Percutaneous nephrolithotomy under combined sonographic/radiologic guided puncture: results of a learning curve using the modified Clavien grading system.

PURPOSE: Aim of our study was to evaluate the results of percutaneous nephrolithotomy (PNL) with sonographic/fluoroscopic guided puncture during a learning curve and to compare them to an expert. METHODS: The first 75 consecutive patients undergoing conventional PNL or Mini-PNL by a novice percutaneous surgeon without direct supervision by an expert were evaluated and divided into three groups of 25 patients each. The results were compared to 50 cases treated by an expert. RESULTS: For mean stone sizes of 22.3 ± 12.9, 22.7 ± 14.9, and 31.1 ± 25.5 mm, the OR-time was 124 ± 35, 106 ± 37, and 9

Rhabdomyolysis and acute renal failure following prolonged surgery in the lithotomy position.

Operative positions commonly used in urogenital surgeries when perineal exposure is required include the lithotomy and the exaggerated lithotomy positions (LPs), which expose patients to the risk of rhabdomyolysis. We report a patient with bladder outflow obstruction, benign prostatic hypertrophy and a very large bladder stone, which was removed with cystoscopy and cystolitholapaxy in the LP. The procedure was complicated by posterior bladder perforation and abdominal distention leading to prolonged surgery duration (5.5 h). The patient developed rhabdomyolysis and acute renal failure (ARF) wi

Primary hyperparathyroidism in pediatric patients.

OBJECTIVE: Primary hyperparathyroidism (HPT) is unusual in children. We reviewed our experience with HPT to better characterize these children. METHODS: The retrospective review of patients <19 years old who underwent parathyroid resection for primary HPT from 1970 to 2000 was performed at a single institution.. RESULTS: Fifty-two patients were identified. Median age was 16.8 years (range: 4-18.9) with a female-to-male ratio of 3:2. Eighty-five percent had an elevated parathyroid hormone (PTH) level, and 15% had an inappropriately normal PTH level during hypercalcemia. Serum calcium was elev

[Ultrasound diagnosis of ruptured abdominal aortic aneurysm in renal colic].

The ruptured abdominal aortic aneurysm is a rare, but life-threatening disease that demands fast diagnosis. We report in the following about a patient who was admitted with symptoms of urinary obstruction and an acute abdomen. Ultrasound investigation at admission confirmed urinary obstruction of the right kidney. Causative for that however was a right pararenal ruptured abdominal aortic aneurysm, which led secondarily to ureter compression. Thus, in our opinion, primary ultrasound examination represents an important instrument in differential diagnosis.

Dissolution of uric acid stones causing acute obstructing anuria in three consecutive infants.

We report on 3 infants with obstructive anuria due to bilateral uric acid stones. In all patients the precipitating cause of acute obstructive renal failure was choking of bilateral ureteropelvic junction with numerous small uric acid stones without cause pelvicaliceal dilatation. While the chemical dissolution therapy succeeded in dissolving the stones in 2 patients, unilateral percutaneous nephrostomy application associated with alkalization achieved dissolution of obstructing stones in the remaining 1 patient.

Cultures of exfoliated epithelial cells from different locations of the human urinary tract and the renal tubular system.

Exfoliated human urinary tract epithelial cells and renal tubular cells from urinary sediments of healthy adults, of urological patients and of internal patients were isolated and cultured. Cells started proliferating within 1 week after seeding a sediment. Proliferating cells formed colonies of different morphologies, designated as type-1 or type-2 cell colonies. Type-1 cell colonies showed irregular contours and spindle-like cells within the colonies. Subcultivation of type-1 cells for up to six passages was possible. Type-2 cell colonies showed smooth-edged contours and subcultivation was n

Potassium-magnesium citrate versus potassium chloride in thiazide-induced hypokalemia.

BACKGROUND: The purpose of this study was to compare the value of potassium-magnesium citrate (KMgCit) with potassium chloride in overcoming thiazide-induced hypokalemia. METHODS: Sixty normal subjects first took hydrochlorothiazide (HCTZ; 50 mg/day). After three weeks of treatment (or earlier if hypokalemia developed), they were randomized to take KMgCit (42 mEq K, 21 mEq Mg, and 63 mEq citrate/day) or potassium chloride (42 mEq/day) for three weeks while continuing on HCTZ. RESULTS: KMgCit significantly increased the serum potassium concentration from 3.42 +/- 0.30 mEq/L on HCTZ alone to a

MAG3-F0 scintigraphy in decision making for emergency intervention in renal colic after helical CT positive for a urolith.

UNLABELLED: Patients with renal colic are evaluated with clinical, laboratory, and imaging methods for stratification for emergency decompression, medical treatment, or discharge and follow up. The current standard practice is heavily based on unenhanced helical CT for detecting uroliths. However, the presence of a urolith does not necessarily mean that the kidney is obstructed and requires emergency decompression. In this study, technetium-mercaptoacetyltriglycine (MAG3) diuretic scintirenography was used to detect obstruction in patients with renal colic. The contribution of this test to pat

Imaging in pediatric urology.

A multitude of disorders of the genitourinary tract can occur in children. Although some entities may be diagnosed clinically, radiologic imaging is often necessary for diagnosis and management. The radiologic work-up has been discussed using a problem-oriented approach in five clinical situations: urinary tract infection, hydronephrosis or hydroureter, trauma, swollen scrotum, and hematuria. This discussion provides some general guidelines, although the evaluation of each child may need to be individualized depending on their specific clinical symptomatology.

Effect of urinary tract infection on disintegration of calcium-containing stones.

To evaluate the possible effect of urinary tract infection (UTI) on the fragility of calcium-containing stones (calcium-oxalate, calcium-phosphate), the results of extracorporeal shock wave lithotripsy (ESWL) therapy in 81 patients with UTI and 30 patients with no sign of UTI were comparatively evaluated. ESWL parameters (shock wave number, kilovolt value, treatment time) and stone-free status during 3 and 6 months of follow-up were assessed for each patient, and the results were evaluated with respect to stone presence. Our results indicated that the presence of UTI may decrease stone fragili

[Outlook and experience in using ofloxacin in urology].

Ofloxacin was used in the empirical and etiotropic therapy of patients with severe urinary tract infection. The trial was based on the previous experience with ofloxacin in the treatment of various forms of urological infection with determination of the pathogen susceptibility and study of the drug pharmacokinetics in patients with normal and impaired renal function. For the first 3 to 5 days in the present study ofloxacin was administered in a dose of 200 mg twice a day followed by its oral use in the same dosage for 3 to 5 days. The treatment schemes were corrected by the bacteriological fin

Relative hypoparathyroidism and calcitriol up-regulation in hypercalciuric calcium renal stone formers--impact of nutrition.

The issue of secondary hyperparathyroidism in idiopathic hypercalciuria (IH) was addressed in 61 male idiopathic calcium stone formers (SF) who underwent metabolic evaluation on a free-choice diet as well as bone mineral density (BMD) measurements by dual-energy X-ray absorptiometry. They were divided into hypercalciurics (HCSF, n = 30, UCa X V > 7.5 mmol/day) and normocalciurics (NCSF, n = 31, UCa X V < 7.5 mmol/day). At identical blood Ca2+ levels, parathyroid hormone (PTH) was lower in HCSF (25.3 +/- 1.8 pg/ml) than in NCSF (31.4 +/- 1.8 pg/ml, p = 0.017). Since neither fasting urinary hydr

Partial ileal bypass surgery in the treatment of heterozygous familial hypercholesterolemia: a review.

Partial ileal bypass (PIB) surgery is a method in the treatment of heterozygous familial hypercholesterolemia (FH). Since the first report in 1964 about 150 cases of FH who underwent the surgical procedure have been described. This number is very low when compared to other types of cholesterol-lowering treatment. On average, PIB decreases the level of plasma total cholesterol by 35% in FH patients, and the surgical procedure can be considered the most effective, single cholesterol-lowering method. PIB-induced reduction of plasma cholesterol is permanent. Further decrease of plasma cholesterol

Morbid obesity: problems associated with operative management.

A review of the problems associated with extensive jejunoileal bypass for morbid obesity in a series of 175 carefully selected patients is presented. Five postoperative deaths occurred (3%). Nonfatal complications occurred in 21%, with wound infections (14 patients) being the most common. Good results marked by weight reduction to the range of ideal weight without significant electrolyte or metabolic aberrations was observed in 82% of the patients receiving the current dimensional modificatiom of end-to-end jejunoileal bypass (30 cm to 20cm). An additional 13% had fair results and only 5% had

Uric acid levels mediate the association between four dietary indices and kidney stones in US adults: A cross-sectional study of NHANES 2007-2018.

BACKGROUND: Diet and uric acid are closely linked to the formation of kidney stones. However, the specific dietary indices associated with kidney stone risk and the mediating role of uric acid remain unclear. This study investigates the association between four dietary indices and kidney stone risk while exploring the mediating role of uric acid. METHODS: Data were obtained from the National Health and Nutrition Examination Survey (NHANES). Four dietary indices were calculated: the Mediterranean Diet (MED), Healthy Eating Index-2020 (HEI-2020), Alternate Healthy Eating Index (AHEI), and Dieta

G6PC Downregulation Promotes Renal Calcium Oxalate Stone Formation via Lactate-Induced SNAIL1 K206 Lactylation and Epithelial-Mesenchymal Transition.

Nephrolithiasis is increasingly prevalent worldwide. Although metabolic dysregulation has emerged as an important feature of the disease, the specific metabolic programs within tubular epithelial cells that drive stone formation remain poorly understood. Here, we use single-cell transcriptomics to profile mouse models of nephrolithiasis and identify a marked suppression of gluconeogenesis in renal proximal tubular epithelial cells, driven by downregulation of the rate-limiting enzyme glucose-6-phosphatase (G6PC) and associated with lactate accumulation. We further show that loss of G6PC or lac
