Common skin cancers.

Skin cancers are the most common types of all cancers, and the most commonly occurring skin cancers are basal cell carcinoma, squamous cell carcinoma, and malignant melanoma, in that order. This article discusses the clinical presentations and subtypes of these 3 types of cancer and the current state of treatments for them. Also discussed are precursor lesions to these malignancies, including actinic keratosis, a squamous cell carcinoma precursor, and atypical or dysplastic nevus, a potential precursor of malignant melanoma.

Occlusion of subclavian-innominate veins: the increasing problem of receiving improper care.

Acute upper vein obstruction necessitates emergency treatment and always requires surgery. However, it is not treated appropriately most of the time. Leaving this condition untreated, or treating it incorrectly, can result in permanent disability of the patient, as well as malpractice and negligence lawsuits against the treating physicians. Thus, all physicians must be familiar with the proper care of acute upper vein obstruction. We provide in this article an easy-to-follow algorithm and an outline of steps for successful treatment. We also discuss two situations that should not be confused:

Transgender health.

Transgender persons represent an underserved community in need of sensitive, comprehensive health care. This article presents a literature-based review of the health needs of the transgender patient. Physicians, whether or not they choose to provide hormone therapy, will likely encounter patients with gender identity issues at some point in their practice. A transgender health assessment should involve recognition of possible gender identity disorder, history-taking with respect to prior and current use of hormones or surgical interventions, as well as general physical, mental, and sexual heal

Pharmacological treatment of obesity. Past, present, and future.

Physicians have struggled with the pharmacological treatment of obesity. In the past, thyroid hormone was often used inappropriately. Dangerous drugs such as dinitrophenol and amphetamines were prescribed, with serious side effects. In the 1980s, a 3 1/2-year study using antiobesity medications with different mechanisms of action supported the theory that patients treated with combination therapy experienced greater weight loss than the placebo-treated patients and that those who remained on therapy were more likely to keep the weight off. Thus, physicians began prescribing "fen-phen" to their

Tetanus: a case report.

Tetanus is a rare disease in the United States, with only 40 to 60 cases reported annually. This potentially lethal disease affects unimmunized or partially immunized persons, with the elderly at greatest risk. Tetanus usually follows an injury such as a puncture wound, abrasion, or laceration. The pathogen is an anaerobic, spore-forming, gram-negative, ubiquitous bacillus known as Clostridium tetani. In the vegetative form, it produces a lethal toxin, tetanospasmin, which enters the central nervous system through retrograde axonal transport, resulting in characteristic muscle spasms and tonic

Celiac sprue in an asymptomatic elderly man.

The authors discuss the case of an asymptomatic patient with celiac sprue presenting with hematologic abnormalities on screening laboratory examination. The patient was initially treated for vitamin B-12 and folate deficiency, and subsequently for iron deficiency with limited success until the diagnosis was established. The absence of such classic features of celiac disease as steatorrhea and diarrhea is not an uncommon presentation. Unexplained iron deficiency should alert the clinician to the possibility of celiac sprue.

Evaluation of fetal cardiac arrhythmias. Ultrasound findings and neonatal outcome.

During a four-year period, 3,882 fetal diagnostic ultrasounds were performed and 162 patients (4% of all patients scanned) were referred to our perinatal center for evaluation of fetal cardiac arrhythmia. Fetal echocardiography subsequently revealed an arrhythmia in 80 (49%) of these patients. The rhythm disturbances noted were premature atrial or ventricular contractions (n = 65, 81%), tachyarrhythmia (n = 8, 10%), and bradyarrhythmia (n = 7, 9%). Three of the bradycardic fetuses evaluated had complete heart block associated with anatomic abnormalities. In seven tachycardic fetuses, the findi

Diagnostic and therapeutic advances in pediatric cardiology (2).

The practice of pediatric cardiology has undergone remarkable changes over the past decade. Cardiac structural abnormalities may frequently be diagnosed with echocardiography alone, and treatment for many of these structural problems may be corrected during cardiac catheterization. Arrhythmia diagnosis and management have made similar progress, such that diagnosis of arrhythmia mechanisms and catheter ablative therapy may be performed during a single procedure, sparing the young patient a lifetime of antiarrhythmic drug therapy. Cardiac transplantation is now included among the treatment optio

Heat illness.

A 17-year-old girl collapsed during a field hockey practice on a hot August afternoon, apparently suffering from heat illness, which is responsible for about 5,000 deaths annually. A panel of experts explores this girl's case, discusses heat illness in general, and makes recommendations about diagnosis, prevention, and treatment.
