like depression, are early and pervasive features in MSA, which may contribute to disease progression. Despite changing concepts of behavioral changes in this synucleinopathy, the underlying pathophysiological and bi

Should genetic testing for SCAs be included in the diagnostic workup for MSA?

OBJECTIVE: To examine the prevalence of mutations in spinocerebellar ataxia (SCA) genes in patients who were clinically diagnosed with multiple system atrophy (MSA). METHODS: Genetic tests for SCA were performed in 302 of 528 patients who met the diagnostic criteria for MSA based on clinical features. Generally, when a patient had cerebellar symptoms or cerebellar atrophy on neuroimaging, genetic tests for SCA types 1, 2, 3, 6, 7, and 17, and dentatorubropallidoluysian atrophy were done, and when a patient had parkinsonism without cerebellar symptoms, genetic tests for SCA types 2, 3, and 17

Narcolepsy and disorders of excessive somnolence.

Recent studies provide valid criteria that help differentiate idiopathic narcolepsy from other disorders of excessive daytime somnolence [3]. Research to date suggests that idiopathic narcolepsy might properly be considered a disorder of excessive sleepiness with dysfunctional REM-sleep mechanisms, clinically evidenced as cataplexy and electrophysiologically recognized as SOREMPs. Given these criteria, a diagnosis can generally be made using a combination of history, PSG, and MSLT. Traditionally, the medical treatment of idiopathic narcolepsy has centered on a two-drug regimen (stimulants for

The right cerebral hemisphere: emotion, music, visual-spatial skills, body-image, dreams, and awareness.

Based on a review of numerous studies conducted on normal, neurosurgical and brain-injured individuals, the right cerebral hemisphere appears to be dominant in the perception and identification of environmental and nonverbal sounds; the analysis of geometric and visual space (e.g., depth perception, visual closure); somesthesis, stereognosis, the maintenance of the body image; the production of dreams during REM sleep; the perception of most aspects of musical stimuli; and the comprehension and expression of prosodic, melodic, visual, facial, and verbal emotion. When the right hemisphere is da

To what extent is sleep rebound effective in reversing the effects of paradoxical sleep deprivation on gene expression in the brain?

Sleep is essential to maintaining health and well-being. It has been demonstrated that some of the biological alterations caused by paradoxical sleep deprivation (PSD) are not completely reversed after a period of sleep rebound (SR). The purpose of this study was to determine to what extent the specific molecular changes that occur in the rat cerebral cortex after 96 h of PSD can effectively be reversed during 24h of recovery. Total RNA from the right cerebral cortex of Wistar male rats and GeneChip Rat Genome 230 2.0 arrays were used to perform comprehensive microarray analysis of gene expres

Effect of oral appliance therapy on neurobehavioral functioning in obstructive sleep apnea: a randomized controlled trial.

STUDY OBJECTIVES: This study aimed to assess the efficacy of a custom-made mandibular advancement splint for the treatment of obstructive sleep apnea with respect to neuropsychological functioning and mood state. METHODS: A randomized controlled crossover design was used in which 73 participants (mean age = 48.4, SD = 11.0, % men = 80.8) with at least 2 symptoms of obstructive sleep apnea and an apnea hypopnea index > or = 10 per hour underwent treatment with both mandibular advancement splint and an inactive oral device. Polysomnographic, neuropsychological and self-report measures were cond

Neurophysiological observations in diseased newborns.

The early statements that the EEG alone could correctly be used for the assessment of gestational age (cf., among others, Dreyfus-Brisac, 1964; Parmelee et al., 1968; and Nolte et al., 1969), even in pathological babies, need some restrictive qualifications. Likewise, the more recent statements that certain neurophysiological parameters, including nerve conduction velocity and basic EEG patterns are independent not only of gestational age and body weight, but also of various abnormal factors in the pre- and perinatal periods, appear also too rigid, at least as regards the EEG and the other par

Spatio-temporal gait signatures across prodromal and early Parkinson's disease in two independent cohorts.

Gait abnormalities are key features of Parkinson's disease (PD) and might be present in isolated REM Sleep Behavior Disorder (iRBD), a prodromal marker of alpha-synucleinopathy. To assess spatio-temporal gait parameters from prodromal (iRBD) to early/moderate PD stages in normal, fast and dual-task conditions. This prospective multicenter study included two independent cohorts with Polysomnography-confirmed iRBD, drug-naïve PD (naïve-PD), early and moderate treated PD (early-PD, mid-PD), and age-matched healthy controls (HC). Gait was assessed using mobile health technology. A total of 324 par

Non-motor symptom subtypes in early Parkinson's disease.

BACKGROUND: Parkinson's disease (PD) is a clinically heterogeneous disorder characterized by a broad spectrum of non-motor symptoms. Defining non-motor subtypes may help clarify early pathophysiological mechanisms, predict disease progression, and guide personalized care. OBJECTIVE: To identify non-motor PD subtypes through an unsupervised approach, evaluate their consistency with proposed models of PD pathology, and assess their prognostic value. METHODS: Data from 824 newly diagnosed, drug-naïve PD patients enrolled in the Parkinson's Progression Markers Initiative were analyzed. Five core

Progression of clinical diagnostic features and cognitive decline in mild cognitive impairment with Lewy bodies.

BACKGROUND: Mild cognitive impairment with Lewy bodies (MCI-LB) may be identified prospectively based on the presence of cognitive impairment and several core clinical features (visual hallucinations, cognitive fluctuations, parkinsonism, and REM sleep behavior disorder). MCI-LB may vary in its presenting features, which may reflect differences in underlying pathological pattern, severity, or comorbidity. We aimed to assess how clinical features of MCI-LB accumulate over time, and whether this is associated with the rate of cognitive decline. METHODS: In this cohort study, 74 individuals seen

Symptom diagnosis codes informing under-diagnosis of Lewy body dementia in Medicare beneficiaries from different ethnoracial backgrounds.

BackgroundResearch suggests that Lewy body dementia (LBD) is underdiagnosed, but reasons for this are under-studied.ObjectiveTo investigate the frequency of diagnosis codes aligned with LBD symptoms in Medicare beneficiaries.MethodsThe frequency of ICD-10 codes associated with dementia and core and supportive LBD features was assessed in a 15% random sample of Medicare fee-for-service beneficiaries (2017-2020). The frequency of diagnosis codes was assessed in the full cohort and a cohort with an LBD diagnosis code, overall and stratified by ethnoracial group. Differences between groups were as

A Population-Based Approach to the Argument on Brain-First and Body-First Pathogenesis of Lewy Body Disease.

OBJECTIVE: To explore the clinical progression of the brain-/body-first categories within Lewy body disease (LBD): Parkinson's disease (PD), dementia with Lewy bodies (DLB), and PD dementia. METHODS: We used of the Rochester Epidemiology Project to establish a population-based cohort of clinically diagnosed LBD. We used two definitions for differentiating between brain- and body-first LBD: a previously hypothesized body-first presentation in patients with rapid eye movement sleep behavior onset before motor symptoms onset; and an expanded definition of body-first LBD when a patient had at lea

Movement Disorders: A Brief Guide in Medication Management.

Movement disorders can be challenging to manage and often use a specific set of medications. Because it is a complex and broad field within neurology, many providers are unfamiliar with the classes of medications. This paper details medications used for specific conditions, explains why these medications are helpful, and shares pearls and pitfalls related to each agent, focusing on parameters such as dose titration, side effect profiles, and specific drug-drug interactions and challenges. We focus on the most commonly encountered movement disorders, including essential tremor, Parkinson's dise

Neural substrates of rapid eye movement sleep behavior disorder in Parkinson's disease.

OBJECTIVES: To investigate neural substrates of symptomatic rapid eye movement sleep behavior disorder (RBD) in Parkinson's disease (PD) by analyzing brain changes based on both hypothesis-free and hypothesis-driven neuroimaging analyses. METHODS: A total of 63 subjects (14 PDRBD-, 24 PDRBD+, and 25 age-matched healthy controls = HC) were enrolled in this study. RBD was defined by RBD screening questionnaire with video-polysomnographic confirmation. All subjects underwent volumetric and diffusion tensor imaging. The whole brain gray- and white-matter changes were analyzed and the central asce

Restless legs syndrome and periodic limb movements after ischemic stroke in the right lenticulostriate region.

We report the first instance of restless legs syndrome (RLS) associated with periodic limb movements (PLM) and disruption of sleep architecture occurring in a patient following ischemic infarction in the right lenticulostriate region. Recently, a role for the basal ganglia-brainstem system in the control of motor behaviors and in the regulation of awake-sleep states has been proposed. The purported roles of these structures may be relevant in explaining the occurrence of the RLS in our patient. The discrete brain localization observed in this patient may be a clue to a better understanding of

The link between inflammation and autonomic dysfunction in alpha-synucleinopathies - narrative review.

INTRODUCTION: Alpha-synucleinopathies are a group of disorders characterized by the accumulation of the pathological form of alpha-synuclein. This group comprises Parkinson's disease, Parkinson's disease dementia, multiple system atrophy, dementia with Lewy bodies, pure autonomic failure, and idiopathic REM sleep behavior disorder. Despite sharing a similar molecular basis, these disorders differ in their clinical presentation. Characteristic manifestations include signs of autonomic failure such as orthostatic hypotension, postural orthostatic tachycardia syndrome, constipation, gastroparesis
