The treatment of chronic hemodialysis vascular access by directional atherectomy.

Directional atherectomy (DA) was developed as a new therapeutic modality for coronary artery disease. For the past 3 years, we have applied DA intervention to vascular access failure with either poor blood flow rate or high venous pressure. DA intervention was performed on 27 lesions of 16 hemodialyzed patients. A reduction of stenosis to less than 20% of that before treatment was judged a technical success, and the rate of technical success was 84%. All patients showed adequate blood flow rates after DA intervention, indicating initial success. Although restenotic events occurred frequently,

Meso-atrial shunt for Budd-Chiari syndrome: evaluation of patency by magnetic resonance angiography, with color Doppler ultrasound and angiographic correlation.

We report the case of a boy with Budd-Chiari syndrome. At 3 years of age a meso-atrial shunt was inserted for thrombosis of the hepatic veins with associated inferior vena cava occlusion by an enlarged caudate lobe. Seven uneventful years later, the patient was reassessed because of concern about insufficient length of the synthetic shunt. Ultrasound with color Doppler imaging (CDI) and magnetic resonance imaging (MRI) with magnetic resonance angiography (MRA) clearly depicted a patent graft, without kinking, stenosis or other sign of graft dysfunction. Conventional selective angiography confi

[Clinical features and imaging findings of posterior cerebral artery infarction].

The clinical features and imaging findings of posterior cerebral artery infarction (PCAI) in 18 cases verified by CT scan and/or MRI, DSA, SPECT were reported, and their interrelation were analyzed. It was found that the main clinical feature of PCAI was visual defect with amnesia syndrome, the lesion was predominant in the right hemisphere. In examining the structure and function of brain and vessels, each of CT, MRI, ECT and DSA had its own advantage and might complement the others, CT and MRI were better in displaying the structure of cerebral parenchyma than ECT and DSA, whereas, MRI was s

Effects of induced arterial hypertension for vasospasm on unruptured and unsecured cerebral aneurysms (growth and rupture). A retrospective case-control study.

OBJECTIVES: Unruptured cerebral aneurysms (UCAs) often coexist with the ruptured one but are typically left unsecured during the weeks following aneurysmal subarachnoid hemorrhage (aSAH). We compared the rate of UCAs rupture or volume growth (≥5 mm3) between patients exposed to induced arterial hypertension (iHTN) for vasospasm and those not exposed (control group). MATERIALS AND METHODS: From 2013 to 2021, we retrospectively included consecutive adult patients with aSAH who had ≥1 UCA. Custom software for digital subtraction angiography (DSA) image analysis characterized UCAs volume, going b

Vitamin K Antagonist (Phenprocoumon) and Subarachnoid Hemorrhage: A Single-Center, Matched-Pair Analysis.

BACKGROUND: Demographic changes are leading to an aging society with a growing number of patients relying on anticoagulation, and vitamin K antagonists (VKA) are still widely used. As mortality and functional outcomes are worse in case of VKA-associated hemorrhagic stroke, phenprocoumon treatment seems to be a negative prognostic factor in case of subarachnoid hemorrhage (SAH). The purpose of this study was to analyze whether phenprocoumon treatment does worsen the outcome after non-traumatic SAH. METHODS: All patients treated for non-traumatic SAH between January 2007 and December 2016 in ou

Non-Contrast-Enhanced Silent Scan MR Angiography of Intracranial Anterior Circulation Aneurysms Treated with a Low-Profile Visualized Intraluminal Support Device.

BACKGROUND AND PURPOSE: The Low-Profile Visualized Intraluminal Support Device comprises a small-cell nitinol structure and a single-wire braided stent that provides greater metal coverage than previously reported intracranial stents, as well as assumed strong susceptibility artifacts. This study aimed to assess the benefits of non-contrast-enhanced MRA by using a Silent Scan (Silent MRA) for intracranial anterior circulation aneurysms treated with Low-Profile Visualized Intraluminal Support Device stents. MATERIALS AND METHODS: Thirty-one aneurysms treated with Low-Profile Visualized Intralu

Severe Mesenteric Ischemia in a Pregnant Woman with Antiphospholipid Syndrome.

The antiphospholipid syndrome (APS), either primary or secondary form, is considered an autoimmune disease with the presence of at least 1 clinical and 1 laboratory abnormalities as defined by the Sydney criteria. Clinical criteria include vascular thrombosis of venous, artery, small vessel in any organ, and recurrent pregnancy pathologies. Mesenteric ischemia is a rare and threatening manifestation of APS. We herein report a case of a 34-year-old pregnant woman referred to our Emergency Room with thoracic and abdominal acute pain. Her past medical history was remarkable for positivity to anti

[A dural arteriovenous fistula detected during the chronic phase of cerebral venous sinus thrombosis].

A 47-year-old woman developed a headache and tinnitus in her right ear followed by sudden-onset left hemiparesis. She had no history of trauma or thrombophilia. Arteriography showed an occlusion of the superior sagittal sinus, and a stenosis of the right transverse sinus. She was treated with anticoagulants; thereafter her deficits and sinus thrombosis immediately improved. However, 7 months after the onset, she noticed tinnitus in her left ear. Arteriography showed a dural arteriovenous fistula extending from the left occipital artery to the left transverse and sigmoid sinus, but no recurrenc

Embolization of a Large Rapidly Growing Aortic Pseudo-Aneurysm Not Amenable to Open or Endovascular Repair.

BACKGROUND: To report the case of a rapidly growing aortic false aneurysm because of Q fever infection that was managed by embolization. CASE REPORT: An 80-year-old man was admitted to our unit for an aortic false aneurysm from a chronic Q fever infection. During his stay, the aneurysm showed rapid progression necessitating urgent treatment. The patient was unfit for surgical repair; moreover, the location of the aneurysm at the level of the superior mesenteric artery prohibited the use of an endovascular graft. He was managed by coiling of the aneurysmal cavity with multiple detachable coils

Large and giant ventral paraclinoid carotid aneurysms: surgical techniques, complications and outcomes.

OBJECTIVES: Surgical treatment of large and giant ventral paraclinoid carotid aneurysms (vPCAs) continues to be challenging and technically demanding for vascular neurosurgeons. Due to both physiological barriers and limitations of current surgical technology in the treatment of vPCAs, higher rates of morbidity and mortality are a given. To improve the current available data and assess the most effective microsurgical management techniques with the goal of decreased morbidity and mortality, we present this retrospective study of a series of 15 consecutive patients with large and giant vPCAs.

Eagle syndrome causing cerebral infarction attributed to the synergistic effect of mild compression of the styloid process and carotid artery arteriosclerosis: a case report.

BACKGROUND: Cerebral infarction in Eagle syndrome is caused by blood flow disruption attributed to direct compression of the internal carotid artery by the styloid process. Herein, we report a rare case of Eagle syndrome in which mild compression of the styloid process and stylohyoid ligament caused tortuosity of the proximal internal carotid artery, leading to severe stenosis and cerebral infarction. CASE PRESENTATION: An 80-year-old Japanese female patient was urgently transferred from another hospital after experiencing sudden-onset aphasia. Upon arrival at the Emergency Room, magnetic res
