Surgical treatment of a case of congenital Dupuytren's disease.

Dupuytren's disease in infants has been rarely reported. We present a congenital case with the onset of the disorder since his birth. A 31-month-old boy with congenital flexion contracture of his left long and ring finger was suspected to the diagnosis. To correct the deformity and confirm the diagnosis, fascieotomy was performed. Postoperative histopathological examination proved the diagnosis. No recurrence and functional disturbance of the hand were observed at 21 months' follow-up.

Validity of joint space width measurements in hand osteoarthritis.

OBJECTIVE: To investigate the validity of joint space width (JSW) measurements in millimeters (mm) in hand osteoarthritis (OA) patients by comparison to controls, grading of joint space narrowing (JSN), and clinical features. METHODS: Hand radiographs of 235 hand OA patients (mean age 65 years, 83% women) and 471 controls were used. JSW was measured with semi-automated image analysis software in the distal, proximal interphalangeal and metacarpal joints (DIPJs, PIPJs and MCPJs). JSN (grade 0-3) was assessed using the osteoarthritis research society international (OARSI) atlas. Associations be

Outcomes of hand surgery in the patient with rheumatoid arthritis.

PURPOSE OF REVIEW: Surgeons have long been striving to develop new surgical procedures to improve functional outcomes for a variety of hand and wrist deformities resulting from rheumatoid arthritis (RA). The purpose of this review is to assess the latest surgical outcomes related to hand and wrist surgery in the patient with RA. RECENT FINDINGS: There is a lack of outcome studies in RA hand and wrist surgery to justify many of the surgical procedures proposed for the treatment of the rheumatoid hand. However, advances made by the silicone metacarpophalangeal arthroplasty (SMPA) group regardin

Classification of osteoporosis and osteopenia in postmenopausal women is dependent on site-specific analysis.

Despite the availability of guidelines from the World Health Organization study group for the classification of osteoporosis in postmenopausal Caucasian women, confusion still exists about the number of sites used for diagnosis and the clinical utility of peripheral bone mass assessments. To examine the diagnosis of osteoporosis and osteopenia based on bone density measurements at single or multiple sites using central and peripheral measurements, we studied 115 ambulatory, community-dwelling, Caucasian postmenopausal women. Bone mineral density of the hip, PA spine, forearm, and finger were a

Imaging of acute injuries to the wrist and hand.

This article reviews the basic characteristics of common fractures and dislocations in the hand and wrist. There is increasing recognition that fractures and dislocations of the hand and wrist can result in long-term pain and dysfunction. This article includes descriptions and, when helpful, classifications for these fractures with attention to those features that are of prognostic and therapeutic significance. With prompt recognition and proper characterization of these injuries, patients can be triaged to the orthopedic surgeon for early and more aggressive treatment.

Complications of plate fixation in the hand skeleton.

Plate fixation of metacarpal and phalangeal fractures is designed to provide rigid internal fixation in order to facilitate early motion and thereby minimize joint and tendon complications. In a series of plate fixations, 16 of 38 (42%) of proximal phalangeal and metacarpal shaft fractures developed complications of stiffness, malunion, nonunion, and tendon rupture. Complications occurred more frequently for phalangeal than for metacarpal fractures and more frequently when there were associated bone or soft tissue injuries. The technique is demanding and secondary procedures are frequently req

A novel closed reduction with extension block and flexion block using Kirschner wires and microscrew fixation for mallet fractures.

BACKGROUND: Some patients with mallet fractures who undergo extension block pinning complain of exposed wires, which delay their return to sports and causes inconvenience while performing tasks that require the use of hands during the early postoperative period. The purpose of this retrospective study was to present and evaluate a novel surgical procedure for mallet fractures. METHODS: We treated 20 patients (14 males and six females; mean age, 38.4 years; range 17-68 years) with displaced mallet fractures involving >30 % of the articular surface using the closed reduction and microscrew fixa

A comparative study of the heterodigital neurovascular island flap in thumb reconstruction, with and without nerve reconnection.

41 heterodigital neurovascular island flaps were used to cover defects of the tactile pad of the thumb in 17 years. With an average follow-up of 75.5 months, 30 patients were reviewed. 17 were treated by the original Littler technique and 13 were treated with the same flap reconstruction but with division of the digital nerve innervating the flap and re-anastomosis of this nerve to the proximal nerve end of the ulnar digital nerve of the thumb. Good aesthetic and functional results were achieved in both groups. Sensory acuity did not appear to decrease with time. The nerve reconnection techniq

Rapid Functional Hand Stabilization in Austere and Operational Environments: A Motion-Permissive Approach.

INTRODUCTION: Upper extremity injuries, particularly those involving the hand, are a significant source of morbidity and reduced operational readiness within military populations. Hand and finger trauma are among the most common musculoskeletal injuries encountered across deployed and non-deployed military settings, frequently affecting the ability to perform mission-essential tasks. In austere and resource-constrained environments, early stabilization strategies must balance protection of injured structures with preservation of function and operational capability. Current military trauma syst

Metacarpophalangeal joint extensor tendon subluxation: a reconstructive stabilization technique.

Extensor tendon subluxation can result from a disruption to the extensor retinacular system and/or the sagittal band at the metacarpophalangeal joint. When conservative treatment is insufficient to correct the subluxation, surgical treatment is necessary and various surgical techniques exist. We present a novel stabilization technique to centralize the extensor tendon using a junctura tendinum to lengthen an extensor digitorum communis tendon graft. This technique is simple and effective, creating a strong repair without associated stiffness. To achieve a less morbid, stable repair, we mimicke

Small joint involvement in psoriatic arthritis is associated with onycholysis: the Reykjavik Psoriatic Arthritis Study.

OBJECTIVES: Psoriasis and psoriatic arthritis (PsA) are associated with nail changes. Recent reports suggest that nail changes may be a part of the enthesitis of PsA and that they predict the onset of arthritis among patients with psoriasis, but they have not reported on subclasses of nail changes. However, earlier reports suggested that onycholysis is the nail change most strongly associated with PsA. If nail changes in PsA are a sign of enthesitis, they might be associated with small joint disease in general and the objective of this study was to test this hypothesis. METHODS: A total of 15

Second toe extensor digitorum brevis provides a simultaneous abductorplasty to free vascularized metatarsophalangeal joint transfer for posttraumatic thumb composite metacarpophalangeal joint defect.

BACKGROUND: For functional reconstruction of a thumb metacarpophalangeal joint defect with loss of opposition, we need to address both joint mobility and thumb opposition. Free vascularized second toe joint transplantation provides joint replacement, as well as extensor and flexor tendons for apprehensile thumb restoration. Furthermore, the extensor digitorum brevis (EDB) allows a simultaneous abductorplasty for the reconstruction of traumatized abductor pollicis brevis. METHODS: Seven patients underwent one-stage vascularized second toe metatarsophalangeal joint transfers to thumb metacarpop

Hemi-hamate arthroplasty provides functional reconstruction of acute and chronic proximal interphalangeal fracture-dislocations.

PURPOSE: Hemi-hamate resurfacing arthroplasty is a treatment alternative for the management of severe acute and chronic dorsal proximal interphalangeal (PIP) fracture-dislocations. This study was designed to determine whether this procedure would successfully restore function after such injuries. METHODS: Hemi-hamate reconstructions were performed on 33 patients (mean age, 34 years) who presented to 1 hand surgery practice with dorsal PIP fracture-dislocations. Eligible patients experienced unstable dislocations with comminuted metaphyseal fractures involving at least 50% of the volar middle
