Tibial diaphyseal malunion: A problem-oriented approach to assessment and correction.

Tibial shaft fractures are the most common long bone injuries, and malunion remains a significant complication despite advances in fixation. Tibial diaphyseal malunion is typically multiplanar, involving coronal, sagittal, rotational, and translational deformities with limb shortening, leading to altered biomechanics, abnormal joint loading, and potential degenerative changes. Biomechanical studies suggest that coronal deformity exceeding 5°, rotational deformity ≥10° and shortening ≥10 mm also increase the risk of joint degeneration, although long-term clinical correlations remain variable. Nail-Plate Combination Fixation for Periprosthetic Distal Femoral Fractures Allows Early Weight-Bearing With Low Reoperation and Revision Rates.

BACKGROUND: A combination retrograde femoral intramedullary nail (IMN) and locking plate for periprosthetic distal femoral fractures (PDFFs) may offer improved construct strength and early weight-bearing versus locked plating constructs or IMN fixation. This study compared clinical and radiographic outcomes in PDFF treatment with nail-plate combination (NPC), locked plating, or IMN. METHODS: Patients aged ≥ 18 years undergoing PDFF surgery between January 1, 2015, and April 1, 2024, at three institutions were retrospectively identified. Patients were included with NPC, locked plate only (i.e.

Ultrasonographic bridging callus as an early predictor of tibial fracture healing.

PURPOSE: This study assessed whether ultrasonographic (U/S) detection of bridging callus can serve as an early sign of tibial fracture healing. By comparing U/S with serial radiographs, the goal was to identify the best predictors of fracture union. Previous studies indicate that early radiographic bridging callus may signal eventual healing, and U/S could detect this sooner than X-rays. Thus, U/S may provide earlier predictions of bone union or impaired healing. METHODS: A prospective evaluation was conducted on a consecutive cohort of patients with tibial fractures managed by intramedullary

The Masquelet technique of induced membrane for healing of bone defects. A review of 8 cases.

BACKGROUND: Segmental defects of long bones are notoriously difficult to treat. This study evaluates eight cases in which the Masquelet technique of induced membranes was used. The primary purpose was to assess the results compared to other types of bone reconstruction and share our tips and tricks to improve the outcome. METHOD: Retrospective study based on patient records and radiographs. Eight patients operated between 2011 and 2014 were included. Three had infected non-unions. Outcome measures were time-to full weight-bearing, time to radiographic consolidation, need for secondary bone gr

Outcome of traumatic subtrochanteric femoral fractures fixed using cephalo-medullary nails.

We performed a retrospective review of 101 patients sustaining 102 traumatic, non-pathological subtrochanteric fractures treated with cephalo-medullary nailing over a period of 6 years from 1999 to 2005. Mean follow up was 24 weeks (range: 6-96). Twenty patients were lost to follow up and 21 died, whilst 57 of 60 fractures were followed to union. There were 19 fractures fixed in varus, as defined as a varus angulation of greater than 10 degrees at the fracture site. Implant failure, 9 of the 10 malunions and all 3 of the non-unions occurred in the varus group whilst only 1 malunion occurred in

[Surgical management of bone metastases of the lower extremity with AO interlocking nail].

Between January 1991 and June 1995 we have operated on 19 patients (9 male, 10 female) with 22 skeletal metastases of the lower limb (19 femora, 3 tibiae) using a static interlocking nail. Closed intramedullary nailing without resection of the metastasis has been established as our standard procedure. We have stabilized 15 patients with advanced osteolysis and seven pathological fractures. Sixteen patients underwent postoperative local radiation therapy with 40 Gy. As intraoperative complications we have observed one fracture of an osteolysis and one death due to fat embolism. Postoperatively

Elastic nailing versus plate and screws fixation for pediatric femoral shaft fractures: a systematic review and meta-analysis of high quality randomized controlled trials.

INTRODUCTION: Femoral shaft fractures are the most common pediatric femoral injury, accounting for 64%-68% of cases in children under 18 years. While younger children are usually treated non-surgically, older children often require fixation. Elastic stable intramedullary nailing (ESIN) and plating are the two main surgical options, but the optimal method remains debated. METHODS: This systematic review and meta-analysis followed PRISMA guidelines and was registered in PROSPERO. Medline/PubMed, ScienceDirect, Scopus, and Embase were searched for randomized controlled trials (RCTs) comparing ES

Functional outcomes and adverse effects following three interventions for displaced midshaft clavicular fractures: A Bayesian network meta-analysis of randomized controlled trials.

INTRODUCTION: The efficacy of the most commonly used interventions for clavicle fractures remains controversial. These interventions are: open reduction and plate fixation (ORPF), non-surgical intervention (NSI), and use of an intramedullary nail (IMN). In adult patients with clavicle fractures, choosing which intervention might be best is challenging. MATERIALS AND METHODS: PubMed, Journals@Ovid Full Text, Cochrane Central Register of Controlled Trials (CENTRAL), Ovid MEDLINE, and Embase were performed to search English-language studies from the inception to February 2020. Use of the Reamer-Irrigator-Aspirator for cancellous bone graft harvesting directly from non-union sites: A novel approach in femoral and tibial shaft reconstruction.

AIMS: The aim of this study was to assess the safety and the union rate when cancellous bone graft was harvested directly from non-union site using the Reamer-Irrigator-Aspirator (RIA) in femoral and tibial non-unions with intramedullary nail (IMN) fixation. Our hypothesis was that harvesting from the same site would yield similar results to the conventional RIA technique in terms of union and complication rates, with a single reaming thus restricting the morbidity to a single site. MATERIALS AND METHODS: Data was retrospectively analyzed from a single tertiary care center. We included 116 re

Polymer-Based Resorbable vs Non-resorbable Implants for Pediatric Upper Extremity Fractures: A Systematic Review.

BACKGROUND: Pediatric upper extremity fractures are common and frequently require surgical intervention. Traditional fixation methods using metallic implants, such as Kirschner wires and elastic stable intramedullary nails, offer reliable mechanical stability but necessitate secondary procedures for hardware removal. Polymer-based resorbable implants offer an advantage by eliminating the need for secondary procedures for hardware removal while maintaining efficacy. This systematic review evaluates the safety, efficacy, and clinical outcomes of resorbable compared to non-resorbable implants in

Humeral shaft fractures: a cost-effectiveness analysis of operative versus nonoperative management.

BACKGROUND: Humeral shaft fractures can be managed operatively or nonoperatively with functional bracing in the absence of neurovascular injury, open fracture, or polytrauma. A consensus on optimal management has not been reached, nor has the cost-effectiveness perspective been investigated. METHODS: A decision tree was constructed describing the management of humeral shaft fractures with open reduction-internal fixation (ORIF), intramedullary nailing (IMN), and functional bracing in a non-elderly population. Probabilities were defined using weighted averages determined from systematic review

Outcomes of Primary Tibiotalocalcaneal Nailing for Complicated Diabetic Ankle Fractures.

BACKGROUND: Ankle fractures in patients with complicated diabetes have significantly increased the rates of complications and poorer functional outcomes when treated nonoperatively, and there have been only modest reductions when treated operatively. We hypothesized that the minimally invasive, robust construct that tibiotalocalcaneal fixation with an intramedullary nail offers would result in high rates of limb salvage, acceptable rates of complications, and less loss of function, in this difficult patient population. METHODS: This was an institutional review board-approved retrospective stu

Effects of changing strategies of fracture fixation on immunologic changes and systemic complications after multiple trauma: damage control orthopedic surgery.

In the treatment of polytrauma patients, multiple studies have shown how the timing and the type of the initial surgery of long bone fractures influence the incidence of systemic complications. Database analyses documented that unduly long surgical procedures undertaken early after trauma increase the risk of acute respiratory distress syndrome (ARDS), especially when femoral shaft fractures are stabilized. Animal research and prospective clinical studies also support the fact that the type of stabilization of a femoral shaft fracture may influence the systemic response to trauma. Reamed intra

Extraction of total knee arthroplasty intramedullary stem extensions.

Intramedullary stem extensions will need to be extracted during total knee arthroplasty (TKA) revisions, especially repeated ones. These stems have various designs and lengths, can be straight or offset, cemented (partially or totally) or cementless, smooth or rough. This diversity adds to the difficult of extracting them, which the surgeon must anticipate before starting the revision procedure. Porous metaphyseal metal components (cones, sleeves) are being used increasingly during revision TKA. They pose specific extraction challenges and complicate the extraction of the stems with which they

A Large Extraskeletal Chondroma: An Unusual Location in the Lower Extremity, Huge Extraskeletal Chondroma: An Unusual Localization in the Leg.

INTRODUCTION: Osteomyelitis with multiresistant bacteria in non-union following fracture treated with osteosynthesis requires complete removal of infected sequestrum and dead bone. For consecutive bone defects, it is frequently necessary to bridge with a fixator external. The treatment is not only challenging due to reduced bone stock but also characterized by decreased bioavailability of antibiotics. CASE REPORT: We report a two-step-surgery approach to preserve the bone stock using autologous cancellous bone in a bacterial infected non-union for subsequently leg length reconstruction. The 2
