Surgery for Crohn's disease in infants and children.

The course of Crohn's disease is quite variable in children. To assess the frequency and indications for surgery with current medical therapy, the authors reviewed the cases of 204 children (ages, 0.2 to 18.8 years at diagnosis, median, 12.8 years) who had Crohn's disease treated at a single institution from December 1968 to January 1994, with a median of 3.8 years of follow-up (range, 0.0 to 22.2 years). Ninety-four children (46%) required surgical resection for the following indications: (1) failure of medical therapy with persistent symptoms or growth retardation (n = 44, 47%), (2) intraabd

[Fistula following emergency total colectomy: an efficient method of treatment].

A personal case of enterocutaneous fistula secondary to total emergency colectomy is reported with particular emphasis on an effective form of radiological management, based on the percutaneous sclerosing of the fistulous tract using absolute ethanol and a hyperosmotic contrast medium (Angioconray 80%). By this approach a rapid closure of the fistulous tract was obtained. Moreover the procedure was easy to perform, well tolerated by the patient and devoid of untoward side effects.

Entero-pancreatic reflexes revealed by duodenal anesthesia in the dog.

This study was designed to improve our understanding of duodeno-pancreatic reflexes, the existence of which was suggested by the previous observation of a reduction in secretin-stimulated pancreatic secretion during local anesthesia of the duodenal mucosa. The effects on this reduction in secretin-stimulated secretion of cholinergic or adrenergic blocking agents (alone or in combination) and of truncal vagotomy, were studied in conscious dog with gastric and pancreatic fistulae. For each agent and for secretin alone in normal and vagotomized dogs, a comparison was made of pancreatic secretion

Computed tomography evaluation of local and extraintestinal complications of Crohn's disease.

Complications of Crohn's disease were demonstrated by computed tomography of the abdomen and pelvis in 7 patients. Four patients had developed abscesses, 2 located in the psoas region and 2 involving the retroperitoneum or the liver. Other abnormalities documented on CT included inflammatory masses, fistulae, carcinoma, mesenteric thickening, and extraintestinal complications such as gallstones, renal calculi, and sacroileitis. Computed tomography was found to be useful in the detection and follow-up of abdominal complications of Crohn's disease.

The influence of surgical transection and anastomosis on the rate of cell proliferation in the colonic epithelium of normal and DMH-treated rats.

Normal and DMH-treated male rats aged 18-20 weeks underwent surgical transection and anastomosis of the transverse colon. Animals were subsequently killed at intervals of 14, 30 and 72 days. Three hours prior to sacrifice animals were injected with vinblastine sulphate and mitotic indices were subsequently estimated in histological sections. Possible differences between experimental and control groups were tested using a Student's t-test. The results show that the accumulated mitotic indices in normal and DMH-treated colon are statistically similar. The results also show that transection and a

[X-ray examination for the detection of complications after abdominal surgery (author's transl)].

The incidence of complications in a consecutive series of 515 abdominal surgical operations was studied. Abdominal complications of varying degree of severity were observed in 62 cases (16%). They were of four types: haemorrhage, ileus, peritonitis and complications on the part of the biliary tract and pancreas. The use of radiological methods for detecting these complications is reviewed. Direct roentgenography is the method of choice in cases of suspected ileus, perforation or foreign body. Administration of contrast media should be reserved almost entirely to cases of stress ulcer and to di

The interpretation and management of duodenal lesions other than benign ulcer.

Five patients, each with a unique disease, illustrate some of the unusual subtleties of interpretation, the challenges of operative approach, the avoidance of pitfalls and the management of complications in local duodenal surgical procedures. A Brunner's gland polyp was resected reversing prior pancreatitis and cardiac alterations. A rare ampullary gangliocytic paraganglioma, hemorrhaging, was managed by duct isolations, resection and papilloplasty. A perforated malignant bleeding ulcer with eventual common bile duct obstruction and mimicking a benign ulcer, was approached with a variety of ef

Using ultrasonographic features in pediatric Crohn's disease activity index severity.

BACKGROUND: The diagnosis and follow-up of Crohn's disease (CD) often require invasive instrumental examinations, which carry a high risk of iatrogenic injury. This study aimed to determine the frequency of ultrasound features in each stage of the Pediatric Crohn's Disease Activity Index (PCDAI). METHODS: This cross-sectional study included 22 pediatric patients with Crohn's disease. Disease activity was assessed using the PCDAI. The state of CD activity was categorized into four groups: remission (PCDAI scores less than 10), mild (PCDAI scores of 10-27.5), moderate (PCDAI scores of 30-37.5),

Impairment of the Peritoneal Surface as a Decisive Factor for Intestinal Adhesions in Intraperitoneal Onlay Mesh Surgery - Introducing a New Rat Model.

BACKGROUND: Meshes implanted intraperitoneally are known to cause adhesions potentially resulting in complications such as chronic pain, enterocutaneous fistula, or mesh infection. This study introduces a model for investigation of intestine-to-mesh adhesions and evaluates as to whether missing of visceral peritoneum is causative. METHODS: In 18 rats, rectangular 1.5 x 2 cm patches of an uncoated polypropylene mesh (Ultrapro(®)) were sewn to the inner abdominal wall next to the cecum. Additionally, a meso-suture ensured contact between cecum and mesh. Rats were assigned to 2 groups: in 8 rats

Risk factors for postoperative abdominal infection of colorectal cancer: A systematic review based on the PRISMA guidelines.

BACKGROUND: Intra-abdominal infections are a common complication of colorectal cancer surgery. Postoperative abdominal infections can cause systemic inflammatory response syndrome, which seriously affects the prognosis of patients. With the widespread application of antibiotics, the detection rate of drug-resistant bacteria has increased annually, resulting in increased pressure on antibiotic treatment selection. To improve the prognosis of postoperative patients with colorectal cancer, it is important to actively search for risk factors leading to postoperative abdominal infection and formula

Preoperative Hypoalbuminemia as a Predictor of Severe Postoperative Complications in Patients Undergoing Whipple Pancreatoduodenectomy.

BACKGROUND: Pancreaticoduodenectomy (PD) known as Whipple procedure is still one of the most complex abdominal surgeries used for treatment of periampullary tumors. PD is often followed with postoperative complications (pancreatic, biliar or intestinal fistula, haemorrhage, intraabdominal abscess, delayed gastric empting. Severe postoperative complications (SPC) can be reason for reoperation and reason of bad outcome of treatment and life treathening condition. OBJECTIVE: To investigate predicitive value of preoperative hypoalbuminemia for severe postoperative complications (SPC) in patients …

Surgical complications in relation to functional outcomes after ileoanal anastomosis in pediatric patients with ulcerative colitis.

BACKGROUND: Significant surgical complications are common in patients with ulcerative colitis who undergo proctocolectomy. We assessed the effects of these complications on the functional outcomes of such patients. MATERIALS AND METHODS: The medical records of 47 consecutive patients who underwent ileoanal anastomosis (IAA; J-pouch IAA, n = 37; straight IAA [SIAA], n = 10) for ulcerative colitis between 1985 and 2004 at a median age of 13.7 years (range, 5.5-19.3 years) were reviewed. Anastomotic leakage and stenosis, fistula, intestinal obstruction (IO), wound complications, pouchitis/entero
