Psoriatic arthritis is a psoriasis-related spondyloarthropathy that occurs in 20-30% of patients with psoriasis. Various imaging studies have demonstrated that there is a considerable proportion of undiagnosed psoriatic arthritis among patients with psoriasis. Since early detection and treatment of psoriatic arthritis could, ultimately, allow the prevention of clinical and radiologic progression of the disease, there is the need to establish clinical indicators to detect this risk. Nail psoriasis has been proposed as a predictor for the development of psoriatic arthritis. The inflammation invo

OBJECTIVE: The interleukin (IL)-10 single nucleotide promoter polymorphism (SNP) -2849A is associated with decreased IL-10 production as measured by lipopolysaccharide (LPS) stimulated whole blood cultures. A low innate production of IL-10 using the same assay is associated with an increased risk of familial osteoarthritis (OA). We investigated the association of 7 novel SNP located downstream of the IL-10 transcription start site: -2849,-2763, -1330, -1082, -819, and -592, constituting the 4 ancient haplotypes, with distal interphalangeal (DIP) OA.
METHODS: The study population comprised con

Trigger thumb is an uncommon problem in infants and children. The authors reexamined 21 patients (30 thumbs) who underwent a release procedure, with an average follow-up of 181.3 months (15.1 years). Twenty-three percent of patients had a loss of interphalangeal motion and 17.6% had metacarpal phalangeal hyperextension, and this was unrelated to age at the time of surgery. There was no recurrence of triggering or nodules and no functional deficit. All seven patients who had a longitudinal incision had concerns about their scar appearance. It is the authors' belief that a transverse skin incisi

Kirschner wire (K-wire) fixation of fractures and dislocations of the hand and wrist is a common procedure. Of the 590 K-wire fixations performed on 236 patients, 36 (15.2%) experienced complications which included osteomyelitis, tendon rupture, nerve lesion, pin tract infection, pin loosening or migration. There were no deep soft-tissue pin infections or pyarthrosis. Technical failure, mainly when the procedure was performed by residents, and poor patient compliance were the major causes of complications. K-wire fixation is a simple but demanding procedure that cannot be left to an inexperien

Eleven patients had correction of extra-articular malunions of the proximal phalanx by in situ osteotomy stabilized with a dorsal plate, which healed within 7 weeks. The patients' average age was 27 years; 8 of the patients were male. After an average follow-up period of 35 months (range, 25-54 months), all the patients had correction of the overlapping or scissoring of the digits during flexion. The rotation deformity improved from 17 degrees to 2 degrees and the angular deformity present in 6 patients improved from 14 degrees to 0 degrees. None of the patients lost motion. On average, the pa

This study reports the results of a study on the intertester reliability of manual muscle strength testing in leprosy patients with confirmed motor function loss of at least one nerve. Three testers graded the muscle strength of 72 patients in random order. Both hands and feet were graded. Strength was graded on a modified Medical Research Council Scale (9 points, 5, 4+, 4, 3+, 3, 2+, 2, 1, 0). The following movements were tested for strength: little finger and index finger abduction, intrinsic position of all four fingers, thumb abduction and opposition, foot dorsiflexion and eversion and ext

Phalangeal cone-shaped epiphyses are an ideal object for the radiologist to study with temporal reasoning, to examine their shape, diagnostic usefulness, natural history and effect on pathophysiology. Radiographs of the hands of 60 patients with cartilage hair hypoplasia (CHH), 69 with trichorhinophalangeal syndrome I (TRP I) and 11 with TRP II were examined, including 26 longitudinal observations. The early phases of cone development were recorded. In CHH a characteristic mesophalangeal type of cone in 42/47 patients of the "Age Suitable for Classification" (ASC) was detected. All 46 TRP I an

Well known complications of proximal interphalangeal joint fractures and fracture dislocations are stiffness, chronic instability, and degenerative arthritis. The Compass PIP Hinge is a dynamic external fixator that allows protected proximal interphalangeal mobilization after closed reduction, open reduction and internal fixation, volar plate arthroplasty, or other salvage procedures. To help avoid these problems, 20 patients, 12 treated within 2 weeks of injury (Group I) and 8 treated more than 4 weeks after injury (Group II), are reported. Articular surface involvement among Group I cases av

Background Minimally invasive sacroiliac joint fusion (MI SIJF) is an established intervention for chronic sacroiliac joint dysfunction, yet outcome variability across the literature suggests that surgical success may heavily depend on preoperative selection accuracy. Traditional diagnostic pathways relying on transient anesthetic blocks are susceptible to false-positives and variable responder rates. The purpose of this study is to evaluate the clinical effectiveness of a rigorous, multi-stage selection protocol, termed the "diagnostic funnel," which utilizes a therapeutic durability threshol

BACKGROUND: Extra-articular but severely comminuted distal basal fractures of the proximal phalanx (PP) are rarely reported. Therefore, the aim of this study was to achieve proper union and desirable outcomes using low-profile locking plates/screws. We introduced our own surgical approach and reported the clinical/radiographic outcomes via retrospective case series.
METHODS: Thirty-eight patients diagnosed with comminuted basal fracture of the PP were admitted between January 2014 and December 2020. Nineteen of these patients met the inclusion/exclusion criteria and were investigated in this

A 39-year-old man reported with progressive thickening of the skin of the hands and feet and an inability to flex his hand. It was largely asymptomatic; however, brisk walking caused excessive sweating, pain, and widening of the fissures on the soles of the feet. He was unable to walk barefooted. According to his mother, the first episode presented with blistering at 7 days of age. Ever since, the condition has steadily worsened to acquire the current status. He was married at the age of 18 years, and had a stillborn child 18 months afterwards. Presently, he has three children, two girls aged

The surgical strategy of treatment of the opposable triphalangeal thumb is correction of the radio-ulnar deviation, reduction of the additional length and joint stabilization. The commonest procedures are: (1) removal of the extra phalanx and stabilization of the remaining joint; and (2) a combined reduction osteotomy with resection of the distal joint followed by arthrodesis. We treated 20 patients (33 hands). In 17 hands the extra phalanx was removed, and in 16 hands we used the combined osteotomy procedure of distal joint removal and arthrodesis. None of the patients in either group had an

INTRODUCTION: Popliteal pterygium syndrome is a congenital malformation that includes orofacial, musculoskeletal and genitourinary anomalies. It is a rare autosomal dominant disorder due to a mutation of the IRF6 gene on 1q32.2.
CASE PRESENTATION: A one-month-old Moroccan baby boy was diagnosed with typical features of popliteal pterygium syndrome and carried the c.250C>T; p.Arg84Cys mutation of the IRF6 gene.
CONCLUSIONS: We report on the first description of a Moroccan popliteal pterygium syndrome patient. This diagnosis allowed us to provide an appropriate course of management to the pati

PURPOSE: To report the medium- to long-term outcomes of joint distraction using a unilateral external fixator in the treatment of chronic post-traumatic proximal interphalangaeal (PIP) joint contractures.
METHOD: Between September 2001 and October 2011, 94 consecutive patients (98 PIP joints) with a mean age of 43 years (range, 17-69 y) were treated with external fixation for chronic flexion deformity of the PIP joint from trauma. The average time from injury to surgery was 48 months (range, 6-84 mo), and the duration of joint distraction was 10 days (range, 7-22 d). Patients were followed fo

PURPOSE: Giant cell tumor of the tendon sheath (GCTTS) has a high recurrence after excision and can be a management challenge. Although experience with radiation therapy for GCTTS is limited, it is purported to control infiltrative cases and prevent recurrence. We describe our approach to primary and recurrent GCTTS, as well as our identification of infiltrative cases and their treatment with radiation therapy.
METHODS: We reviewed 58 patients (32 men and 26 women) with GCTTS in the hand excised at 1 center between 1998 and 2009. Mean age at the time of excision at our center was 50 years. A
