Neurovascular complications and functional outcome in displaced supracondylar fractures of the humerus in children.

We retrospectively studied the complications associated with a displaced supracondylar fracture of the humerus in children and its treatment. Between 1978 and 1997, 200 displaced fractures were treated by operative means. In 190 cases closed reduction and percutaneous pinning was performed. In 10 cases vascular impairment or unsatisfactory reduction necessitated open exploration. Functional and cosmetic success was achieved in 90% of all operated children. In 33 (16.5%) of all cases we found neurological impairment. All recovered without sequelae, except for one case with persistent radial ner

Cementoplasty and the oncologic population.

The first and only description of percutaneous cementoplasty, to date, has been described in the French medical literature in 1994. In this series of 12 cases, radiologists successfully instilled a cement derivative into the acetabulum under fluoroscopic control. As in these cases, the major indication for cementoplasty is to provide pain control and stabilization of an osteolytic lesion. Potential complications include physical or thermal damage to the adjacent neurovascular structures, either during needle positioning or from cement leakage, respectively. Although no absolute contraindicatio

Comparative Assessment of Proximal Humeral Bone Density Using CT Osteoabsorptiometry, Bone Microarchitecture Analysis, and a HU-Based Calibration Method: A CT and Micro-CT Study in Elderly Body Donors (65-86 Years).

Background: Local bone quality of the proximal humerus is a key determinant of fracture risk and implant stability in osteoporotic bone. Beyond established HU-based calibration, CT-osteoabsorptiometry (CT-OAM)-derived indices and microarchitecture-oriented workflows warrant systematic cross-modality evaluation. Methods: Twelve proximal humeral heads from six body donors (age 65-86 years; bilateral specimens) were analyzed using paired clinical CT and high-resolution micro-CT. Bone quality was quantified by (i) a HU-calibrated cancellous vBMD method (Krappinger et al.), (ii) a CT-OAM-inspired w

Medial Approach Derotational Humeral Osteotomy in Patients with Brachial Plexus Birth Palsy.

BACKGROUND: Medial-approach derotational humeral osteotomy is indicated in patients with brachial plexus birth palsy (BPBP) who have internal rotation contracture, a condition that substantially limits upper-extremity function and creates cosmetic concerns as a result of excessive internal rotation1. This procedure enhances the range of motion of the arm by surgically externally rotating the humerus, thereby facilitating essential activities such as bringing the hand to the mouth and neck without the need for compensatory movements. In addition, the medial approach offers cosmetic benefits; th

Are Vascularized Fibula Autografts a Long-lasting Reconstruction After Intercalary Resection of the Humerus for Primary Bone Tumors?

BACKGROUND: A vascularized fibula graft (VFG) is the vascular autograft most frequently used to restore large segmental long bone defects, particularly in the upper limb. Because the use of a vascularized fibula involves an operation in an uninvolved extremity with potential morbidity, it is important to document that this type of reconstruction is successful in restoring function to the humerus. However, the long-term results of VFG after intercalary resection of the humeral diaphysis for bone tumors are still unknown. QUESTIONS/PURPOSES: (1) What was the complication rate of reconstruction?

Anatomic healing of greater tuberosity improves range of motion and functional outcomes after reverse total shoulder arthroplasty for proximal humerus fractures: an updated systematic review and meta-analysis on 21 studies.

BACKGROUND: Reverse shoulder arthroplasty (rTSA) is increasingly being used to treat complex proximal humerus fractures, especially in the senior population. The question of whether it is worthwhile to repair the greater tuberosity (GT) after rTSA for proximal humerus fractures still needs to be answered. METHODS: A systematic review was performed on PubMed, Scopus, Cochrane library, and Embase for studies available up to February 2024 focused on tuberosity repair after rTSA for proximal humerus fracture and reporting clinical outcomes and range of motion (ROM) between patients with anatomic

Outcomes of coronoid-first repair in terrible triad injuries of the elbow.

BACKGROUND: Clinical outcomes of terrible triad injuries (TTIs) of the elbow are historically poor. To date, it is still debatable whether the coronoid needs to be fixed and if so, how and in which sequence. METHODOLOGY: Between 2010 and 2013, 13 patients were treated surgically for acute TTIs of the elbow at a Tertiary Level 1 Trauma Centre by a single surgeon, using a standardized protocol, which included coronoid-brachialis complex fixation via pull-through trans-osseous sutures, radial head fixation or prosthetic replacement and a repair of the lateral ulnar collateral ligament. Repair of

How reliable are radiographic measures of the pediatric elbow? Measurement and comparison of four parameters in healthy children's elbow.

The reproducibility of radiographic parameters used to assess pediatric supracondylar fracture reduction, including Baumann angle, lateral capitellohumeral angle (LCHA), carrying angle, and anterior humeral line (AHL) crossing the capitellum, was evaluated. Two observers undertook a total of 2368 measures on 148 elbow radiographs from 107 patients aged 0-12 years. The observers, who were blinded to their own measurements and each other's measurements, performed the measurements twice with a 1-month interval using an electronic goniometer. The average value and reliability of the measurements w

Evaluation of enhanced recovery after surgery care in postoperative rehabilitation of pediatric supracondylar humerus fractures: A retrospective cohort study.

Pediatric supracondylar humerus fractures can lead to joint stiffness, muscle contractures, and functional limitations, severely affecting children's quality of life. This study evaluates the impact of an enhanced recovery after surgery (ERAS) care program on postoperative rehabilitation in such fractures. In this retrospective cohort study, 100 children who underwent surgery for supracondylar humerus fractures between March 2021 and May 2024 were divided into 2 groups: an ERAS care group (n = 50) and a conventional care group (n = 50) based on the nursing protocols received. The outcomes meas

Health-economic evaluation of orthogeriatric co-management for patients with forearm or humerus fractures: an analysis of insurance claims data from Germany.

Orthogeriatric co-management (OGCM) describes a collaboration of orthopedic surgeons and geriatricians for the treatment of fragility fractures in geriatric patients. While its cost-effectiveness for hip fractures has been widely investigated, research focusing on fractures of the upper extremities is lacking. Thus, we conducted a health economic evaluation of treatment in OGCM hospitals for forearm and humerus fractures. In a retrospective cohort study with nationwide health insurance claims data, we selected the first inpatient stay due to a forearm or humerus fracture in 2014-2018 either tre

Linked total elbow arthroplasty as treatment of distal humerus fractures.

Treatment of distal humerus fractures may be challenging, especially in the elderly patient. Total elbow replacement has been proposed as an option in selected patients. We present the results of a linked elbow replacement in 16 patients with a comminuted fracture of the distal humerus which was not considered amenable to reliable open reduction and internal fixation. At a mean follow-up of 57 months, average range of motion was from 28 degrees to 117 degrees of flexion-extension. Five patients with moderate to severe pain (31%) were not satisfied with the results of the operation. Three patie

Complication of the use of a radiofrequency device in arthroscopic shoulder surgery: second-degree burn of the shoulder girdle.

Radiofrequency ablation (RF) energy devices are increasingly being used in arthroscopic surgery. This is a case report of a 53-year-old man who had second-degree burns of the shoulder and chest wall as a result of fluid overheating due to RF. During an extensive arthroscopic bursal dissection of the left shoulder, the suction device on the RF wand and the valve was left open, allowing fluid to drip onto the chest and arm of the patient. Three days after his surgery, on his first postoperative visit, the dressings were removed, and the patient was noted to have severe blistering on the lateral
