INTRODUCTION: Endoscopic injection of a bulking agent is a common first-line approach to the treatment of vesicoureteral reflux (VUR). Patients with recurrent superficial bladder tumors have been treated by vesical and intradermal administration of Bacillus Calmette-Guerin. The pattern of recurrence in 9 patients has been altered favorably. Although the findings are still preliminary they appear to hold promise of a new therapeutic approach to the treatment of a group of neoplasms for which effective therapy is still lacking.

The bladder neck fascial sling may become a more popular procedure if surgical modifications could ensure more rapid postoperative voiding, yet not compromising the 85% long-term cure rate. In a study of 80 women, we found that increasing the distance from the Prolene knot to the rectus fascia, while simultaneously observing the closure of the bladder neck with a cystoscope, resulted in 92% of women voiding the same day and 93% experiencing no stress incontinence at 1 year.

Trans- or periurethral bladder neck injections are minimal invasive therapies for stress urinary incontinence which can be performed under local anaesthetic as an outpatient procedure. So far the best documented substance is collagen of bovine origin but other substances as silicone, autologous fat, chondrocytes and ethylenevinylalcohol are available. Success rates are between 60 and 80% with deteriorating long term results of approximately 30% requiring reinjection usually after 18 to 24 months.
