Epidemiology of Judo-Related Injuries in 21 Seasons of Competitions in France: A Prospective Study of Relevant Traumatic Injuries.

BACKGROUND: Judo is a full-contact fighting sport that may lead to severe injuries. There are limited data available on the incidence of judo-related injuries. The French Judo Federation has established a surveillance system to document the frequency and type of injuries during judo competitions. PURPOSE: To describe the incidence rates and types of judo-related traumatic injuries during 21 seasons of competitions in France with respect to athlete (judoka) age, sex, and level of performance. STUDY DESIGN: Descriptive epidemiology study. METHODS: Between 1993 and 2014, each physician in char

Musculoskeletal problems in pediatric acute leukemia.

Acute leukemia (AL) in children can mimic several orthopedic pathologies at presentation, with a variable delay in the correct diagnosis. This is a major problem, which may result in fractures, loss of mobility, and deformity, with resultant adverse effects on quality of life. Here, we studied the clinical and radiological musculoskeletal manifestations in children with AL. We reviewed 328 children [208 boys (62%), median age 7.2 years] with acute lymphoblastic (279, 85%) or myeloid (49, 15%) leukemia, treated between January 1982 and December 2003 by the Paediatric Oncology Service, Second Un

Contrasting Outcomes in Chronic Monteggia Fracture Management: Key Lessons From Two Pediatric Cases.

Chronic Monteggia fracture-dislocations in children, defined by presentation beyond four weeks, pose significant surgical challenges. While ulnar osteotomy is central to reconstruction, the optimal technique for ensuring stable radiocapitellar reduction remains a topic of debate. We present the cases of two nine-year-old patients with chronic Bado type I Monteggia lesions. The first patient underwent transverse ulnar osteotomy with annular ligament repair and temporary radiocapitellar fixation but developed recurrent radial head dislocation after K-wire removal. The second patient was treated

Implant selection and fixation in radial head arthroplasty: A qualitative analysis of surgeon decision-making.

BACKGROUND: Radial head arthroplasty (RHA) requires surgeons to choose an implant and a fixation strategy where comparative trial evidence is sparse. How training, intraoperative judgement and tactile feedback shape these choices remains poorly characterised. METHODS: Ten UK consultant orthopaedic surgeons sub-specialised in upper-limb arthroplasty and elbow surgery were recruited through purposive and snowball sampling and interviewed using a semi-structured guide. Transcripts underwent inductive thematic analysis until no new conceptual categories emerged. RESULTS: Five themes and 13 sub-t

Vague Presentation of Cat Scratch Disease in a Child.

Background: Prolonged fever for more than a week or fever of unknown origin in pediatric patients with or without soft tissue infection should raise suspicion for Bartonella henselae infection. Case Report: A 10-year-old female presented to urgent care with a "bug bite" on the left ring finger, cough, and 2 to 3 days of low-grade fever. Ten days later, her symptoms progressed to soft tissue swelling of the left elbow without fracture on radiograph. Magnetic resonance imaging revealed multiple reniform masses with avid contrast enhancement consistent with suppurative adenitis. She was admitted

Acute Joint Blockage due to Abrasion-Related Dislocation of a Silastic Radial Head Prosthesis: A Histological Examination after 14 Years of Durability.

The implantation of a radial head prosthesis can take place as a therapeutic option after radial head fracture. There are various implants for this purpose. Many studies and case reports about silastic radial head prosthesis implantation describe foreign body reactions with accompanying synovitis and poor functional results. A few studies have investigated the reason for the material failure and the accompanying synovitis. The case report presented shows an unusually long durability of an in situ 14-year silastic radial head prosthesis. 14 years after implantation, a previously full-time worki

Two cases of Gorham-Stout disease with good response to zoledronic acid treatment.

Gorham-Stout disease - also called vanishing bone syndrome - is a rare bone disease characterized by a progressive intra-osseous proliferation of non-neoplastic vascular tissue resulting in massive osteolysis. Here, we report two clinical cases of Gorham-Stout disease. Case 1: a 56-year-old woman with 20 years of history of pain and swell in elbows, ankles and wrist. Then she was diagnosed as systemic lupus erythematosus (SLE) with glomerulonephritis type III. After other pathologies were ruled out Gorham-Stout disease was diagnosed. Intravenous zoledronic acid (5 mg) was indicated and after t

Neoprene Orthopaedic Supports: An Underrecognised Cause of Allergic Contact Dermatitis.

Thioureas, often contained within neoprene to provide water resistance, are an important cause of allergic contact dermatitis (ACD) in those who use neoprene products. We wish to present three cases of thiourea-induced ACD from three different orthopaedic supports containing neoprene. The first case was a 67-year-old woman who developed an itchy rash on her heel three weeks after using a neoprene insole for plantar fasciitis. The second case was a 47-year-old man who developed an itchy rash on his wrist after wearing neoprene wrist splints for psoriatic arthropathy. The third case was a 77-yea

Reconstruction of rotator cuff tears in wheelchair-bound paraplegic patients.

BACKGROUND: Rotator cuff surgery in wheelchair-bound patients is challenging, and clinical data on this condition are limited. We hypothesized that rotator cuff repair in these patients might improve functional outcomes. METHODS: In a retrospective study, data on 13 paraplegic patients (8 men and 5 women; 16 shoulders) who underwent rotator cuff repair were analyzed. The average age at the time of surgery was 48.7 years. The causes of paraplegia were poliomyelitis in 9 patients, spinal fracture in 4, and cerebral infarction in 2. Open rotator cuff repair was performed in 11 patients and arthr

Humeral lengthening by distraction osteogenesis: a safe procedure?

This study was conducted to assess the safety of humeral lengthening using an Ilizarov frame. We retrospectively reviewed 26 humeral segments in 17 patients that were lengthened at our department between 1993 and 2011. There were varying aetiologies including achondroplasia, epiphyseal dysplasia, Oilier disease, trauma or infection of the proximal humeral growth-plate, unicameral bone cyst and brachial plexus injury. Mean age at start of surgery was 17.05 years (range : 5-40). The mean lengthening achieved was 8.85 (3-13) cm. Mean lengthening percentage was 353% (range : 10-48). Average healin

Extracorporeal shockwave therapy in musculoskeletal disorders.

The sources of shockwave generation include electrohydraulic, electromagnetic and piezoelectric principles. Electrohydraulic shockwaves are high-energy acoustic waves generated under water explosion with high voltage electrode. Shockwave in urology (lithotripsy) is primarily used to disintegrate urolithiasis, whereas shockwave in orthopedics (orthotripsy) is not used to disintegrate tissues, rather to induce tissue repair and regeneration. The application of extracorporeal shockwave therapy (ESWT) in musculoskeletal disorders has been around for more than a decade and is primarily used in the

Quantifying Solid Waste Production and Sustainability Barriers in Pediatric Orthopaedic Surgery: A Mixed-methods Study.

BACKGROUND: Climate change presents a critical public health challenge, and the health care sector is a substantial contributor to greenhouse gas emissions. Operating rooms generate a disproportionate share of hospital waste, with orthopaedic surgery producing more waste than any other specialty. However, waste production in pediatric orthopaedic surgery remains poorly studied. This study aimed to quantify solid waste generated during common pediatric orthopaedic procedures and assess the perspectives of operating room staff regarding sustainability practices and barriers to waste reduction.

Epidemiologic Trends in Common Injury Mechanisms and Associated Consumer Products in Pediatric Radial Head Subluxation Cases.

BACKGROUND: Nursemaid's elbow injuries occur in young children, with axial traction on the arm as a commonly-described mechanism. Prior demographic studies have noted a significant increase in this injury over the last three decades. We sought to examine incidence of nursemaid's elbow over time and assess the impact of COVID-19 on injury frequency. Additionally, certain household products and female gender have been frequently seen in cases presenting to emergency departments. The present study aims to further investigate injury scenarios to better understand the difference in injury rates and

Mechanical distraction for the treatment of posttraumatic stiffness of the elbow in children and adolescents.

BACKGROUND: Elbow contracture is a recognized sequela of elbow injuries in children and adolescents, but previous studies of operative treatment with formal capsular release have demonstrated unpredictable outcomes and unfavorable results. METHODS: Over a period of five years, fourteen children and adolescents with a mean age of fourteen years who had posttraumatic stiffness of the elbow were managed according to a prospective protocol. Eleven patients had undergone a mean of three previous operative procedures before the index operation. After intraoperative distraction with an external fixa

[SERM & bone metabolism: protective effects of SERM against fracture risk and its long-term beneficial effects].

The Multiple Outcomes of Raloxifene Evaluation (MORE) Trial enrolled 7,705 women with osteoporosis, defined by prevalent vertebral fractures and/or a bone mineral density (BMD) T score at -2.5 or below, who were treated with placebo or raloxifene at a dosage of 60 or 120 mg/day for 3 years. Raloxifene hydrochloride therapy significantly reduces the risk for vertebral fractures at 3 and 4 years administration group. The fact that Raloxifene administration causes the significant 25% reduction in overall risks in postmenopausal women indicate a favorable risk-benefit profile. These data suggested

Posterior instability of the shoulder after supracondylar fractures recovered with cubitus varus deformity.

Cubitus varus is a common complication after supracondylar fractures of the humerus, and there have been several discussions about the timing of correction of deformity. Although surgery is performed mainly for cosmetic purposes, the authors in this article show the relationship between cubitus varus and dislocation of the ulnar nerve and posterior instability of the ipsilateral shoulder with a Bankart lesion in three children. A special type of osteotomy to obtain three-dimensional correction was made. All shoulders were found to be clinically stable at follow-up with full pain-free range of
