Plate Fixation of Proximal Humerus Fractures: How to Get It Right and Future Directions for Improvement.

PURPOSE OF REVIEW: Open reduction and internal fixation with locking plates (ORIF-LP) has been used for decades for the surgical management of proximal humerus fractures. Despite good outcomes have been widely published in the literature, unacceptably high rates of complications (up to 40%), many of them yielding poor outcomes and requiring reoperation (up to 25%), have also been reported, especially in elderly patients. Most common complications are related to implant failure, with intra-articular screw penetration as the most frequent and devastating. RECENT FINDINGS: Advances in patient se

Predictors of wound leakage in patients undergoing surgery for neck of femur fractures.

AIMS: Persistent wound leakage following joint arthroplasty is a known risk for periprosthetic joint infection. Little is known of the predictors of wound leakage, particularly in patients with a fractured neck of femur. We aimed to determine patient and surgical risk factors for wound leakage in this cohort. MATERIALS AND METHODS: All patients undergoing surgery for a fractured neck of femur at Leicester Royal Infirmary between May and August 2017 were included. Patients were identified from a prospective database and placed into two groups: those with wound leakage later than three days pos

Intercalary prosthetic replacement is a reliable solution for metastatic humeral shaft fractures: retrospective, observational study of a single center series.

BACKGROUND: Treatments for metastatic fracture of the humeral shaft continue to evolve as advances are made in both oncological and operative management. The purposes of this study were to critically evaluate the effectiveness of intercalary endoprostheses in treating metastatic humeral shaft fractures and to clarify the surgical indications for this technique. METHODS: Sixty-three patients treated surgically for 66 metastatic fractures of the humerus shaft were retrospectively reviewed. Intramedullary nailing (IMN) was performed in 16 lesions, plate fixation (PF) in 33 lesions, and prostheti

Complications after intramedullary nailing of femoral fractures in a low-income country.

BACKGROUND: Some surgeons believe that internal fixation of fractures carries too high a risk of infection in low-income countries (LICs) to merit its use there. However, there have been too few studies from LICs with sufficient follow-up to support this belief. We first wanted to determine whether complete follow-up could be achieved in an LIC, and secondly, we wanted to find the true microbial infection rate at our hospital and to examine the influence of HIV infection and lack of follow-up on outcomes. PATIENTS AND METHODS: 137 patients with 141 femoral fractures that were treated with int

Intramedullary nailing of humeral diaphyseal fractures. Is distal locking really necessary?

PURPOSE: Distal interlocking is regarded as an inherent part of the antegrade humeral nailing technique, but it exposes both the patient and surgeon to radiation, is time consuming, and has a potential risk of damaging neurovascular structures. We have presented our technique of diaphyseal humeral nailing without any distal interlocking in this paper. MATERIALS AND METHODS: We have presented a series of 64 consecutive patients (33 male and 31 female, mean age: 41.5 years) with humeral shaft fractures treated with antegrade rigid intramedullary nailing without distal interlocking following a s

Sonographically assisted percutaneous removal of screws in dynamization of the interlocking intramedullary nail.

OBJECTIVES: Dynamization is a method of removing the interlocking screw(s) farthest from the fracture site for improving healing in femoral and tibial fractures that show delayed healing after static interlocking nailing. We describe a simple sonographically assisted technique for percutaneous dynamization of deep-seated impalpable screws. METHODS: Between March 2001 and March 2005, 20 dynamization procedures were completed using the developed technique under the diagnosis of delayed healing or a nonunion at a mean of 4.8 months after index surgery. After adequately positioning the involved l

[Application of intramedullary blocked osteosynthesis for treatment of patients with open fractures of the shin and femur in combined injuries].

Actual problems of using intramedullary blocked osteosynthesis for open fractures of the shinbones and femoral bones in patients with combined injuries are considered. The most optimal terms of operative treatment were established depending on the severity of the injuries by ISS scale and on classification of injuries of the soft tissues by Gastillo-Andersen. The schemes of antibacterial prophylactics of infectious complications are proposed both during operation and at the postoperative period. Under consideration are the questions of intraoperative boring of the bone marrow canal of the shin

[SURGICAL TECHNIQUE OF BLOCKING INTRAMEDULLARY OSTEOSYNTHESIS OF FOREARM FRACTURES].

An introduction of blocking osteosynthesis was recently included into practice of treatment of forearm fractures. The forearm has a very difficult biomechanics and extremely important functional value. There are really high requirements to accuracy of reposition and stability of forearm fracture fixation. The blocking osteosynthesis successfully combines fixation stability with functionality, although its performance technique isn't so simple, that forces experts to use other methods of fixation. It is possible successfully use advantages of the method by studying and keeping variety of operat

Percutaneous femoral derotational osteotomy for excessive femoral torsion.

Femoral derotational osteotomy is an acceptable treatment for excessive femoral torsion. The described procedure is a minimally invasive single-incision technique based on an intramedullary saw that enables an inside-out osteotomy, preserving the periosteum and biological activity in the local bone and soft tissue. After the osteotomy is complete and correction is achieved, an expandable intramedullary nail is used to achieve immediate stability, without the need for locking screws. Indications, tips, and pitfalls related to this novel osteotomy technique are discussed.

Extramedullary versus intramedullary tibial cutting guides in megaprosthetic total knee replacement.

BACKGROUND: In a standard total knee replacement, tibial component alignment is a key factor for the long term success of the surgery. The purpose of this study is to compare the accuracy of extramedullary and intramedullary tibial cutting guides used in indigenous and imported implants respectively, in positioning of the tibial components in megaprosthetic knee replacements. METHODS: A comparative study of the accuracy of extramedullary and intramedullary tibial cutting guides was carried out in 92 megaprosthetic knee replacements for distal femoral tumors. [Correction of heel strike in a hindfoot stump not adequate for weightbearing following Chopart amputation. Tibiocalcaneal arthrodesis and intramedullary nailing].

Chopart amputation leaves a very efficient hindfoot stump allowing full plantigrade weightbearing, even without an artificial limb in place for short distances. It must be said that this presupposes that the sole is covered with skin originally taken from the sole of the foot and that the tread of the heel is correct. Imbalance of the tendons frequently causes an increasing equinovarus deformity, which can lead to secondary problems. We report on a 28-year-old man with a stump in a fixed equinovarus deformity following traumatic Chopart amputation. Weightbearing was not possible, and constant

Outcomes of One-Stage Total Hip Arthroplasty With Postoperative Intra-Articular Antibiotic Infusion for Chronic Infection After Periarticular Internal Fixation Failure.

BACKGROUND: Chronic infection following periarticular internal fixation is challenging, with no standard treatment protocol. This study evaluated the results of one-stage total hip arthroplasty (THA) using intra-articular (IA) antibiotic infusions to treat this condition. METHODS: A retrospective analysis (2013 to 2023) included 26 patients who had chronic infection following periarticular internal fixation undergoing one-stage THA with IA antibiotic infusions. Initial internal fixation involved six cases of plates and screws, 11 cannulated compression screws, and nine intramedullary nails. I

Compression treatment of long bone pseudarthroses.

In 122 cases of non-union and delayed union, rigid fixation with Swiss compression plates without the use of casts, produced 92.6% success with the first operation. Half of the cases, especially in the well aligned fractures with hypertrophic callus, were treated without bone grafts and without resecting the pseudarthrosis. Non-resection did not adversely affect the healing time or rate. There were 9 failures or 7.4% chiefly in the difficult hypovascular atrophic bone-grafted group. Many of these cases were treated with shingling and grafts as well; 4 cases showed signs of healing but refractu

Evaluation of results of interlocking nails in femur fractures due to high velocity gunshot injuries.

BACKGROUND: Femoral shaft fractures are common in adult population due to vulnerability to road traffic accident and firearm injuries. There are various treatment modalities to treat the femur shaft fracture, i.e., Plating & screws, Intramedullary nailing, External fixator and Interlocking nails. Comminuted fractures due to gun shot injuries are a challenging problem for orthopaedic surgeons. The objective of this study is to evaluate the role of interlocking nailing in the management of femoral fractures due to high velocity gunshot injuries. METHODS: This descriptive study was conducted on

Distalization with Motorized Intramedullary Nail for High Dislocation of the Hip Before Arthroplasty: A Case Report.

CASE: We present 3 patients (a 19-year-old man, a 37-year-old woman, and a 66-year-old woman) with chronic high hip dislocation and severe limb length discrepancy, managed with gradual soft tissue distraction using a motorized intramedullary nail and a temporary cup before total hip arthroplasty. All patients had at least 1 year of follow-up. Complications included transient foot drop (resolved), and 1 case of early infection and persistent foot drop. CONCLUSION: This approach facilitates near-anatomical cup positioning without osteotomy but still carries risks. Further refinement, including

In vivo study to assess fat embolism resulting from the Reamer-Irrigator-Aspirator 2 system compared to a novel aspirator-based concept for intramedullary bone graft harvesting.

INTRODUCTION: Fat embolism (FE) following intramedullary (IM) reaming can cause severe pulmonary complications and sudden death. Recently, a new harvesting concept was introduced in which a novel aspirator is used first for bone marrow (BM) aspiration and then for subsequent aspiration of morselized endosteal bone during sequential reaming (A + R + A). In contrast to the established Reamer-Irrigator-Aspirator (RIA) 2 system, the new A + R + A concept allows for the evacuation of fatty BM prior to reaming. In this study, we hypothesized that the risk of FE, associated coagulopathic reactions an
