Use of the anterior cricoid split operation in infants with acquired subglottic stenosis.

Ten infants with acquired subglottic stenosis were treated successfully using the anterior cricoid split operation. Eight had been preterm infants who required endotracheal intubation and assisted ventilation for the respiratory distress syndrome. Two had been born at term; 1 required mechanical ventilation for aspiration of meconium and the other intubation for obstructive apnea. All had been extubated successfully in less than 2 wk and were discharged home with no signs of subglottic stenosis. These infants were referred at 2 to 13 months of age for evaluation of upper airway obstruction. By

Anterolateral extrapharyngeal approach for cervical osteophyte-induced dysphagia. Literature review.

While it is estimated that hypertrophic cervical osteophytes occur in up to 20% to 30% of the population, they are only rarely associated with dysphagia. Pathophysiologically, dysphagia may occur secondary to 1) mechanical compression with partial obstruction, or 2) periesophageal inflammation caused by pharyngoesophageal motion over the osteophytes. A careful history, indirect laryngoscopy, cineesophagography, and lateral cervical spine films establish the diagnosis in most patients. While routine rigid endoscopy is potentially hazardous in view of the recognized risk of inadvertent pharyngoe

[Pediatric Anesthesia - Anesthesia for Congenital Heart Defects - Anesthesiological Approaches for Diagnostic and Surgical Procedures].

Congenital heart disease (CHD) is the most common congenital malformation, affecting approximately 1 in 100 newborns. Due to advances in clinical management of CHD, the population of patients with congenital heart disease is growing. Gaining knowledge and expertise in the perioperative management of patients with CHD is hence becoming increasingly important not only for specialized, but for all anesthetists. The perioperative management of patients with CHD varies depending on the anatomy and pathophysiology of the specific defect and the planned intervention. A pathophysiological classificatio

Standing NanoNeedle Arthroscopy of the Distal Interphalangeal Joint for Removal of Osteochondral Fragments of Distal P2 and the Extensor Process in a Horse.

BACKGROUND: In horses, standing arthroscopy offers a valuable alternative to general anesthesia for the reduction of anesthetic-associated risks, and provides improved visualization in joints where access is facilitated by weightbearing. These techniques are particularly advantageous for patients with increased anesthetic risk or financial limitations. To the authors' knowledge, the removal of osteochondral fragments from the distal interphalangeal joint in a standing, sedated horse using small-diameter arthroscopic equipment has not been previously reported. CASE PRESENTATION: A 7-year-old A

Mechanisms of Propofol in Alleviating Neuropathic Pain by Inhibiting Microglial Pyroptosis in Spinal Cord Through the KDM3A/WNT5A Axis.

Propofol is a short-acting intravenous anesthetic commonly used for induction and maintenance of general anesthesia and sedation during medical procedures. This study aimed to explore the mechanism of propofol on microglial pyroptosis in spinal cord in neuropathic pain (NP). A chronic constriction injury (CCI) model of NP was established in rats. The paw withdrawal mechanical threshold (PWMT) and paw withdrawal thermal latency (PWTL) were measured in rats. The tissue pathology was observed. The expression of NLR family pyrin domain containing 3 (NLRP3) and Iba1 was measured. Microglial cells (

[Sonographic diagnosis of lingual abscess formation in 2 horses with feeding difficulties].

Feeding disorders in horses may have various underlying causes. A thorough clinical examination of the oral cavity is essential for clarification and is often supplemented by imaging techniques such as oral endoscopy, radiography, and computed tomography (CT) of the head. Sonographic examinations of the head and oral cavity are less commonly described diagnostic methods for the investigation of feeding disorders. This case report describes 2 horses presented to the clinic due to impaired feed intake. In both cases the radiographic examination of the head did not reveal any abnormalities relate

Ultrasound-guided femoral nerve block combined with lateral femoral cutaneous nerve block in a patient with congenital insensitivity to pain and anhidrosis: a case report.

Congenital insensitivity to pain with anhidrosis (CIPA), also known as hereditary sensory and autonomic neuropathies (HSAN I-V), is an exceptionally rare autosomal recessive disorder. The pathogenesis of CIPA remains not fully elucidated. Clinical manifestations primarily include the absence of pain perception, painless injuries to the extremities, oral mutilation, anhidrosis accompanied by hyperthermia, and delayed intellectual development. Given the autonomic dysfunction in CIPA patients, previous reports on anesthetic management for this rare disorder have predominantly depended on general

A Rare Outcome for Transient Parotitis After Upper Gastrointestinal Endoscopy: A Case Report and Brief Review of the Literature.

Upper gastrointestinal (GI) endoscopy, though generally safe, can rarely cause complications such as transient parotitis, which typically resolves within 24 hours. Parotitis may occur due to salivary duct blockage, venous congestion from straining, or reflex parasympathetic stimulation. We discuss a 33-year-old chronic alcoholic man who developed right parotid gland swelling immediately following an upper GI endoscopy, conducted without sedation to evaluate his epigastric pain, vomiting, anorexia, and weight loss. His blood tests and abdominal ultrasound were normal. Patient developed sharp pa

Frameworks for value-based care in the nonoperating room setting.

PURPOSE OF REVIEW: Nonoperating room anesthesia (NORA) presents a unique opportunity for the application of value-based care (VBC) principles to procedures performed in the office-based and nonoperating room inpatient settings. The purpose of this article is to review how value is defined in NORA and enabling principles by which anesthesiologists can maximize value in NORA. RECENT FINDINGS: In order to drive value, NORA providers can target improvements in clinical outcomes where NORA lags behind operating room-based anesthesia (death, over-sedation, nerve injury), implement protocols focusin

[Overtreatment in intensive care medicine].

Overtreatment, which is therapy that is neither indicated nor desired by the patient ("non-beneficial"), presents an inherent and huge problem of modern medicine and intensive care medicine in particular. Overtreatment concerns all aspects of intensive care medicine, may start already before admission at the emergency scene, the inappropriate admission to the intensive care unit, overuse in diagnostics and especially in blood sampling, in invasive procedures and in organ support therapies. It manifests itself as "too much" in sedation, relaxation, volume therapy, hemodynamic support, blood pro

[Regional anesthesia - are the standards changing?].

Regional anesthesia has undergone many changes over the years and the increasing use of ultrasound has certainly played an important role in this. Apart from individual case reports in the literature of very different blocking options, some new procedures seem to have become established and can be broadly applied. Among these are blockades, by which ultrasound-guided injection of local anesthetics is carried out in fascial or muscular layers rather than around target nerves (e.g. cervical plexus blocks and truncal blocks). In addition, the precision with which ultrasound can be used to identif

Inferior hip dislocation after falling from height: A case report.

INTRODUCTION: Traumatic inferior hip dislocation is the least common of all hip dislocations. Adult inferior hip dislocations usually occur after high-energy trauma, very few cases are reported without fracture. PRESENTATION OF CASE: A 26-year-old female was brought to the emergency department with severe pain in the left hip, impaired posture and restricted movement following a fall from 15m height. The hip joint was fixed in 90° flexion, 15° abduction, and 20° external rotation. No neurovascular impairment was determined. On radiologic examination, a left ischial type inferior hip dislocati

A multidisciplinary approach to the management of anterior mediastinal masses in children.

PURPOSE: Anterior mediastinal masses (AMM) pose a diagnostic challenge to surgeons, oncologists, anesthesiologists, intensivists, and interventional radiologists as induction of general anesthesia can cause airway obstruction and cardiovascular collapse. We hypothesized that in the majority of patients, diagnosis can be obtained through biopsy of extrathoracic tissue. METHODS: We performed a retrospective review of all patients in the solid tumor oncology clinic with a diagnosis of AMM between 2002 and 2012 including preoperative evaluation and management prior to obtaining a tissue diagnosis

Orthostatic Dysregulation during Postural Change on the Dental Chair and Intraoperative Monitoring by Heart Rate Variability Analysis.

This is the first case report of orthostatic dysregulation (OD) manifested during postural change on the dental chair and intraoperatively monitored by heart rate variability (HRV) analysis. OD-associated autonomic dysfunction is induced by postural changes and easily leads to disturbance in circulatory dynamics; however, most dental practices have not yet realized the importance of managing OD. We measured autonomic activity in a patient with OD during dental therapy and assessed the clinical significance of HRV analysis for OD. The patient was a 17-year-old Japanese female. She was diagnosed

Histological diagnosis of interstitial lung diseases by cryo-transbronchial biopsy.

BACKGROUND AND OBJECTIVE: The gold standard for the histological diagnosis of interstitial lung diseases (ILD) is an open lung biopsy (OLB). Tissue samples obtained by forceps transbronchial lung biopsies (TBB) are usually too small. We aim to evaluate the efficacy and safety of cryo-TBB for the diagnosis of ILD and to explore its role as substitute for OLB. METHODS: Seventy-five patients (mean age 56.2 years) with clinical and radiological features suggestive of ILD underwent cryo-TBB under moderate sedation. The diagnostic contribution on the work-up of suspected ILD was assessed. RESULTS:

Clinical uses of melatonin in pediatrics.

This study analyzes the results of clinical trials of treatments with melatonin conducted in children, mostly focused on sleep disorders of different origin. Melatonin is beneficial not only in the treatment of dyssomnias, especially delayed sleep phase syndrome, but also on sleep disorders present in children with attention-deficit hyperactivity, autism spectrum disorders, and, in general, in all sleep disturbances associated with mental, neurologic, or other medical disorders. Sedative properties of melatonin have been used in diagnostic situations requiring sedation or as a premedicant in c
