Evaluation of a pilot outreach program to support point-of-care screening for individuals with diabetes who are experiencing homelessness in Alberta, Canada: results of a mixed methods study.

BACKGROUND: Accessing diabetes care requires effort and time. Homelessness often forces individuals to prioritize securing shelter and safety over medical appointments, particularly for the screening and prevention of long-term diabetes complications. We evaluated the effectiveness and costs associated with a point-of-care screening program with expedited referral pathways within two inner-city community sites in Calgary, Canada serving individuals experiencing homelessness. METHODS: We conducted a non-randomized concurrent-convergent mixed methods pilot study of the program. Adults experienc

Medical liability related legislation and insurance policies around the world: A narrative literature review.

INTRODUCTION: Medical law defines the rules of healthcare by specifying the rights of healthcare consumers while at the same time defining the obligations of healthcare providers. When clinicians fail to provide the standard of care, medical malpractice claims arise, leading to medical liability. While many malpractice claims allege deviation from the lex artis, a substantial proportion do not ultimately establish negligence and are dismissed or settled without a finding of fault. Due to the significant impact of medical negligence on the quality of medical care, as well as on the domestic and

A retrospective evaluation of the BIOFIRE FilmArray Blood Culture Identification 2 panel for the detection of pathogenic yeasts.

Bloodstream infections (BSIs) caused by opportunistic yeasts, namely those of the genus Candida, demonstrate strikingly high mortality rates and require early therapeutic intervention to optimize patient outcomes. Advancements in microbiological diagnosis of BSIs have introduced rapid molecular methods, including multiplex PCR panels, to detect common organisms directly from blood culture media. The BIOFIRE FilmArray Blood Culture Identification 2 (BCID2) assay is one such method and detects seven of the most common yeast species responsible for fungemia, enabling timelier therapeutic optimiza

Application of Nudges to Design Clinical Decision Support Tools: Systematic Approach Guided by Implementation Science.

BACKGROUND: Clinical decision support (CDS) is one strategy to increase evidence-based practices by clinicians. Despite its potential, CDS tools produce mixed results and are often disliked by clinicians. Principles from behavioral economics such as "nudges" may improve the effectiveness and clinician satisfaction of CDS tools. This paper outlines a pragmatic approach grounded in implementation science to identify and prioritize how to incorporate different types of nudges into CDS tools. OBJECTIVE: The purpose of this paper is to describe a systematic and pragmatic approach grounded in imple

Epidural and Subdural Hematomas in the Democratic Republic of Congo: A Retrospective Review of 54 Operated Cases in a Tertiary Neurosurgery Unit in Africa.

Operated epidural hematoma (EDH) and subdural hematoma (SDH) remain a major cause of potentially preventable death after traumatic brain injury (TBI) in low- and middle-income countries. Local outcome data from the Democratic Republic of Congo (DRC) are scarce, hampering evidence-based triage and resource allocation. We reviewed all surgically treated EDH and SDH cases managed at a tertiary neurosurgical center in Kinshasa to characterize epidemiology, treatment strategies, and early prognostic factors. A retrospective cohort study was conducted at Clinique Ngaliema from January 1, 2021, to Ap

Percutaneous spinopelvic fixation technique using external fixation for focal kyphosis reduction in U-type sacral fractures: a case report.

BACKGROUND: Sacral fractures, particularly U-type fractures characterized by a transverse fracture line, result in significant instability and deformity, including focal kyphosis. These fractures challenge biomechanical integrity and neural structures, often leading to long-term disability if not corrected. Surgical approaches vary, but percutaneous spinopelvic fixation offers benefits like reduced soft tissue trauma and expedited recovery. This case report highlights managing a displaced U-type sacral fracture with focal kyphosis using a percutaneous spinopelvic reduction technique followed b

Reduction of traumatic unilateral locked facet of the subaxial cervical spine: what predicts successful closed skeletal traction, and is anterior or posterior surgery superior after unsuccessful closed reduction?

OBJECTIVE: Closed skeletal traction (CST) to reduce unilateral locked facets in the subaxial cervical spine can expedite spinal realignment prior to definitive surgery but is not always successful. What predicts successful closed reduction is not completely understood. In addition, whether open anterior or posterior surgery is superior for achieving successful reduction after failed closed skeletal traction has not been investigated. The authors sought to assess predictors of successful closed reduction with skeletal traction and to compare the efficacy of anterior versus posterior surgery aft

Allogeneic Umbilical Cord Blood Serum Eyedrops for the Treatment of Severe Dry Eye Disease Patients.

Human allogeneic umbilical cord blood serum stands out as a potent adjunct to conventional therapies for ocular surface disorders related to severe Dry Eye Disease. By expediting ocular surface regeneration and fostering epithelial integrity, umbilical cord blood serum not only enhances subjective patient experiences but also improves objective clinical indicators. This makes it particularly useful in patients with corneal ulcers through ocular surface regeneration and anti-inflammatory activity. This retrospective, interventional, non-randomized clinical study aims to explore the efficacy of

Tropistry: A registry-based modular platform to improve care of neglected tropical diseases in nonendemic settings-Study protocol with two targeted conditions: acute schistosomiasis and cutaneous/mucocutaneous leishmaniasis.

BACKGROUND: Neglected tropical diseases (NTDs) are increasingly encountered in nonendemic settings due to migration, international travel, and global mobility. Their clinical management is often challenging: evidence from endemic regions is limited or not directly applicable, and healthcare providers are frequently unfamiliar with their diagnosis and treatment. Consequently, there is substantial heterogeneity in diagnostic workup, therapeutic choices, and follow-up strategies, which may affect patient outcomes and complicate guideline development. Given the sporadic presentation of these disea

Acute Fatty Liver in Pregnancy: Literature Review.

Acute fatty liver of pregnancy (AFLP) is a rare but potentially life-threatening obstetric emergency that typically arises in the third trimester. Although uncommon, its rapid onset, multisystem involvement, and historically high mortality make it an essential condition for clinicians to recognize and manage promptly. Its pathogenesis has been linked to defects in fatty acid metabolism and an abnormality in mitochondrial beta oxidation is a recognised cause of AFLP in a subset of cases. The clinical significance of AFLP lies in its dramatic course and overlap with other hepatic disorders of pr

"It gives me the strength and courage to take care of myself": a qualitative exploration of experiences with STI testing among women who initiated PrEP during pregnancy in Western Kenya.

INTRODUCTION: Sexually transmitted infections (STIs) in pregnancy contribute to poor perinatal outcomes and increased HIV acquisition risk, underscoring the importance of delivering STI/HIV services within antenatal care. Few studies evaluate women's perspectives on the co-delivery of antenatal STI testing and HIV pre-exposure prophylaxis (PrEP). We sought to understand motivations for and experiences with STI testing among pregnant women who initiated HIV PrEP. METHODS: We conducted semi-structured in-depth interviews (IDIs) among a subset of women enrolled in a randomized trial in Western K

Augmenting Telepostpartum Care With Vision-Based Detection of Breastfeeding-Related Conditions: Algorithm Development and Validation.

BACKGROUND: Breastfeeding benefits both the mother and infant and is a topic of attention in public health. After childbirth, untreated medical conditions or lack of support lead many mothers to discontinue breastfeeding. For instance, nipple damage and mastitis affect 80% and 20% of US mothers, respectively. Lactation consultants (LCs) help mothers with breastfeeding, providing in-person, remote, and hybrid lactation support. LCs guide, encourage, and find ways for mothers to have a better experience breastfeeding. Current telehealth services help mothers seek LCs for breastfeeding support, w

Genome-scale model of Rothia mucilaginosa predicts gene essentialities and reveals metabolic capabilities.

Cystic fibrosis (CF), an inherited genetic disorder caused by mutations in the cystic fibrosis transmembrane conductance regulator gene, results in sticky and thick mucosal fluids. This environment facilitates the colonization of various microorganisms, some of which can cause acute and chronic lung infections, while others may positively impact the disease. Rothia mucilaginosa, an oral commensal, is relatively abundant in the lungs of CF patients. Recent studies have unveiled its anti-inflammatory properties using in vitro three-dimensional lung epithelial cell cultures and in vivo mouse mode

Traumatic Aortic Disruption Index is Associated with Mortality and Urgency of Stent Grafting in Blunt Thoracic Aortic (Grade 3) Injuries.

BACKGROUND: Delayed stent grafting for blunt thoracic aortic injuries (BTAIs) is current standard of care. However, given the heterogeneity of pseudoaneurysm presentations, it is currently unclear which severe BTAIs require more urgent intervention. We hypothesize that a Traumatic Aortic Disruption Index (TADI) calculation based on sagittal computed tomography angiography imaging measurements would correlate with urgency of stent grafting. METHODS: All patients at a level-1 trauma center with BTAIs over a 12-year period were identified. A TADI score was then calculated using the length of pse

Development and validation of a nomogram for predicting heterotopic ossification following spinal cord injury.

OBJECTIVE: Heterotopic ossification (HO) following spinal cord injury (SCI) can severely compromise patient mobility and quality of life. Precise identification of SCI patients at an elevated risk for HO is crucial for implementing early clinical interventions. While the literature presents diverse correlations between HO onset and purported risk factors, the development of a predictive model to quantify these risks is likely to bolster preventive approaches. This study is designed to develop and validate a nomogram-based predictive model that estimates the likelihood of HO in SCI patients, ut
