[No motor signs in Parkinson's disease].

In Parkinson's disease, motor signs have long been the main targets of the management of the disease. In recent years, non-motor disorders have elicited increasing interest. These disorders are under diagnosed and managed more difficultly than motor signs and are sometimes perceived as more disturbing by the patients. These signs are polymorphous, sometimes occurring before the motor symptoms but increase with the disease duration and complicating always the late stages. They may fluctuate as the motor signs, while being under the control of dopaminergic pathways, or be linked to the degenerat

Correlations between the same motor cortex cells and arm muscles during a trained task, free behavior, and natural sleep in the macaque monkey.

Traditionally, the neural control of movement has been studied by recording cell activity in restrained animals performing repetitive, highly trained tasks within a restricted workspace. However, the degree to which results obtained under these conditions are valid during natural, unconstrained behavior remains unknown. Using an autonomous, implantable recording system, we examined the relationships between the firing of motor cortex cells and forearm muscle activity in primates under three behavioral conditions: performance of a conventional torque-tracking task, unrestrained behavior, and na

Psychiatric diagnoses of patients visiting a sleep disorder clinic due to dyssomnias.

This study analyzed the demographic characteristics and psychiatric diagnoses of 90 consecutive patients who visited a sleep disorder clinic in Taiwan with complaints of insomnia (difficulty initiating or maintaining sleep, or nonrestorative sleep) or hypersomnia (excessive sleepiness as evidenced by either prolonged sleep episodes or daytime sleep episodes that occur almost daily). All subjects were interviewed using a sleep disturbance questionnaire and the structured Mini-International Neuropsychiatric Interview supplemented by the DSM-IV criteria for psychiatric diagnoses. Among 90 patient

Sleep microarchitecture as a predictor of recurrence in children and adolescents with depression.

Although polysomnographic abnormalities are prevalent in adults with major depressive disorders (MDD), the findings in children and adolescents have been more equivocal. Polysomnographic measures may be of predictive value in assessing course of illness. The present study used standard sleep measures and temporal coherence of sleep electroencephalogram (EEG) rhythms to predict recovery and recurrence in a 1-yr naturalistic follow-up in 47 children and adolescents 8-18 yr of age with MDD. Standard sleep measures did not predict clinical course. On the other hand, temporal coherence measures dis

Homeostatic sleep regulation is preserved in mPer1 and mPer2 mutant mice.

Deep brain stimulation (DBS) is a surgical procedure that directs chronic, high frequency electrical stimulation to specific targets in the brain through implanted electrodes. Deep brain stimulation was first implemented as a therapeutic modality by Benabid et al. in the late 1980s, when he used this technique to stimulate the ventral intermediate nucleus of the thalamus for the treatment of tremor. Currently, the procedure is used to treat patients who fail to respond adequately to medical management for diseases such as Parkinson's, dystonia, and essential tremor. The efficacy of this proced

Sleep disturbance associated with chronic tinnitus.

Sleep disturbance is a common and frequent complaint reported by tinnitus sufferers. Recent studies have shown that when insomnia and depression are associated with tinnitus there is decreased tolerance and increased discomfort with the tinnitus. The purpose of this study was to assess the reported prevalence and severity of sleep disturbance in chronic tinnitus patients. Patients (n = 80) were military personnel without major psychiatric disturbance and their tinnitus was associated with noise-induced permanent hearing loss (NIHL). Mini Sleep Questionnaire (MSQ) scores for sleep disturbance w

Characteristics of the sleep of men in simulated space flights.

A study of the sleep of men in simulated space flight has shown that, during different effects (diminished motor activity, noise, rotation), monotony and time of the exposure is the general pathogenetic factor producing sleep disturbances. The monotony factor was responsible for typical changes in sleep function (difficulty involved in falling asleep, disturbed continuity of sleep as a process, decline in the depth, etc.), whose degree was related not only to the professional importance of the experiment but also to the individual intolerance to the main exposure. In these cases, the action of

Lack of genetic evidence for a role of SLC25A46 in alpha-synucleinopathies.

BackgroundThe SLC25A46 gene encodes a mitochondrial carrier protein previously implicated in neuropathy and optic atrophy. Biallelic variants in SLC25A46 have been described in patients with Parkinson's disease (PD) with optic atrophy, but the evidence supporting a role in PD remains limited.ObjectiveTo assess whether SLC25A46 variants contribute to PD, REM sleep behavior disorder (RBD), or dementia with Lewy bodies (DLB).MethodsWe examined common variants using four representative PD genome-wide association studies (GWAS) and an RBD GWAS and applied summary-data-based Mendelian randomization

Cholinergic degeneration and early cognitive signs in prodromal Lewy body dementia.

INTRODUCTION: Isolated REM sleep behavior disorder (iRBD) is a strong prodromal marker of Lewy body diseases (LBDs) - Parkinson's disease (PD) and dementia with Lewy bodies (DLB). Cholinergic loss is linked to cognitive decline in these conditions, but its trajectory remains unclear. METHODS: In a cohort of 92 iRBD participants with baseline MRI, cholinergic basal forebrain (cBF) volume was measured, with longitudinal changes analyzed in 49 with follow-up scans. Cross-sectional neuropsychological associations were examined across a broader RBD-LBD continuum, including the iRBD cohort plus 65

Pharmacotherapy for sleep bruxism.

BACKGROUND: Sleep bruxism is an oral activity characterized by involuntary teeth grinding or clenching during sleep. Several forms of treatment have been proposed for this disorder, including behavioural, dental and pharmacological strategies. OBJECTIVES: To evaluate the effectiveness and safety of pharmacological therapy for the treatment of sleep bruxism compared with other drugs, no treatment or placebo. SEARCH METHODS: We searched the Cochrane Central Register of Controlled Trials (CENTRAL) (Issue 8, 2014), MEDLINE (1966 to August 2014), EMBASE (1980 to August 2013) and LILACS (1982 to A

Sleep-wake cycles and the management of respiratory failure.

Sleep is characterized by many changes in the respiratory system, including a reduction in respiratory motor output associated with the loss of wakefulness, increased upper airway resistance, and blunted protective reflexes (such as load compensation), that result in reduced alveolar ventilation. The development of carbon dioxide retention appears to be linked to the exaggeration of sleep-related changes on ventilation by coexistent respiratory system disorders. Sleep-disordered breathing is becoming increasingly recognized in subjects with neuromuscular diseases, who may be prone to nocturnal

Quality of life in patients with an idiopathic rapid eye movement sleep behaviour disorder in Korea.

There have been few quality of life studies in patients with idiopathic rapid eye movement sleep behaviour disorder. We compared the quality of life in idiopathic rapid eye movement sleep behaviour disorder patients to healthy controls, patients with hypertension, type 2 diabetes mellitus without complication and idiopathic restless legs syndrome. Sixty patients with idiopathic rapid eye movement sleep behaviour disorder (24 female; mean age: 61.43 ± 8.99) were enrolled retrospectively. The diagnosis was established based on sleep history, overnight polysomnography, neurological examination an

The effects of trazodone on sleep in patients treated with stimulant antidepressants.

BACKGROUND AND PURPOSE: To evaluate the effects of trazodone on subjective and objective measures of sleep in depressed insomnia patients treated with selective serotonin reuptake inhibitors (SSRIs). SSRIs can exacerbate or cause new insomnia while alleviating other symptoms of depression. Trazodone has been reported to be an effective hypnotic for patients with antidepressant-associated insomnia. PATIENTS AND METHODS: Twelve female patients were given either 100 mg trazodone or placebo for 7 days in a double-blind crossover design with a 7-day washout period. Polysomnographic recordings were

Sleep apnoea among Australian Aboriginal and non-Aboriginal patients in the Northern Territory of Australia-a comparative study.

Australian Aboriginal and Torres Straight Islanders (ATSI) are noted to have a higher burden of chronic health conditions. However, there is a paucity of data on obstructive sleep apnoea (OSA) in this population. In this retrospective study, we evaluated the clinical and polysomnographic (PSG) characteristics of ATSI and non-ATSI adult patients who underwent diagnostic PSG between 2011 and 2015. There were a total of 3078 patients. Of the total, 403 (13%) were of ATSI origin. Among those of ATSI origin, 61% were male and 39% females, while among the non-ATSI cohort, 66% were males. The median

Evaluation of polygenic risks for narcolepsy and essential hypersomnia.

In humans, narcolepsy is a sleep disorder that is characterized by sleepiness, cataplexy and rapid eye movement (REM) sleep abnormalities. Essential hypersomnia (EHS) is another type of sleep disorder that is characterized by excessive daytime sleepiness without cataplexy. A human leukocyte antigen (HLA) class II allele, HLA-DQB1*06:02, is a major genetic factor for narcolepsy. Almost all narcoleptic patients are carriers of this HLA allele, while 30-50% of EHS patients and 12% of all healthy individuals in Japan carry this allele. The pathogenesis of narcolepsy and EHS is thought to be partia

Transcriptional alterations of genes related to fertility decline in male rats induced by chronic sleep restriction.

The adverse effects of sleep disorders on male fertility are of increased concern. In this study, a rat model of chronic sleep restriction (CSR) was established using the modified multiplatform method. The effects of CSR on the fertility of male rats were evaluated first based on sexual behavior. Serum hormones, including testosterone (T), prolactin (PRL), luteinizing hormone (LH) and follicle-stimulating hormone (FSH), and sperm parameters (concentration, viability, motility, deformation rate) were measured, and testicular histology was analysed by hematoxylin and eosin staining. The transcri

EEG stabilization and cognitive gains after pulse steroid therapy in developmental and/or epileptic encephalopathy with spike-wave activation in sleep: A prospective cohort study.

BACKGROUND: Developmental and/or epileptic encephalopathy with spike-wave activation in sleep (D/EE-SWAS) is a rare paediatric epileptic encephalopathy characterized by marked activation of epileptiform discharges during non-REM sleep, leading to seizures and neurocognitive regression. Although corticosteroids are frequently used, prospective data on structured pulse steroid regimens and long-term neurocognitive outcomes from resource-limited settings are scarce. OBJECTIVE: To characterize the clinical, electroencephalographic, and neuropsychological profile of children with D/EE-SWAS and to
